What is 10 units of Mounjaro?

Ten units of Mounjaro is not a dose that exists on a UK prescription. Mounjaro is the brand name for tirzepatide, a once-weekly injection licensed in the UK for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

"Units" are the markings on an insulin-type syringe. They measure the volume of liquid drawn up, not the amount of medicine. How much tirzepatide sits in that volume depends entirely on how concentrated the liquid is, and that varies between products. A number of units therefore tells you nothing about milligrams unless you know the exact concentration of what is in the vial.

03 · Units versus mg versus ml

Units, mg and ml: three different measures

Milligrams measure the weight of the medicine itself. Millilitres measure the volume of liquid. Units, on a syringe, are simply graduation marks for volume. Only milligrams tell you how much tirzepatide you are getting, which is why every Mounjaro strength is named in mg.1

This is why questions such as "how many units is 10 mg of tirzepatide?" or "how many ml are in one pen?" have no single answer you can safely use from a web page. A conversion only works for one specific product at one specific concentration. Change the product and the answer changes.

04 · Why units confusion happens

Why do people talk about units at all?

Units come from insulin. Insulin has long been drawn up with syringes marked in units, so many people with diabetes and many online communities think in that language. When tirzepatide became popular, advice written for syringe users spread widely.

Unlicensed vials sold outside regulated supply are the other source. Those products are drawn up with a syringe, so sellers quote units. They are not the licensed KwikPen product, and their contents cannot be assumed to match the label. A licensed pen avoids the whole problem: the strength is printed on it and the device delivers the dose.1

05 · 10 units versus 10mg

10 units versus the 10mg pen

The 10mg KwikPen is a real, licensed strength, and it is probably what many people mean when they say "10". It is one of six: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Ten units on a syringe is a volume marking with no fixed link to that strength.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 The figures are tied to milligram strengths, which is another reason to talk in mg.

06 · Is it enough?

Is "10 units" enough?

Whether a dose is enough cannot be judged in units, because units do not identify a dose. The question that matters is whether your prescribed strength suits you, and that is decided by your prescriber, not by a number from the internet.

Treatment is stepped up in strengths from the starting pen, and the maintenance strength differs from person to person. Our prescribing team reviews you as you go.

Do not adjust, stretch or split a pen to reach a number you have read about. Doses should follow your prescription and the leaflet.13 If you feel your current dose is not working, tell your prescriber so they can review it safely.

07 · Unlicensed vial risks

The risks of unit-based vials

Buying vials dosed by units carries risks that a regulated pharmacy supply avoids. You cannot verify what is in the vial, how concentrated it is, or how it was stored. A small error in drawing up can mean a very different dose from the one intended.

The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting suspected problems.7 If you have had a side effect or suspect a fake product, you can report it through the Yellow Card scheme.19 Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

How tirzepatide works

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.

Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber, and used alongside a reduced-calorie diet and increased physical activity. Individual results vary.

09 · NHS and private access

NHS and private routes

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.6

10 · Related unit questions

The same confusion appears with other numbers. People ask about 30 units of Mounjaro, Mounjaro 25 units or 10 ml of Mounjaro, and the answer is the same each time: the licensed pen is dosed in mg, so these figures do not map to a UK dose.

Likewise, a question like what is 45 units of tirzepatide in mg cannot be answered reliably without knowing a vial's concentration. Even then, a conversion is not a prescription. Use it only to understand a product, never to set your own dose.1

Is 2.5mg of Mounjaro equal to a set number of units?
