Does it wear off between injections?

Tirzepatide has a half-life of about five days, so the amount in your body rises after an injection and falls gradually until the next one.1 That is how a once-weekly medicine works.

04 · Dose and trial results

What the trials say about dose

SURMOUNT-1 enrolled 2,539 adults. Over 72 weeks, people on tirzepatide lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg, compared with 3.1% with lifestyle changes alone.2

Looking at how many people reached each threshold at 72 weeks: 85% on 5mg, 89% on 10mg and 91% on 15mg lost at least 5% of body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.2 As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. Trial averages do not predict your own result.

05 · Staying on 2.5mg

Can you stay on 2.5mg and still lose weight?

Everyone starts Mounjaro on 2.5mg once a week, and this starting dose is about letting your body adjust, not about weight loss itself; if you are on a low dose and feel the effect is modest, ask for a review.

06 · Injection site

Does injection site matter for how well Mounjaro works?

A change in skin sensation is listed as an uncommon side effect in the UK patient leaflet (up to 1 in 100 people). Tell your prescriber about any new or lasting symptom.

Mounjaro is injected under the skin once a week; the licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection, and you should rotate the site with each dose.13 If you think a dose did not work properly, for example a pen that seemed faulty, contact the pharmacy promptly. Faulty, damaged or wrong items must be reported within 3 days. You can also report suspected problems through the Yellow Card scheme.19

07 · The contraceptive pill

Does Mounjaro make the pill less effective?

Yes, it can. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.1

The SmPC advises switching to a non-oral method, or adding a barrier method such as condoms, for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. This applies to the pill only; implants, coils and injections are unaffected.17

Mounjaro must not be used in pregnancy and should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.1

08 · Stopping and restarting

Stopping, restarting and the second time

After a final dose, tirzepatide takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

Restarting after a break is a prescriber decision based on your history.

09 · What not to add

Things not to add if progress slows

Do not combine Mounjaro with other GLP-1 medicines, unlicensed research peptides such as retatrutide (which has no UK licence) or compounded copies. These combinations have no safety data and no regulated supply chain.1 If something promises faster results outside a licensed pathway, that is the warning sign.

Likewise, "natural GLP-1" supplements such as berberine are not medicines and have no comparable trial evidence. Dosing is not standardised, and they are not a substitute for a review.
