01 · The verdict
Injectable tirzepatide is the only tirzepatide you can choose
The honest answer to "which is better" is that you are not choosing between two forms of tirzepatide. Mounjaro is a once-weekly injectable prescription medicine made by Eli Lilly, licensed in the UK for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose KwikPen in six strengths from 2.5mg to 15mg.
The real decision is between tirzepatide as a weekly injection and an oral GLP-1 tablet that contains a different active ingredient. Wegovy tablets contain oral semaglutide, and Foundayo contains orforglipron. Each has its own product information and its own trials.2021 A tablet is a legitimate option for some people, and injections are not automatically superior. No tablet has been shown to match tirzepatide either, because no trial has compared them directly.

Decision factors that matter more than the format
Start with the evidence behind each option. For the tablets, read each product's SmPC and ask your prescriber how they apply to you.
Next, think about routine. A weekly injection means one fixed day each week, at any time of day, with or without food.1 Some people find one injection easier than remembering a tablet. Others would rather avoid needles altogether. Both preferences are reasonable, and neither changes how the medicine has been tested.

04 · What tirzepatide is
What tirzepatide is, and why it is a pen
Tirzepatide is the active ingredient in Mounjaro, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It is supplied as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.
This is the answer to the first half of the question. The licensed tirzepatide product is the pen. The UK product information describes it as an injection given under the skin once a week. It does not describe a tablet form. If a website, social post or seller claims to offer "oral tirzepatide", treat it with caution: whatever it is, it is not the licensed Mounjaro pen. Offers to supply without a consultation or prescription are a red flag too.
Each KwikPen contains four weekly doses, so one pen covers four weeks. That design is part of why the pen is the licensed delivery system, and why altering how it is used falls outside the licence.

Why does this matter for your decision? Because comparing "oral tirzepatide" with "injectable tirzepatide" assumes a choice that the licensed product range does not offer. The more useful comparison is between Mounjaro and other licensed GLP-1 medicines, judged on trial evidence, tolerability, routine and your prescriber's advice.
It also helps to keep the language precise. "GLP-1 medicine" is a family label. Tirzepatide belongs to a newer group because it acts on two receptors, while semaglutide acts on one. Two medicines being in the same family does not mean their trial results, doses or side-effect profiles are interchangeable, so always check the product information for the specific medicine you are considering.
05 · How it works
How tirzepatide works: two receptors, not one
Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.
This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1 When people ask whether an oral GLP-1 "works as well" as an injection, the first thing to check is which medicine is being compared. A tablet that contains semaglutide or orforglipron is a different drug with a different trial record, whatever the format.
Slower stomach emptying is also the reason for the contraceptive caution on Mounjaro: it can reduce absorption of oral contraceptives because the stomach empties more slowly. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you use an oral contraceptive, raise it in your consultation so your prescriber can advise.7
Tirzepatide has a half-life of about five days. After a final dose it takes roughly 4 to 5 weeks to clear the system.1 There is no way to speed this up, and nothing needs doing while it clears.
Mounjaro works the same morning or evening, and if you miss a dose you can take it as soon as you can within 4 days (96 hours) of the scheduled time.
Appetite, fullness and the pace of stomach emptying vary from person to person, and the first weeks are about adjustment rather than results. Your prescriber reviews how you are responding and tolerating the medicine as the dose changes.

06 · What trials show
What the trials show for the injection
In SURMOUNT-1, a trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
The same trial reports how many people reached particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2
To make a percentage concrete, a worked example: for someone starting at 100 kg, 20.9% is about 21 kg. That is straight arithmetic on the trial average, not a prediction for you. Trial averages do not predict your own result.
The head-to-head evidence is SURMOUNT-5. Adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy).15 Separately, in the STEP 1 trial, adults on semaglutide 2.4mg lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo.9 STEP 1 was a different trial, so its figure should not be set against SURMOUNT-1 as though the two were tested together. Both medicines are effective licensed treatments.
What about tablets? Wegovy tablets and Foundayo have their own trials, OASIS 4 for oral semaglutide and ATTAIN-1 for orforglipron.2223 If you want those figures, read the trial papers and each product's SmPC.
So is an oral GLP-1 as effective as the shot? Anyone who gives you a firm "yes" or "no" is going beyond the evidence.

07 · Oral GLP-1 tablets
Which GLP-1 can be taken orally?
Two GLP-1 weight-loss tablets are now licensed in the UK: Wegovy tablets, which contain oral semaglutide, and Foundayo, which contains orforglipron.2021 Neither contains tirzepatide. Each has an SmPC on the electronic medicines compendium that sets out its dosing, warnings and who it is for.
The trial literature for these tablets includes OASIS 4, a trial of oral semaglutide at a dose of 25 mg in adults with overweight or obesity, and ATTAIN-1, a trial of orforglipron, an oral small-molecule GLP-1 receptor agonist, for obesity treatment.2223 Those trials were not designed to compare the tablets with Mounjaro.
Do oral GLP-1s work? These tablets have been through clinical trials and are licensed in the UK. "Work" is not the same as "work equally", though. They are separate medicines with their own evidence, and no trial puts any tablet head to head with tirzepatide.
Which GLP-1 pill is best for weight loss? No licensed medicine is best for everyone, and any ranking that ignores your health profile, other medicines and tolerability would be guesswork. A prescriber reviewing your questionnaire can say which options are suitable. Ask about the tablet route in your consultation if that is what you prefer.

08 · Using the pen
What injecting weekly actually involves
Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1 Each KwikPen contains four weekly doses. For many people, the idea of an injection is the main reason they ask about tablets, so it helps to know what the routine looks like.
Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. The licensed injection sites are those three, and the product information does not rank one above the others. Choose a site you can reach comfortably and rotate it.
You set the dose by turning the dose knob until the indicator shows the correct dose in the window. Follow the pen's instructions and the patient information leaflet for the injection steps.13
When is the best time of day? There is no "best" time. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off.

A weekly pen does have a practical advantage over a daily medicine: there are only four injection days in a four-week pen. The trade-off is that it involves a needle. Neither factor changes the evidence, so choose on what you will realistically keep up, and discuss it with your prescriber.
You can also change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That flexibility helps if your schedule changes or you are travelling.
09 · Dose ladder
Dose ladder, missed doses and pace
Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.111 Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1
Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.
Some people notice appetite change and early loss in week one, while others feel little until a higher dose. Both are normal. Early loss includes water weight. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.
Your prescriber reviews your response. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.
This stepwise approach is a feature of the injection that readers sometimes overlook when asking about tablets. The pen's strengths allow fine adjustment in 2.5mg steps.
Altering doses outside the licence means unknown dosing accuracy and no safety data. Use the licensed ladder and talk to your prescriber instead.
10 · Storage and travel
Storing and travelling with the pen
Keep Mounjaro pens in a fridge at 2 to 8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.
Storage is a real consideration when you compare formats, so it is worth being plain about it. A pen needs the fridge before first use and some care afterwards. Tablets have their own storage instructions in their SmPCs.
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.
Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, and the UK product information is the one that applies to a UK pen.

11 · Cost over a year
What each route costs over a year
The standard price of one 2.5mg Mounjaro pen, which covers four weeks, is £169.99. Your first 2.5mg pen costs £129.99 with code FIRST40. Standard prices for the other strengths are £218.99 for 5mg, £279.99 for 7.5mg, £290.99 for 10mg, £319.99 for 12.5mg and £329.99 for 15mg. That is £42.50 per week at the starting dose, rising to £82.50 per week at 15mg.
If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. Over a first year of 13 pens, a person who stays on 5mg after the 2.5mg starting pen pays £2,711.88, up to £3,599.88 for someone who steps up through every dose to 15mg. For a 7.5mg, 10mg or 12.5mg maintenance dose, use that dose's monthly price. These totals are before discounts: a new customer's first order is £40 less, and every later order is £25 less than the standard price.

12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
13 · NHS and ordering
NHS access, private supply and how ordering works
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
