07 · Fluids and drinks
Hydration, coffee and alcohol
Drink regularly through the day, because reduced appetite means less fluid from food. Vomiting or diarrhoea can dehydrate you quickly, and dehydration is the main route to the rare kidney problems reported with this class of medicine.13 Keep water within reach, and treat an inability to keep fluids down as a reason to contact your clinical team, not something to push through.
Coffee and tea: there is no interaction between Mounjaro and caffeine, so coffee and tea are fine. Strong coffee on an empty, slower-emptying stomach can aggravate nausea or reflux for some people, so try having it with food. High-calorie coffee drinks, such as sweetened lattes and syrups, are an easy place for the calorie deficit to leak. The "coffee rules" that circulate on social media have no clinical basis.
Alcohol: there is no direct interaction between Mounjaro and alcohol. However, drinking on a reduced appetite and an emptier stomach can hit harder than usual, and alcohol adds calories that work against your goals. If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar. Moderation is the sensible rule, and many people find their desire for alcohol drops on treatment.
Sugary soft drinks and fruit juices add calories without helping with fullness. Water, sparkling water, herbal tea and unsweetened drinks are simpler choices.
08 · What to limit
What should I avoid on Mounjaro?
Nothing is formally off limits, but it helps to limit very fatty, fried and very sugary foods, because many people feel worse after them.13 Even bananas are allowed.
Think of limiting rather than avoiding:
- Greasy or fried food: chips, battered food, rich pastries and fatty takeaways are foods many people feel worse after.
- Very sugary food and drinks: they add calories quickly and many people find them unpleasant on treatment.
- Oversized portions: the stomach empties more slowly, so a big plate can leave you uncomfortably full for hours.
- Very rich sauces and creamy dishes: fine in small amounts, harder in large ones.
- Ultra-processed snack foods: easy to overeat and low in protein and fibre, so they crowd out the food you need.
You can still have a treat. What changes is that portion size tends to need more attention, and you may find that your tastes shift. If a food reliably makes you feel unwell, leave it for now and try it again later at the next dose or in a smaller portion.
09 · Side effects and eating
Eating on bad days: nausea and low appetite
Mild nausea, altered appetite and tiredness in the days after each injection are the commonest early effects and usually settle.1 On those days, plain, small, regular meals are the practical answer. Bland food, cold food and sipped fluids are often easier to tolerate than hot, spicy or rich dishes.
Some people pick their injection day to suit this. Many choose a weekend morning, so early side effects fall on days off.1 Mounjaro works the same in the morning or evening, with or without food, and what matters is keeping to the same day each week.
Ideas for low-appetite days:
- Choose protein-rich drinks or yoghurt if solid food feels like too much.
- Eat little and often rather than pushing through a full plate.
- Sip fluids steadily rather than gulping.
- Keep gentle movement going where you can. Lighter activity still counts on injection days.
If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. Tell your prescriber about anything new or lasting that worries you. You can also report suspected side effects through the Yellow Card scheme.19
10 · Exercise alongside food
Movement is part of the plan
Increased physical activity is part of the licence, not an optional extra.1 Nothing special is required. The evidence-backed pattern is regular moderate activity, such as brisk walking, plus resistance work for muscle. Start where you are and build up.

You do not need a gym. Bodyweight exercises, resistance bands, carrying shopping and stair climbing all count. The important thing is that the habit lasts.
11 · Realistic pace
How quickly does it work, and can food speed it up?
Weight loss on Mounjaro is gradual by design, so think in months, not days. The starting weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.1 Trials measured results at 72 weeks, and trial averages do not predict your own result.
Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Some people notice an appetite change and early loss in the first week, while others feel little until a higher dose. Both are normal.
