Start with your trend, not a single weigh-in. Weight loss on Mounjaro is gradual by design, and most measurable progress builds month by month.4 Early loss includes water weight, so the pattern over weeks tells you far more than one day on the scales.

Then ask three plain questions. Is my appetite manageable? Can I tolerate the side effects? Is the trend still going the right way? If the answers are yes, staying on 10mg is a sensible and common choice. If any answer is no, raise it at your next review rather than adjusting the pen yourself.

03 · 10mg in the dose ladder

Where 10mg sits on the dose ladder

10mg is the fourth step on a six-strength ladder, and it is a strength many people choose to rest on. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1

After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Those are minimum gaps. Your prescriber can take longer on any step if that suits you better.

The pens come in 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg strengths, each containing four weekly doses.1 A person on 10mg is therefore taking one weekly injection from a pen labelled 10mg, with no need for any splitting or adjusting. The point of a ladder is that you can stop at the step that works. You are not failing by staying on 10mg, and you are not obliged to try 12.5mg or 15mg just because they exist.

04 · What the trial data shows

What the trial data says about 10mg

Trial averages show meaningful weight loss on 10mg. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

The same trial reported how many people reached larger losses. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Those numbers describe groups, not you. A trial average does not predict your own result.

So are higher doses more effective? On average in that trial, the higher dose showed a somewhat higher average loss, but higher is not automatically better. Tolerability, how you feel and how much you can sustain matter alongside the size of the average.

05 · Reasons to stay put

Good reasons to stay on 10mg

A good reason to stay is that the dose is working and you feel well. Weight loss that continues, an appetite you can manage and side effects you barely notice are all signs you have found your level.

A second reason is tolerability. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If a higher dose would bring those back, staying on 10mg avoids that trade-off.

A third reason is simplicity. A steady dose means a steady routine: the same day each week, the same pen strength, the same habits. That consistency helps you judge progress by trend.

A fourth reason is that you may already be near your goal. If you are close, there may be little to gain from escalating, and your prescriber can talk through what maintenance looks like.

None of this means you should stop reviewing. Staying on a dose is an active decision that your prescriber revisits as your weight, health and circumstances change.

06 · Reasons to ask about changing

When to ask about stepping up or down

Ask for a review when progress stalls, when appetite returns strongly, or when side effects make daily life hard. Those are the situations in which a change may be considered, and the change should be agreed with your prescriber.

If appetite is the problem, read our guide on being still hungry on 10mg. Hunger can have many causes, including meal composition, sleep and stress, and your prescriber can help you work out what applies. A step up is one option within the licence, but it is not the only one and not always the right one.

Stepping down within the licence is a prescriber decision; altering doses outside the licence means unknown dosing accuracy and no safety data.

If you are not responding, a formal review applies. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4

07 · Safety: no dose tinkering

Why you should not adjust the dose yourself

Do not change the amount in the pen or the schedule on your own.

Altering doses outside the licence means unknown dosing accuracy and no safety data.1

Staying on 10mg means exactly that: a full 10mg injection once a week, on the same day. Anything else is a different treatment plan, and it should be one your prescriber has agreed.

08 · Staying on track week to week

Keeping your 10mg routine steady

A steady routine is what makes staying on 10mg work. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 There is no best time of day: it works the same morning or evening, and what matters is the same day each week.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as there are at least 3 days between two doses.1 Rotate the injection site with each dose between the stomach area, the front of the thigh, or the back of the upper arm if someone else gives the injection.

Store pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. 09 · If you stop or go lower

What happens if you step down or stop

Stepping down is a legitimate option when it is done with your prescriber. If your goal is mainly to reach a comfortable resting dose, our page on staying on the lowest dose covers the other end of the ladder and explains why a lower strength is not a failure.

If you stop altogether, the medicine does not vanish at once. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

Any plan to stop, step down or restart should be agreed with your prescriber, who can look at your history and goals. Treatment is used alongside a reduced-calorie diet and increased physical activity, and those habits are what you carry forward whichever dose you end up on.

For some people, NHS routes may also be relevant. Mounjaro is available on the NHS, but access is being phased in over several years, with the primary-care rollout beginning in June 2025 with the highest-need groups first.4 Your GP can tell you what applies locally.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
