If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £879.96 at standard prices. What is Mounjaro and how does it work?

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, called a KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹ Each pen is four weekly doses.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide, sold as Wegovy and Ozempic. That is a difference in mechanism. It is not a statement about which medicine suits an individual, which is a decision for you and your prescriber.

Why the strengths matter for cost

Because there are six strengths, the cost of Mounjaro is not one number. Most people begin at 2.5mg, then move up through the strengths with at least 4 weeks on each dose, depending on how they respond.¹ That means your monthly cost can change as your prescribed dose changes, and a provider that is inexpensive at the starting dose may not be at higher doses. How long it stays in the body

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.¹

05 · Who can be prescribed it

Who can be prescribed Mounjaro?

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹ A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.

That assessment is the reason price should never be your first filter. A pharmacy that offers a very low price but does not carry out a proper clinical assessment is not a legitimate UK pharmacy, whatever the number on the page. Genuine treatment always involves a prescription and a clinical consultation.

NHS or private?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 and above with four or more qualifying conditions. Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.⁴⁶

06 · How effective it is

How effective is Mounjaro for weight loss?

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.²

What the percentages mean in kilograms

As a worked example only, for someone starting at 100 kg, 15% is about 15 kg, 19.5% is about 19.5 kg and 20.9% is about 21 kg. Those figures are simple arithmetic on the trial averages. They are not a prediction for you, and trial averages do not predict your own result. Your starting weight, dose, diet, activity and how you tolerate the medicine all play a part.

Why this matters when you compare prices

Your prescriber decides your dose, and a higher dose is not automatically better. Choosing a provider on price alone, or trying to start at a higher dose than your prescriber recommends, is not a safe way to approach treatment.

It is also worth being sceptical of any seller that promises specific results or uses dramatic weight-loss claims to sell a low-priced product. If a provider quotes a number with no source, treat that as a warning sign.

Many people also want to know what happens after they stop. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.¹⁴ Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

07 · Side effects and safety

What are the side effects of Mounjaro?

Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Common effects, up to 1 in 10, include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions.¹³ Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.¹

Serious side effects: when to seek help

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for:

• severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis);
• pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems);
• signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing.

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion). Your prescriber may need to adjust those medicines.¹

Contraception

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The product information advises switching to a non-oral method, or adding a barrier method such as condoms, for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. This applies to the pill only. Implants, coils and injections are unaffected.¹⁷ If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive.

Reporting a side effect

The safest way to avoid fake Mounjaro is to buy only from a pharmacy you can verify on the GPhC register. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷ Never buy from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.

Where genuine stock comes from

Mounjaro is manufactured by Eli Lilly. Genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies, and every pen carries a batch number and expiry date that can be checked.⁷ Our supply is through official LillyDirect, direct from Eli Lilly's UK distribution. Red flags that a low price is too good to be true

• The seller offers to supply without a consultation or prescription.
• The listing is on a social media account, a marketplace or a salon rather than a registered pharmacy.
• There is no way to find the seller on the GPhC register.

• The seller promises specific results or uses dramatic weight-loss claims.

Checking a pen when it arrives

A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been in use for more than 30 days. How to store your Mounjaro pen

Keep Mounjaro pens in a fridge at 2 to 8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, which is useful for short trips, and must then be thrown away. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.¹

Why storage affects what you spend

Most of the avoidable mistakes are simple: leaving a pen in a hot car, a bag in direct sun, or too close to the freezer compartment of the fridge. If you travel, plan how you will keep the pen not above 30°C and make sure it is used within the 30 days that apply once it is in use.

For unopened pens, store them in the fridge until first use, and they keep until the printed expiry date. If you are unsure about a pen that has been out of the fridge before you started using it, read the patient information leaflet and contact the pharmacy before injecting.¹³

What a KwikPen is designed to do

Each KwikPen is a multi-dose pen intended for four weekly doses at the strength printed on it. Use it as designed, then dispose of it safely in your sharps bin. Pen instructions for preparing and injecting each dose come in the box, so follow the pen's instructions each time rather than relying on memory.

Doses go up gradually

Most people start at 2.5mg and move up through the strengths, with at least 4 weeks on each dose, depending on how they respond. Your prescriber decides when to increase, and the ladder runs from 2.5mg up to 15mg, so you should not assume it stops at any particular strength.¹ Side effects are most likely when starting or moving up a dose, so take increases at the pace your prescriber sets.

Travelling with a pen

Pack the pen in its carton with the cap on and keep it cool. If you fly, keep it in your hand luggage rather than the hold, where temperatures can fall low enough to freeze it. Mounjaro versus Wegovy, tablets and other options

Mounjaro and Wegovy are both licensed once-weekly injections for weight management, and which suits you is a decision for you and your prescriber. Mounjaro contains tirzepatide, which activates GIP and GLP-1 receptors. Wegovy contains semaglutide, a single-action GLP-1 medicine.¹

Wegovy dose ladder

Wegovy pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly. The dose steps up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, so the 2.4mg maintenance dose starts from week 17. Wegovy tablets

Wegovy is also available as a tablet. Wegovy tablets are taken on an empty stomach after fasting for at least 8 hours, swallowed whole with a sip of water (up to half a glass, about 120ml). Daily injections and orlistat

Liraglutide, sold as Saxenda and in products such as Erly, is supplied at 6mg per millilitre, and it is a daily injection, unlike once-weekly Mounjaro and Wegovy. Comparing trial results sensibly

In SURMOUNT-1, people taking tirzepatide lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks.² Results from different trials should not be set side by side as if the medicines had been compared directly. The first large head-to-head trial of tirzepatide and semaglutide for weight loss is SURMOUNT-5, where adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy).¹⁵
