03 · Flying with Mounjaro

Flying with Mounjaro: hand luggage, labels and cool bags

You can fly with Mounjaro, and the pens should travel in your hand luggage, because hold luggage can fall below freezing.¹ A pen that has frozen must never be used, so the hold is the one place to avoid.¹³

Ideally, pack the pen in a cool bag together with the prescription label or a copy of your prescription. Keep the pen cap on to protect it from light.¹³ Keep the pen in its original box where you can, so the dispensing label and batch details stay with it.

Rules for carrying prescription injectables differ between countries, so check the destination's rules before you fly.¹ Airlines and airports also publish their own guidance on medicines and needles, and it is worth reading before you go. If you have a connection, your hand luggage stays with you, which suits a pen that must not be left in a cold hold.

Keeping your pen safe in hot weather

An in-use pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.¹³ That is why most holidays need no special arrangements, and why a pen you are already using is easier to travel with than a spare.

For hot climates, use an insulated pouch.¹ Think about the places a pen actually gets hot: a parked car, a beach bag in full sun, a windowsill, a hot boot. If your accommodation has a fridge, store the pen there, while remembering that it must never freeze, so keep it away from the coldest back wall and the freezer compartment.

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.¹ If an unopened pen has been out of the fridge, check with the pharmacy before using it, rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, and the UK licence is the one that applies to your pen.¹

Before every injection, look at the pen. Do not use one that has been frozen, has been above 30°C, looks cloudy or discoloured, has particles in it, is past its expiry, or has been in use out of the fridge for more than 30 days.¹³ The solution in a genuine pen should be clear and colourless. When in doubt, contact the pharmacy before injecting.

05 · Injection day and missed doses

The most useful rule for travellers is that the injection day matters more than the time of day. Mounjaro works the same morning or evening, with or without food, so a time-zone change does not need a complicated plan; what matters is the same day each week.¹

If your day falls somewhere inconvenient, such as a travel day, you can move it. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.¹ Many people prefer to bring the dose forward or push it back to a calmer day. If you do miss the day, the rule is simple. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.¹ If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within 3 days of each other.¹

06 · Pausing for a holiday

Should you stop Mounjaro for a holiday?

Stopping Mounjaro for a holiday is usually not needed, and it should be a planned decision with your prescriber rather than a last-minute one. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system; a short break is not a clean reset, and there is no way to speed this up.¹

The longer-term picture matters too. Mounjaro manages weight; it does not cure the biology behind it.¹⁴ In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.¹⁴ That was a trial of stopping altogether, not of a short holiday gap, but it shows why stopping should always be planned with your prescriber.

Mounjaro is licensed as a once-weekly medicine, and altering doses outside the licence means unknown dosing accuracy and no safety data.

07 · Eating, activity and side effects

Holiday food and drink can feel different on Mounjaro because the medicine helps you feel fuller sooner and slows how quickly the stomach empties.¹ Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.¹³ If you are in a higher-dose phase or have just stepped up, think about timing so the days after your injection are not your busiest travel days.

Many people choose a weekend morning so early side effects fall on days off.¹ On holiday, the same logic applies: choose a quieter day rather than a flight or an excursion.

Staying active is part of the licence, not an optional extra. Regular moderate activity such as brisk walking, plus resistance work for muscle, is the evidence-backed pattern, and on days with side effects lighter movement still counts.¹ A holiday of walking and swimming fits that comfortably.

Flying is not on the list of side effects in the UK Mounjaro patient leaflet. If you feel unwell on or after a flight, treat it as you would any new symptom: speak to your prescriber, and report suspected side effects through the Yellow Card scheme.¹⁹ Drink fluids sensibly and eat smaller meals if you feel full or queasy.

Tell your travel insurer about your treatment and your reason for taking it. Policies differ, and no general rule applies to every insurer, so read the declaration wording or ask them directly.

Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
