The hour does not matter because Mounjaro is a once-weekly injection and works the same morning or evening. Taking it at breakfast or at bedtime changes when the injection happens, not how the medicine behaves afterwards.

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

The reason timing is so forgiving comes down to how long the medicine stays in your system. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 A half-life that long means a few hours either way on injection day is small against the weekly cycle.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Pick a day and time you will remember and stick to it. Many people choose a weekend morning so early side effects fall on days off.13 This is the single most useful piece of timing advice, and it comes down to protecting your routine rather than optimising an hour.

  • If I feel tired or queasy, can I take it easy that day and the next?
  • Will I be at home, or could travel get in the way?

If most answers favour a particular day, that is your injection day. Write it somewhere visible and make it feel as ordinary as any other weekly task, such as putting the bins out.

A recurring phone alarm on the same day is the simplest tool. Pair it with something you already do, such as a Sunday morning coffee. Keep the pen in the fridge where you will see it, with the pen cap on to protect it from light. 07 · Changing day or missing a dose

You can change your injection day, and you can recover from a missed dose, as long as you follow the spacing rules. These rules protect you from taking doses too close together.

Changing your injection day

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.13 In practice, count from your last injection, choose the new day that is at least that far away, and then keep to the new day each week. The time of day can move freely, since the licence does not fix an hour. The day is the part with a minimum gap, so check the gap whenever you switch.

If you miss a dose

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.13 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.13

This is the rule to keep handy. It means a forgotten Sunday morning does not become a crisis. If you realise on Monday or Tuesday, you can take it then, and return to Sunday the following week. If you only notice much later, skip it and wait for your normal day.

What not to do

Do not double up to "catch up". Never take two doses within 3 days of each other. Do not try to split a dose, share a dose across days or deliver smaller amounts more often. That approach is not licensed.

When to speak to your prescriber

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.13 You do not need an empty stomach, and a meal does not change the instruction. If you are asked whether to take it on an empty stomach, the answer is that either is fine.

Where to inject

Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.13 Rotating helps avoid irritating the same patch of skin, so keep a mental note of where you went last time.

Setting the dose on the pen

Storing the pen between doses

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.13

Storage does not change what time you inject, but it can affect your plans. Taking a pen out of the fridge a little before injecting may feel more comfortable for some people, but follow the leaflet rather than any rule of thumb from social media.

Travelling with your pen

You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.13

09 · First weeks and results

The first weeks are an adjustment phase, and the time of day you inject is unlikely to be what shapes them. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1

The dose ladder

After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.111 Each step is a chance to check how you feel, so keep your injection day steady as the dose changes.

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result, and none of these figures depended on injecting at a particular hour.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Keeping to the licensed schedule is the safest way to take Mounjaro, and that includes not trying to "hack" the timing or the dose.

If you are tempted to inject smaller amounts more often to ease side effects, speak to your prescriber instead. They can adjust your pace within the licence, which is far safer.

Mounjaro is available on the NHS, but access is being phased in over several years.
