How tirzepatide (Mounjaro) works

Mounjaro is the brand name for tirzepatide, a weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

05 · Semaglutide and Wegovy

Semaglutide: Wegovy and Ozempic

Semaglutide is the active ingredient in Wegovy and Ozempic, and it acts on the GLP-1 receptor alone. Wegovy is the version licensed for weight management. Its pens come in a ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, taken weekly.18

The dose steps up after at least four weeks, reaching the 2.4mg maintenance dose from week 17. The 2.4mg dose is the licensed maintenance dose for weight management.18 Our Wegovy pen guide explains handling and the injection routine.

Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. A provider that lists both is naming the same active ingredient under two brands with different licences. Check which brand is actually prescribed to you, and why.

06 · Liraglutide, the daily option

Liraglutide: Saxenda and Victoza

Liraglutide is an older GLP-1 medicine, supplied as Saxenda and Victoza, and sold in some products under other names. It is supplied at a concentration of 6mg per millilitre. It is a daily injection, unlike weekly Mounjaro and Wegovy.

The daily schedule is the main practical difference. A weekly pen means a single injection day to remember, while a daily injection is a routine to keep up every day.

07 · Oral GLP-1 tablets

Oral GLP-1 tablets: Wegovy tablets, Foundayo and Rybelsus

Wegovy tablets contain oral semaglutide, and Foundayo contains orforglipron, a small-molecule oral GLP-1 receptor agonist.2021 Rybelsus is also an oral semaglutide brand.

08 · NHS access

What are the criteria for Mounjaro on the NHS?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46

Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. Your GP is the right person to ask where you sit in the local rollout. NHS access to Wegovy runs through specialist weight-management services; ask your GP.

09 · Interactions and safety

Safety, interactions and side effects

Because Mounjaro slows stomach emptying, it can affect how quickly some oral medicines are absorbed, particularly when you start treatment or increase the dose.1

Tell your prescriber about everything you take, especially warfarin or other narrow-margin medicines, insulin, or sulfonylureas (where doses may need reducing to avoid low blood sugar). No adjustment is needed for paracetamol.1

The medicine can also reduce absorption of oral contraceptives, because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

If you notice a new symptom, speak to your prescriber, and report suspected side effects through the Yellow Card scheme.19

10 · Which is safer

Which is safer, Wegovy or Mounjaro?

Neither is safer in general. Both are licensed prescription medicines, and the right one depends on your health history, other medicines and how you tolerate side effects. A prescriber should make that call, not a headline.

11 · Supplements and shortcuts

Poor man's Ozempic, natural mimics and over-the-counter pills

Berberine is often called nature's Ozempic or the poor man's Ozempic. It is a low-cost supplement with no clinical oversight, and its dosing is not standardised. It is not an established equivalent of a licensed GLP-1 medicine.

The same goes for the top over-the-counter weight loss pill, and for ways to mimic Ozempic naturally. Eating more protein and fibre, building activity and sleeping well all support weight management, but they do not copy how a prescription medicine works. Products promising dramatic weight-loss claims deserve scepticism.

13 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1 Pregnancy: Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
