Planning the cost over a year

A monthly price understates what treatment costs because Mounjaro is stepped up gradually. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £879.96 at standard prices. A first year of 52 weeks and 13 pens costs £2,399.88 if you settle at 5mg, up to £3,279.88 if you settle at 15mg.

These totals are before discounts. A new customer's first order is £40 less, and every later order is £25 less. If you settle at 7.5mg, 10mg or 12.5mg, work from that dose's monthly price: £244.99, £274.99 or £289.99.

Not everyone needs the top dose. The dose that suits you is decided with your prescriber, not by the price list, which is why a low starting price tells you little about your long-run spend.

To compare chemists fairly, check the same dose, the same quantity and the full cost at the checkout. A headline starting price can look low while later doses cost far more, so look at the whole ladder from 2.5mg to your likely maintenance dose.

Use this checklist when you price up any pharmacy, high street or online:

• Dose and pen size. One pen is four weekly doses. Make sure you are comparing a full pen, not a part-supply.

An unusually cheap pen is a warning sign, not a bargain. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified.⁷ Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷

Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.¹² Anyone offering to supply without a consultation or prescription is not operating legitimately.

06 · NHS and diabetes access

The NHS route and who can use it

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Whether Mounjaro will be free for you on the NHS depends on where you sit in that phased rollout, so ask your GP which group you fall into.

People with type 2 diabetes ask whether they can get it on the NHS. Mounjaro is licensed in the UK for type 2 diabetes as well as weight management,¹ and NICE has separate guidance on its use in diabetes.⁸ Your GP or diabetes team decides whether it suits your treatment plan.

If you use the NHS for other prescriptions, the standard charge in England is £9.90 per item. It does not apply to private weight-management supply.

07 · Who can be prescribed

Eligibility and what to expect

Price matters less if the medicine is not right for you, so check eligibility first. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.¹ It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber assesses your BMI, medical history and current medicines before deciding.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² For someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages, individual results vary, and treatment works alongside diet and activity, not instead of them.

Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.¹

08 · Medicines and diet

What to avoid, and what not to combine

The main interaction points to raise with your prescriber involve oral contraceptives and diabetes medicines. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.¹ If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar; the two are not banned together.¹³

Your consultation reviews every medicine you take, including those bought over the counter, so list them all.

On diet, Mounjaro is licensed for use alongside a reduced-calorie diet and increased physical activity.¹ Your prescribing team supports you with diet in monthly reviews. Report any suspected side effect to the Yellow Card scheme.¹⁹ Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
