The licensed ladder starts low for tolerability. What you should not do is try to bring the effect forward yourself. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

If you miss a dose, take it as soon as you can within four days (96 hours) of the scheduled time. If more than four days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within three days of each other.1

Mounjaro reduces appetite by acting on two natural gut-hormone receptors at once. Tirzepatide is the first treatment of its kind to activate both GIP and GLP-1 receptors. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 The medicine is a support for eating less, not a replacement for the dietary changes that make weight loss happen. It is used alongside a reduced-calorie diet and increased physical activity.

If you eat very large portions or very rich food while your body is adjusting, you may notice that fullness arrives uncomfortably. Eating more slowly and stopping when you feel satisfied is sensible practical advice, and your prescriber or pharmacist can talk it through with you.

Why does it take time to feel the full effect? Tirzepatide has a half-life of about five days. After a final dose it takes roughly four to five weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1 Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and a regulated UK pharmacy will not supply Ozempic for weight loss.

Before drawing conclusions, run through a few practical checks.

  • How long have you been on the current dose? The dose steps up no sooner than after at least four weeks, so a few weeks on the starting strength is entirely expected.
  • Are you injecting on the same day each week? What matters is consistency.
  • Has the pen been stored correctly? Keep pens in a fridge at 2–8°C and never freeze them. Never use a pen that has been frozen.1
  • Has your food intake drifted? Portions creep back up without anyone noticing, which can mask the effect of the medicine.
  • Basics such as sleep and alcohol. These are part of the standard review when progress slows.1

If everything looks right and the effect is still slight, speak to your prescriber. The licensed route is to discuss whether a dose step-up is appropriate if you are not yet at the maximum.1 What is not appropriate is doubling doses, taking extra injections or adding unlicensed products.

There is no best time of day to inject Mounjaro. It works the same morning or evening, with or without food. What matters is the same day each week.1 Pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off, and that is a practical choice rather than a clinical rule.

It is injected under the skin (subcutaneously) once a week. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.1

The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window. Follow the pen's instructions for the injection itself.

Need to move your day? You can change your weekly injection day if needed, as long as at least three days (72 hours) have passed since your last dose.1 This is useful if your schedule changes or if you want side effects to land on a quieter day.

Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use — useful for short trips — and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen. Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. A sensible expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. Slower early progress does not predict failure — the dose you respond to may still be ahead of you.1

Plateaus are normal too. Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1 Within the licence, the responses are to review food intake honestly, protect protein and activity, check sleep and alcohol, and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.   • Watch your portions over time. Portions drift back up, so check in honestly.

There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and emptier stomach can hit harder than usual, and alcohol adds calories. If you are unsure how alcohol interacts with your own health or other medicines, ask your prescriber or pharmacist rather than guessing. If you take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar, and doses may need adjusting, so tell your prescriber about every medicine you take.

In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises.2

Mounjaro is available on the NHS, but access is being phased in over several years.
