04 · Trial results by dose

How much weight can you lose on Mounjaro?

The largest dataset comes from the SURMOUNT-1 trial. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 The pattern matters as much as the numbers: every dose beat lifestyle change alone by a wide margin.

Averages only tell part of the story, so the trial also reported how many people reached particular milestones. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

05 · Mounjaro versus Wegovy

Is Mounjaro more effective than Wegovy?

In the first large head-to-head trial, tirzepatide produced a larger average loss than Wegovy, though both are effective, licensed treatments. In the first large head-to-head trial (SURMOUNT-5), adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy).15

The same trial reported milestones. In SURMOUNT-5, 751 adults took tirzepatide or semaglutide 2.4mg for 72 weeks. At least 10% of body weight was lost by 81.6% on tirzepatide vs 60.5% on semaglutide; at least 15% by 64.6% vs 40.1%; at least 20% by 48.4% vs 27.3%; at least 25% by 31.6% vs 16.1%. Average loss was 22.8 kg vs 15.0 kg.15

Wegovy is far from ineffective. In the STEP 1 trial, adults on semaglutide 2.4mg (Wegovy) lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo.9 That is a separate trial, so it should not be set beside other trials as a direct comparison.

06 · Why it works

Why is Mounjaro so effective?

Mounjaro acts on appetite through a dual mechanism that single-action medicines lack. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. That is the key point: the medicine makes eating less easier, but it does not do the work on its own. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).

Mounjaro is a once-weekly injectable prescription medicine made by Eli Lilly, and it comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg. Because the strengths scale up, the dose can be matched to how you respond and tolerate treatment.

07 · How quickly it works

Is Mounjaro effective immediately?

No. Mounjaro is not an instant treatment, and the headline trial results were measured over 72 weeks. Treatment starts on the 2.5mg pen and the dose is raised gradually under prescriber review, so the larger effects build with time and dose.2

Early weeks are largely about your body adjusting and your prescriber checking that you tolerate each step.

The trial reported averages at the end of the study, and individual progress is uneven: some weeks show little change and others show more. A plateau for a short spell does not, on its own, mean the medicine has stopped working.

08 · If it is not working

What if Mounjaro is not effective for me?

If results are slower than you hoped, the first step is to speak to your prescriber, not to change the medicine yourself. Trial averages do not predict your own result.

It also helps to be realistic. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Some people worry that the effect fades over time. We cover that in detail in our guides on whether Mounjaro becomes less effective and whether it gets less effective, and on whether the starting dose is effective for weight loss.

What not to do matters just as much. Mounjaro must not be combined with other GLP-1 medicines, unlicensed "research" peptides (such as retatrutide, which has no UK licence), or compounded copies.1 These combinations have no safety data and no regulated supply chain. If something promises faster results outside a licensed pathway, that is the warning sign.

If you are switching from Wegovy or Ozempic because results stalled, there is no licensed direct conversion between semaglutide and tirzepatide. Switching is done under prescriber supervision, and you should never run two such medicines together. Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label.

09 · Pills, tablets and supplements

Do weight loss pills work, and is there a Mounjaro pill?

There is no tablet form of Mounjaro — tirzepatide is injection-only in the UK. Two weight-loss tablets are now licensed in the UK: Wegovy tablets (oral semaglutide, made by Novo Nordisk), approved by the MHRA in June 2026, and Foundayo (orforglipron, made by Eli Lilly), approved in August 2026. They are different medicines from Mounjaro, with their own dosing, eligibility checks and trial results, and both are prescription-only.

Other licensed options work differently: orlistat (tablet) blocks some dietary fat absorption and produces smaller average losses, and Wegovy and Saxenda (liraglutide) are other licensed injectables. Saxenda is a daily injection, unlike once-weekly Mounjaro and Wegovy.

"Natural" supplements (such as berberine) are not medicines, are not regulated for this use and have no comparable trial evidence. They are available cheaply without assessment, and dosing is not standardised. Dramatic weight-loss claims on such products should be treated with suspicion.

The contraceptive pill and Mounjaro

A separate question is how well the contraceptive pill works while you are on Mounjaro. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 This applies to the pill only — implants, coils and injections are unaffected.7

Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.1

10 · Access and genuine pens

Getting effective treatment safely

Mounjaro is available on the NHS, but access is being phased in over several years.6 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.5

Whatever route you take, a fake pen cannot be effective and may be dangerous. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations — some contained insulin rather than tirzepatide.7 Only buy from a pharmacy you can verify on the GPhC register (EverydayMeds is GPhC 9012878), never from social media, beauty salons or unverified websites.

11 · Stopping treatment

What happens when you stop Mounjaro?

Stopping is a decision to make with your prescriber, and the medicine leaves the body slowly. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.

Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 That is why the approach to eating and activity you build while on treatment matters for what comes next. If you are considering stopping because of side effects, cost or a plateau, tell your prescriber first; there may be options short of stopping.

Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load. Treat any medicine as a tool that supports habits you can keep, and review the plan regularly rather than waiting for problems.

12 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
