Do you have to stay on weight loss injections for life?

No. Mounjaro is a prescription medicine you take for as long as you and your prescriber agree it is right for you, and you can stop whenever you choose. Nothing in the licence says treatment must continue for life, and nothing locks you in. The pharmacy does not auto-ship repeat orders either: every repeat order re-checks that the medicine is still suitable for you.

The more useful question is what you want from treatment. Mounjaro works by activating two natural gut-hormone receptors, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties.1 Most people feel fuller sooner and less hungry between meals.1 That effect is part of how the medicine works, and it is what supports a reduced-calorie diet. Once the medicine has cleared, that action is no longer there to support you.

So stopping is possible, but it is a change, not a neutral event. Many people find it helps to think of treatment in three phases: reaching your goal, deciding on a maintenance approach, and choosing whether to continue, reduce or stop. 04 · What stopping involves

What actually happens when you stop

When you stop Mounjaro, the medicine leaves your body gradually over about 4–5 weeks. Tirzepatide has a half-life of about five days, and there is no way to speed up clearance.1 Nothing needs doing while it clears, and you do not need a special routine to 'flush' it.

What you can expect to change is the support the medicine gave you. While you take it, the dual GIP and GLP-1 action helps you feel fuller sooner and less hungry between meals.1 As that effect fades, you rely more on your own eating habits, meal planning and activity. Some people find this straightforward; others notice that appetite feels different. Both experiences are worth discussing with your prescriber.

In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

05 · Planning a stop

How to plan stopping or reducing treatment

The safest way to stop is to decide in advance, with your prescriber, what you want your routine to look like afterwards. A sensible plan covers four things.

  • Your reason. Reaching a goal, side effects, cost or a life change such as pregnancy planning all need different handling. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
  • Your support. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Those habits are what carry on after the last pen.
  • Your dose. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.
  • Your monitoring. Keep weighing on a consistent schedule and look at the trend, not individual days.

Once you have a plan, tell your prescriber. Ongoing reviews are part of the service, and repeat orders re-check suitability, so you are never automatically shipped another pen.

06 · Breaks and missed doses

Is it safe to take a break, such as two weeks off?

A short gap is not unusual, but how you restart matters, so speak to your prescriber before taking a planned break. The missed-dose rule gives you the licensed starting point: if you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on as usual.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

A full two-week break means at least one dose falls outside that window.

You can also change your injection day if there are at least 3 days (72 hours) between two doses.1 If a trip or schedule change is the reason for your break, that may be all you need. 07 · Pace and plateaus

Is there a point where you stop losing weight?

Weight loss on Mounjaro is gradual by design, and progress is judged over months. The first weeks on 2.5mg are an adjustment phase, and most measurable progress builds from 5mg onward.1 The SURMOUNT-1 trial measured results at 72 weeks, so think in months, not days.2

In SURMOUNT-1 (2,539 adults), people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 For someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages; individual results vary and do not predict your own result.

Your prescriber reviews your response. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you.1 That is a built-in decision point, so 'forever' is never the default.

08 · Reading the scales

Why your weight jumps day to day

Daily weight swings are normal and say little about whether treatment is working. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 Weighing at the same time, in similar conditions, on a consistent schedule gives you a cleaner picture than reacting to one morning's result.

This matters especially around decisions about stopping. If you plan to come off treatment, a few weeks of steady readings give you a baseline to compare against, so you can spot a trend early and talk to your prescriber rather than reacting to one reading. Aim for steady progress, not a daily number. Rapid daily loss is not the aim of this medicine, which is intentionally gradual.

09 · NHS and private access

NHS and private access to long-term treatment

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Licensed use is for adults with a BMI of 30 or above, or between 27 and 30 with at least one weight-related condition, alongside a reduced-calorie diet and more activity.1

10 · Other injections

Does the same answer apply to other injections?

The principle is shared across the GLP-1 family: these are medicines that work while you take them. Wegovy (semaglutide) is a once-weekly injection with a dose ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, reaching the 2.4mg maintenance dose from week 17, and the 2.4mg dose is the licensed maintenance dose for weight management.18 Liraglutide products such as Saxenda are daily injections rather than weekly ones.
