Water weight or fat loss: what Mounjaro actually does

Mounjaro does not get rid of water weight as its job; it reduces appetite and helps the body handle insulin. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin.1 Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

Nothing in that description is about flushing fluid.

04 · How it works

How Mounjaro works and why it is not a diuretic

Mounjaro is the brand name for tirzepatide, a once-weekly injection made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes in a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Its distinguishing feature is dual action on GIP and GLP-1 receptors. Together these help regulate appetite, slow stomach emptying and improve the way the body responds to insulin.1 That is a different route from a diuretic, which is a medicine taken to increase urine output.

Slower stomach emptying does explain one thing people notice: large portions sit heavily, so smaller, slower meals work better.1 Eating less overall is the main driver of what happens on the scale.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system.1 Nothing speeds that up, and nothing needs doing while it clears.

05 · What trials show

What the trials say about total weight loss

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

As a worked example only: for someone starting at 100 kg, 15% is about 15 kg and 20.9% is about 21 kg. Trial averages do not predict your own result.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.2

Notice what these numbers are: totals over 72 weeks. They are not weekly rates.

06 · Protect muscle

Protecting muscle so the loss is the right kind

Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance.1 The two levers are adequate protein and resistance exercise two or more times a week.1

Aim to include a protein source at every meal. That is harder when appetite is low, and it matters more, not less, as the dose rises and appetite falls.1 Prioritise protein, vegetables and fibre, and regular fluids; no food is banned, including bananas.1

Regular moderate activity such as brisk walking plus resistance work is the evidence-backed pattern, and on injection days with early side effects, lighter movement still counts.1

07 · Hydration

Hydration: why drinking more is the opposite of a problem

A common worry is that drinking more will "hold on to water". Drinking enough matters more on Mounjaro than usual. Reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly, which is the main route to the rare kidney problems reported with this class.13

Keep water within reach through the day. If you cannot keep fluids down, treat that as a contact-your-clinical-team issue, not something to push through.13

Coffee and tea are fine, as there is no interaction with caffeine. Strong coffee on an empty, slower-emptying stomach can aggravate nausea or reflux for some people, and high-calorie coffee drinks are an easy place for the calorie deficit to leak.13

Alcohol has no direct interaction with Mounjaro, but it can hit harder on a reduced appetite and emptier stomach, and it adds calories.13

If you notice swelling or any new symptom, tell your prescriber. If the swelling affects your face, lips, tongue or throat, or you have difficulty breathing, get medical help straight away. Report suspected side effects through the MHRA Yellow Card scheme.19 Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

08 · Quick fixes

What about "quick" water-weight fixes?

Fast scale drops from dehydration are not progress. Low-cost supplements marketed as "natural GLP-1 boosters" are not equivalents and are not substitutes for prescribed treatment, and no supplement is required with Mounjaro.1 Dosing of such products is not standardised, and they come with no clinical oversight.

A standard daily multivitamin is a reasonable safety net while eating much less. Discuss anything beyond that with a pharmacist.1 Oral supplements, like other oral products, may be absorbed more slowly when you start treatment.1

Dramatic weight-loss claims in adverts deserve scepticism. Reading the scale as a trend, eating enough protein and drinking regularly will tell you more than any quick fix.

09 · Belly fat and inflammation

Belly fat, puffiness and inflammation

Results vary, and treatment works alongside a reduced-calorie diet and increased physical activity.2

Waistlines can lag behind the scale for many reasons, including muscle changes, which is another reason to protect lean tissue. Measuring waist and noting how clothes fit give you more than weight alone.

10 · Who can get it

Who Mounjaro is licensed for

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable.1

It is also licensed for type 2 diabetes, but it is not a cure. Our guide asks whether Mounjaro can get rid of diabetes.

On the NHS, Mounjaro is available but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment.
