Mounjaro is a once-weekly injection in a pre-filled multi-dose pen, the KwikPen, available in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

05 · What stopping involves
What happens when you stop Mounjaro
When you stop, the medicine leaves your system gradually over four to five weeks, and nothing needs doing while it clears. Tirzepatide has a half-life of about five days, so each week the amount in your body falls rather than dropping to nothing at once.1 There is no way to speed this up.
Stopping should always be planned with your prescriber. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14
Preparing for the point when the medicine clears usually means:
- Knowing which meals and routines have worked for you while on treatment, and writing them down.
- Keeping up the activity you have built, so it is already habit by the time appetite support changes.
- Arranging weigh-ins or check-ins with your prescriber or another health professional after stopping.
- Deciding in advance what you will do if you notice your eating drifting, rather than deciding in the moment.
If you stop for a while and later want to restart, speak to your prescriber first. Your prescriber will advise which dose to start on.

Many people ask whether they can stay on a lower strength. Your prescriber can advise on the strength that suits your situation, and that decision should be made at a review, not by guessing or by changing how you use the pen yourself.
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. They describe people on treatment, not people who stopped, so they do not tell you what happens afterwards.
Any change in strength is for your prescriber to set, depending on how you are feeling, your goals and any side effects. Typical questions to ask are:
- Is my current strength still the right one for where I am now?
- Would coming down a strength be reasonable, and what should I watch for?
- How long should I stay on the lower strength before reviewing again?
- What would tell us it is time to go back up, or to stop?
Maintenance is simply the name for the phase after you have reached your goal. The aim is to find the least support that keeps you comfortable and healthy, reviewed at sensible intervals.
07 · Safety over longer use
Is being on Mounjaro long term bad?
The Summary of Product Characteristics and the Patient Information Leaflet are the authoritative documents on side effects, who should not use Mounjaro and what to do if something goes wrong.113 Read the leaflet that comes with your pen, and return to it if anything changes in your health. Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.
Some points are worth knowing whether you are weeks or years into treatment:
- Oral contraceptives: Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Discuss contraception with your prescriber.7
- Insulin and sulfonylureas: doses of these may need adjusting because of the risk of low blood sugar. Never change them on your own; speak to your prescriber or diabetes team.
- New symptoms: tell your prescriber, and report suspected side effects through the Yellow Card scheme.19
Reviews are the main protection for people using a medicine over a long period. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

10 · Plateaus and slowing results
Do you eventually stop losing weight on Mounjaro?
Many people notice their progress slows after a time, and that is worth discussing rather than treating as failure.
At 72 weeks in SURMOUNT-1, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Those figures show a wide spread of outcomes, which is also a reminder that individual results vary, and that trial averages do not predict your own result.
If your weight loss slows, a sensible order of questions is:
- Is my routine still the same as it was? Portions, snacks and movement tend to drift over time without us noticing.
- Am I taking the medicine as prescribed? Missed or irregular doses change the picture; your prescriber can advise on what to do after a missed dose.
- Is the strength right? Your prescriber may suggest staying, adjusting or reviewing, depending on your response and any side effects.
- Have I reached a weight that is right for me? A plateau can simply mean your body has reached a comfortable point, and your prescriber can help judge this.
This is also where the "forever" question often becomes real. When progress slows, some people wonder whether continuing is worth it. The answer depends on your goals. If you are aiming to hold a weight, continuing could be part of that plan. If you have reached your target and feel confident, a supported step-down could be reasonable. Either is a conversation, not a rule.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
