Slower stomach emptying matters for medicines taken by mouth. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you use an oral contraceptive, raise this with the prescriber during your consultation so you can agree a plan.7

Similarly, if you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar. That is a conversation for your prescriber or diabetes team, not something to decide alone. It is not a reason the medicines cannot be used together; it is a reason to plan the doses.

For someone using the medicine for weight management, the insulin effect is part of the overall picture rather than the headline. The headline is appetite and fullness.

NICE has separate guidance for the two uses. The tirzepatide guidance for diabetes sits apart from the guidance on overweight and obesity, so NHS eligibility depends on which condition is being treated.48 If you have diabetes, your diabetes team is the right first contact, because your other medicines may need review.

The same applies if you take other glucose-lowering treatment. Doses may need adjusting because of low blood sugar risk, and the prescriber will ask about all your medicines during the consultation. Always list everything you take, including over-the-counter products.

What the trial evidence shows

SURMOUNT-1 is the main tirzepatide weight-loss trial. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

Note what the comparison group tells you. The lifestyle-only group also lost some weight, which shows that diet and activity matter in their own right. The medicine added to that effort. It did not replace it.

SURMOUNT-5 is the first large head-to-head trial of tirzepatide and semaglutide for weight loss.15 Semaglutide's own trial, STEP 1, stands on its own evidence.9

For a rough sense of scale only, a worked example: for someone starting at 100 kg, 20.9% is about 21 kg. That is a trial average at the highest strength over 72 weeks, not a forecast. Many people will lose less, some more.

07 · Mounjaro versus single-action

Dual action versus single-action GLP-1 medicines

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1

That is a statement about mechanism, not a verdict on which is better for you. Both are effective licensed treatments. Wegovy comes as pens of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17.18 Mounjaro comes as a pre-filled multi-dose KwikPen in strengths of 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

How is liraglutide different? Liraglutide (Saxenda, and products such as Erly) is supplied at a concentration of 6mg per millilitre, and it is a daily injection, unlike once-weekly Mounjaro and Wegovy.

Why mention Ozempic? Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. Wegovy (semaglutide 2.4mg) is licensed for weight management.

08 · Safety and who should not

Downsides, long-term effects and who should not take it

Every medicine has downsides, and GLP-1 medicines are no exception. Most side effects are digestive: the most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. They are most likely when starting or moving up a dose and for most people ease within days to a few weeks.113 Read the patient leaflet before you start, and if you notice anything new, speak to your prescriber and report it through the Yellow Card scheme.19

Regulators monitor these medicines. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link between GLP-1 medicines and suicidal thoughts, and the MHRA keeps this under continuous review.17 If you have concerns about mood, tell your prescriber.

Who should not take Mounjaro? You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. Switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting and for 4 weeks after each dose increase. If you use insulin or sulfonylureas, doses may need adjusting because of low blood sugar risk.7

Storage matters once the pen arrives. Store pens in the fridge at 2–8°C until first use, and they keep until the printed expiry date. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
