GLP-1 is a hormone made in the gut. When it reaches the receptor, the receptor responds and sets off signals inside the cell. Medicines called GLP-1 receptor agonists are built to do the same job. An agonist is simply something that activates a receptor.

A GPCR works like a lock in the cell wall. The hormone is the key. When the key fits, the lock changes shape and starts a chain of signals inside the cell.

Its natural key, GLP-1, is a hormone released by the gut.

How GLP-1 medicines use the receptor

Tirzepatide, the active ingredient in Mounjaro, is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1

Mounjaro is a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in strengths from 2.5mg to 15mg.1

05 · Ozempic and GLP-1

Ozempic, semaglutide and GLP-1

GLP-1 is the natural hormone. Ozempic is semaglutide licensed for type 2 diabetes only. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. Semaglutide acts on the GLP-1 receptor, but Ozempic is a medicine, not the hormone itself.

06 · Evidence and limits

What the evidence does and does not tell you

Receptor science explains why these medicines work. Whether they work for you is shown by clinical trials. In the SURMOUNT-1 trial of tirzepatide, average weight loss rose with dose over 72 weeks, compared with lifestyle changes alone.2

These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result.

Receptor activity in a laboratory is also not the same as a result in a person.

07 · Supplements and shortcuts

Supplements marketed as natural GLP-1

Products sold as the "poor man's Ozempic" or "nature's Ozempic" are low-cost supplements with no clinical oversight. They are not licensed medicines. Dramatic weight-loss claims made for them are not backed by the evidence behind licensed treatments.

Be careful with anyone offering to supply a GLP-1 medicine without a consultation or prescription. Counterfeit medicines are a real risk, and the MHRA publishes alerts on them.7

08 · Safety and reporting

Safety, side effects and who should avoid GLP-1 medicines

Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation. If you notice a new symptom, tell your prescriber and report it through the Yellow Card scheme.19

Body odour is not on the list of side effects in the UK Mounjaro patient leaflet. Tell your prescriber about any new or lasting symptom.

09 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
