What is Mounjaro 10mg?

Mounjaro 10mg is one of six strengths of tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.¹ It comes as a pre-filled multi-dose pen, the KwikPen, in 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

The number on the pen is the dose of each weekly injection, not the total in the pen. Each KwikPen contains four weekly doses.¹³ A 10mg pen therefore gives you four injections of 10mg, which is one month of treatment.

That helps explain why the ladder is stepped slowly. Your body is given time to adjust at each strength before the next increase.

10mg is not a starting dose and it is not a ceiling. It sits in the middle of the upper half of the ladder. The licensed range runs up to 15mg weekly, and your prescriber chooses where you stop based on how you respond and how well you tolerate each step.¹ Many people settle at 5mg, 10mg or 15mg, and higher is not automatically better.

06 · How it works

How does Mounjaro 10mg work?

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.¹ In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic). It does not remove the need for diet and activity. The medicine makes a reduced-calorie diet easier to follow, and the diet is still what the trials were built around.

Tirzepatide has a half-life of about five days. That is why one injection a week is enough, and it also means that after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.¹

For women who take an oral contraceptive, there is one specific interaction worth knowing. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.¹ The medicine can reduce absorption of oral contraceptives because stomach emptying slows.¹ Tell the prescriber what you take during the consultation so they can advise you properly. The same goes for insulin or sulfonylureas: doses may need adjusting because of the risk of low blood sugar, so disclose these medicines too.

Nothing about the mechanism changes at 10mg compared with other doses.

07 · Results at 10mg

What weight loss can you expect on Mounjaro 10mg?

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 19.5% of their body weight on 10mg over 72 weeks. That compared with 3.1% with lifestyle changes alone.² These are trial averages and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

On the other doses in the same trial, the averages were 15% on 5mg and 20.9% on 15mg.² Of participants at 72 weeks, 85% on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg, and 57% on 15mg, lost at least 20%.²

To make the percentage concrete, here is a worked example only: for someone starting at 100 kg, 19.5% is about 19.5 kg. Someone starting at a different weight would see a different number, and a trial average does not predict your own result.

Where does the 10mg figure sit? It falls between the 5mg and 15mg averages. If you respond well at 10mg and tolerate it, staying there can be a sound clinical choice.

This is not a comparison with other medicines. Different trials had different designs and populations, so numbers from separate trials should never be set side by side as though they were head-to-head.

08 · Reaching 10mg

How do you get up to a 10mg dose?

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.¹ After at least 4 weeks the dose increases to 5mg. It can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, then 10mg, then 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.¹

The word "minimum" matters. If you have troublesome side effects, your prescriber can keep you on a dose for longer, and many people do. Staying put for another month is not failure. It is the system working as designed.

Your prescriber reviews your response along the way. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you.² Monthly reviews are included with our service, so a prescriber is looking at your progress each time you reorder.

09 · How to inject

How do you inject Mounjaro 10mg?

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.¹ Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.¹³

Setting the dose. You set the KwikPen by turning the dose knob until the dose indicator shows the correct dose in the window. For exact device steps, follow the pen's instructions and the patient information leaflet. Needles, a sharps bin and injection guidance come with every order.

Best time of day. There is no "best" time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off.¹³

If you miss a dose. Take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.¹

Changing your injection day. You can change your weekly day if needed, as long as at least 3 days (72 hours) have passed since your last dose.¹

10 · Side effects and cautions

What should you expect, and what should you avoid on 10mg?

The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.¹ Experiences differ widely. Some people notice appetite change and early loss in week one, while others feel little until a higher dose. Both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.

Side effects can reappear or intensify after a dose increase, which is one reason the steps are spaced at least 4 weeks apart. If a step up is hard to tolerate, tell your prescriber. They can hold you at the current dose longer or step you back within the licence.

What should you avoid? Avoid improvising with the dose, such as taking extra, partial or more spread-out injections. Avoid taking two doses within 3 days of each other. And avoid treating a supplement as an equivalent: a low-cost supplement with no clinical oversight is not the same thing as a licensed prescription medicine.

What about foods such as bananas? No food is banned on Mounjaro, including bananas. Mounjaro is used alongside a reduced-calorie diet, so the useful question is how a food fits your overall eating plan. Your prescriber can give personal dietary advice if you have a specific concern.

11 · Storage and travel

How do you store and travel with Mounjaro pens?

Keep Mounjaro pens in a fridge at 2 to 8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded.¹³ Keep the pen cap on to protect it from light, and never use a pen that has been frozen.

Because a 10mg pen holds four weekly doses, an in-use pen is normally finished within 4 weeks, well inside the 30-day limit. The limit matters mainly if you are away or if you have paused treatment partway through a pen.

Unopened pens. Store them in the fridge until first use. They keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, so they may not apply to UK KwikPens. The patient information leaflet and our pharmacy team are the right sources.

Flying and holidays. You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing. Ideally use a cool bag, and carry the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates, use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.¹³

On delivery. Our pens arrive in discreet, temperature-controlled packaging by free next-day tracked delivery. Unpack and refrigerate them promptly. If a parcel looks damaged or a pen is faulty, contact us within 3 days. Dispensed prescription medicines cannot otherwise be returned.

Practical habits. Write the date you first used a pen on the box, keep the cap on between doses, and keep spare pens at the back of the fridge, away from the freezer compartment where they could freeze. Keep sharps in the supplied sharps bin and never reuse needles.

12 · Eligibility and the NHS

Who can take Mounjaro 10mg, and what about the NHS?

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹ A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you. Mounjaro should not be used if you are allergic to tirzepatide or any of its ingredients, or during pregnancy or while trying to conceive; it must be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.

Whether you are eligible is a prescriber decision. The free consultation asks about your health and medicines, and a GPhC-registered prescriber reviews the answers. If the prescriber decides Mounjaro is not suitable, the order is refunded automatically in full.

The NHS picture. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If you may qualify for NHS treatment, ask your GP. The two routes are not mutually exclusive in principle, but you should be open with every clinician about what you are taking. If you are not in an early NHS group, a private route lets you start under the same licence, with a prescriber and ongoing monthly review.

The NHS prescription charge in England is £9.90 per item, but it does not apply to private prescriptions. A private treatment cost is the pen price you see on the product page, with no separate consultation fee.

Private supply does not mean a lesser standard. The things to check are that the pharmacy is on the GPhC register, that a prescriber reviews you before dispensing, and that the pens are genuine, sourced directly from Eli Lilly's UK distribution as ours are through LillyDirect. People who are already on the NHS pathway sometimes ask about changing provider. That is a decision to make with the clinician managing your treatment, so tell both teams what you take.

13 · Staying on track

How do you make the most of 10mg, and when should you reassess?

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend and not by individual weigh-ins.²

How to speed up weight loss on 10mg? The honest answer is that you cannot safely hurry the medicine, and the dose is not a dial to adjust yourself. Extra, partial or more frequent injections are not licensed, and altering doses means unknown dosing accuracy and no safety data. What you can influence is everything the treatment is designed to work alongside: a reduced-calorie diet and increased physical activity, taking the injection on the same day every week, and not skipping doses.

Practical habits that support that approach are simple. Eat regular, smaller meals if you feel full quickly, keep a food diary, build activity gradually, and track your weight weekly on the same scales and not daily. A weight that bounces up or down by a kilo or two between days is normal noise, particularly early on when water weight is part of the early loss.

When to reassess. Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you. If you stop. After a final dose it takes roughly 4 to 5 weeks for tirzepatide to clear your system. There is no way to speed this up, and nothing needs doing while it clears. If you plan to stop or change dose, speak to the prescribing team first so the plan is safe.

14 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
