Mounjaro is a once-weekly injectable prescription medicine licensed in the UK for weight management in adults, and the dose ladder is built around reaching a maintenance dose rather than reaching a finish line.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.1
That is different from saying you must stay on it forever. Nobody can honestly tell you the exact length of treatment that will suit you, because it depends on how you respond, how you tolerate the medicine, your health, and your own goals.

05 · What the licence says
What the licence and dose ladder say about long-term use
The licence describes what Mounjaro is for and how to dose it; it does not set a countdown. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly, and in the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
The dose ladder is the clearest guide to how treatment is designed. Everyone starts on 2.5mg once a week. After at least four weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of four-week intervals, up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 The ladder therefore leads somewhere: a maintenance dose that suits you. Many people settle at 5mg, 10mg or 15mg, and your prescriber guides the right pace.1
This structure tells you something useful about the forever question. The treatment is built to get you to a dose you can sustain, not to run for a fixed course and then stop automatically. That is why people who are responding well and tolerating treatment often continue. But sustaining a dose is not the same as being obliged to sustain it. What happens in your body if you stop
If you stop, the medicine clears gradually and nothing needs doing while it does. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1
That gradual fade is worth understanding because it shapes what you notice. As the weeks pass and the level in your body falls, the appetite support winds down. Because the way the medicine works is to help regulate appetite and slow how quickly the stomach empties, it is reasonable to expect that fullness after meals and the gap between hunger pangs will gradually return towards how they were before treatment.1
Do you need to taper?
Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load; do not improvise your own smaller or spread-out doses.
Planning the weeks after your last dose
Because the change is gradual, you have time to prepare. Practical steps that make sense for most people include:
- Telling your prescriber before your final dose, so they can discuss the plan and any follow-up.
- Keeping regular meals and a routine you can sustain, rather than waiting until appetite returns to think about food.
- Keeping up the physical activity you built during treatment, since treatment is designed to work alongside diet and activity, not instead of them.
- Weighing yourself on a regular schedule and judging the trend, not single readings.
- Knowing what you will do if weight starts to drift upwards, including who you will contact and whether restarting is an option you would consider.
Side effects while it clears
The common early effects on starting are mild nausea, altered appetite and tiredness.1 If you feel unwell after stopping, or notice any new symptom, speak to your prescriber or GP, and you can report suspected side effects through the Yellow Card scheme.7
Finally, keep used pens and sharps disposal in mind. Needles, a sharps bin and injection guidance come with every order, and any pen you have already started should be thrown away after 30 days if kept out of the fridge not above 30°C, even if you decide to stop part-way through.

In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 Weight loss with Mounjaro is the result of the medicine working alongside a reduced-calorie diet and increased physical activity.2 When the medicine is no longer in your system, you lose the appetite support it provided, so keeping the weight off depends more heavily on the habits you have built.
Why people ask about "food noise"
People often describe the quiet in their appetite on treatment, and the return of hunger when it ends. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1, which together help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 Remove the medicine and those effects fade with it.
Is it normal to gain weight after stopping Mounjaro?
Some gain after stopping is possible, so plan for it rather than be surprised by it. It is not a sign that you did anything wrong. Early on, some movement on the scales can also be water, which is why the trend over weeks matters more than any single weigh-in.1
What improves your odds
No approach is certain, but the following support keeping weight stable, and all fit with how treatment is meant to be used:

Reviews are how the question of forever gets answered in practice, one step at a time. Rather than deciding years in advance, you and your prescriber check response periodically and adjust. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.
Notice what that says. It is a conversation, not an automatic cut-off. It also applies to the highest tolerated dose, which acknowledges that not everyone can or should climb to the top of the ladder.
What a sensible review covers
When you come to a review, whether with us or elsewhere, these are the topics that matter most:
- Progress by trend. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 Trials measured results at 72 weeks, so think in months, not days.
- Tolerance. Are side effects settled, or are they affecting your daily life?
- Dose. Is the current dose the right one to maintain, or would a lower one hold your progress?

Why higher is not automatically better
It is tempting to assume the aim is always the top dose. The dose ladder says otherwise: many people settle at 5mg, 10mg or 15mg, and higher is not automatically better.1 A person who does well on a middle dose and tolerates it comfortably may have a more sustainable long-term plan than someone pushing to the maximum.

09 · Lower doses and microdosing
Staying on a lower dose: what is and is not appropriate
Stepping down within the licence is appropriate; making up your own smaller doses is not. This is one of the most misunderstood parts of the long-term question, so it is worth being clear.
Some people hope to "stay on forever" more cheaply or gently by taking less than the pen is designed to deliver. Altering doses outside the licence means unknown dosing accuracy and no safety data.1
The licensed way to go lower
Within the licence, your prescriber can hold your current dose longer or adjust it, depending on how you respond and tolerate it. You might:
- Stay at a middle strength such as 5mg or 7.5mg if progress is good and you tolerate it well.
- Hold at your current strength for longer before deciding whether to step up.
- Step down to a lower licensed strength if side effects are troublesome or if your prescriber advises it.
What about stretching pens?
Trying to make pens last longer by injecting less often or in smaller amounts is a form of altered dosing, and the same safety position applies.
Missed doses are different
A missed dose is not the same as deliberate dose-cutting, and there is a clear rule. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 For longer gaps, check with your prescriber before restarting.
10 · NHS and private routes
Long-term treatment on the NHS versus privately
Both routes exist, and the route you use affects how you plan for the long term. Mounjaro is available on the NHS, but access is being phased in over several years.4 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).6 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
Why this matters for the forever question
If you are treated through the NHS, the length and structure of treatment follow the service you are under and its review arrangements. 12 · Living with long-term use
Practicalities of staying on treatment long term
If you do continue, treatment becomes a routine, and a few practical points keep it smooth. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 What matters is the same day each week, so pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off.1
Injection sites
Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 Over months of treatment, rotation is a habit worth building from the start.
Changing your day
You can change your weekly injection day if needed, as long as there are at least 3 days between two doses.1 That flexibility helps with holidays, shift changes and life events.
Storage
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use — useful for short trips — and must then be thrown away. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1 Unopened pens stay in the fridge until first use and keep until the expiry date printed on the box.
Travel
You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.1 If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, since figures circulating online often come from other countries' labels.
Keeping it sustainable
Long-term use works best when it is boring: same day, rotated sites, pens stored correctly, reviews kept. If side effects, cost or lifestyle make it feel burdensome, that is a signal to talk to your prescriber, not to quietly drift. Report any suspected side effect through the Yellow Card scheme.7

What the trial evidence does and does not tell you
The headline trial tells you what treatment achieved while people were taking it, not what happens afterwards. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
To put that in perspective with a worked example only: for someone starting at 100 kg, 20.9% is about 21 kg.
Why this matters for the forever question
The SURMOUNT-1 figures describe outcomes at 72 weeks of treatment. They do not describe what happens after stopping. Anyone telling you that these results prove you can safely stop and keep everything, or that they prove you must stay on for life, is going beyond what this evidence shows.
The useful takeaway is about expectations while on treatment. Results build gradually, and the first weeks on 2.5mg are an adjustment phase. Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1
Comparing medicines fairly
You may see Mounjaro discussed alongside Wegovy or other medicines. Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
