The Mounjaro KwikPen comes in strengths of 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg, so the step above 10mg is 12.5mg.1 After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Note the phrase “a minimum”. That sets the earliest point a step-up is allowed, not a date you must hit. 04 · The full dose ladder

The full Mounjaro dose ladder, rung by rung

Mounjaro is the brand name for tirzepatide, a weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in strengths of 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 From there the licensed route is 5mg, then 7.5mg, then 10mg, then 12.5mg, then 15mg. Each rung is a step of 2.5mg above the one before it once you are past 5mg, and the ladder ends at 15mg weekly.

Because you are reading this, you are probably on 10mg, which is the fourth rung. That means you have already moved through the starter, the first main step and the 7.5mg step. Your own timeline may be longer, and that is not a problem. The licence sets only the minimum gap, and your prescriber chooses the pace.

Doses scale in a straightforward way. Each pen is built to deliver its weekly doses at the strength printed on it, and the dose window on the pen confirms what you are about to inject.

It helps to think of the ladder as a menu of options, not a progress bar. Some people reach the top and do very well. Others feel best on a middle rung and stay there for the long term. Both are legitimate outcomes under the licence, and neither is a better or worse result in itself.

The same goes for people who step up and then step back. If you are unsure which rung you are on, check the strength printed on the pen label and the box, and confirm with your prescriber before you change anything. 05 · Is higher better?

Is a higher Mounjaro dose always better?

No. Higher is not automatically better, and your prescriber will guide the right pace.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg, which is why 10mg is a recognised stopping point rather than an unfinished one.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Look at what those averages say. These averages do not tell you what will happen to you. Some people respond strongly at lower strengths, and some need more. The same trial reported that at 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

So what should drive your decision? Three things matter more than the strength itself. First, whether your current progress suits your goals and your health. Second, how well you tolerate your current dose. Third, how well you are keeping up the habits that make the medicine work, such as eating a reduced-calorie diet and staying active.

A step-up adds more medicine but does not replace those habits. That is a conversation to have with your prescriber at your monthly review, and it is why we keep reviews and dose follow-ups as part of the service.

If you feel good, are losing weight at a rate your prescriber is happy with and have no wish to go higher, staying on 10mg is a sound plan.

06 · Plateaus on 10mg

What to do if progress stalls on 10mg

Check the basics first, and discuss a step-up with your prescriber only after that. Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1

Within the licence, the responses are: review food intake honestly (portions drift back up), protect protein and activity, check the basics (sleep, alcohol), and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.1 A plateau is not a reason to double doses or add unlicensed products.1

In practice, that gives you a sensible order of operations. Start with food. Keep a plain log of what you eat for a stretch of days, and be honest about snacks, drinks and portion size, because small additions are easy to overlook. Then look at protein and movement. Then look at sleep and alcohol, which affect appetite and routine more than people expect.

Only after that does a dose conversation make sense. If you do all of that and the trend has still flattened over a meaningful stretch of weeks, ask your prescriber whether 12.5mg is suitable. The decision will take into account how you have tolerated 10mg, how long you have been on it and where you are against your goals.

Judge progress by trend rather than by individual weigh-ins. Early loss includes water weight, and day-to-day fluctuation is normal, so a single flat or higher reading tells you very little.1 Weekly or fortnightly averages tell you much more, and our free GLPTrackr companion app can help you record them over time.

It is also worth remembering that a pause is not failure. Your body may simply be consolidating.

What you should never do is take extra medicine, shorten the gap between doses or combine Mounjaro with unlicensed products to break through. Those routes carry unknown risks and no safety data. The licensed options are to hold, to step up when your prescriber agrees, or to step down when tolerance is the issue. Those three are enough for nearly every situation.

07 · Making the step-up safely

How to make the move to 12.5mg safely

Keep your routine exactly as it is and change only the pen. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1 A new strength does not change any of that.

Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 Each KwikPen contains four weekly doses.1 That means a new strength usually arrives when you have finished your current pen and have your prescriber's go-ahead.

Check the label before you inject. The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window. When you begin using a new strength, read the number on the box and in the window, because it is easy to reach for the wrong pen if you have an old one in the fridge.

There is no best time of day to inject. Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week.1 Many people choose a weekend morning so early side effects fall on days off, and that can be a useful habit when you start a new strength.

After a dose change, pay attention to how you feel. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If you notice anything that worries you, contact your prescriber. For severe, persistent stomach pain, swelling of the face, lips, tongue or throat, or difficulty breathing, get medical help straight away. You can also report suspected side effects through the MHRA Yellow Card scheme.19

08 · Missed doses and injection day

Missed doses and changing your injection day

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

This matters around a dose change because people sometimes worry that a missed injection means they should jump straight to the next strength, or take a bigger amount to catch up. Neither is right. The missed-dose rule applies at every strength, including 10mg and 12.5mg.

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That gives you room to adapt around holidays, shift work or a hectic week without breaking the licence. Move the day once, then stay on the new day.

Storage is the other practical matter when you are handling a new pen. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded.1 Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

Travelling is straightforward. You can fly with Mounjaro, carrying pens in hand luggage because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription.1 Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, since figures circulating online often come from other countries' labels.1

09 · Why not skip or microdose

Why you should not skip rungs or microdose

The licensed dose ladder already starts low for tolerability, at 2.5mg. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

The same thinking applies in the other direction. Jumping from 10mg to 15mg without 12.5mg in between is not how the ladder is written. The licence describes steps up by 2.5mg at a minimum of 4-week intervals, and that stepwise approach exists so your body can adjust to each increase.1

Why do people look for shortcuts? Usually because progress has slowed, a pen is hard to find, or cost is on their mind. Each of those has a licensed answer. For slowing progress, work through the plateau checklist and talk to your prescriber. For supply, check availability and order in good time. For cost, a reorder discount applies to every future order.

Counterfeit and unregulated products are a related risk. If you see someone offering to supply Mounjaro without a consultation or prescription, treat it as unsafe. 10 · Pace of results

What pace of results to expect around 10mg

Think in months, not days. Weight loss on Mounjaro is gradual by design, and trials measured results at 72 weeks.1 By the time you reach 10mg, you have usually been on treatment long enough to have a feel for your own pattern.

The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.1 In SURMOUNT-1, average weight loss built month by month, with the full averages measured at 72 weeks, not at an early point.2 Slower early progress does not predict failure, and the dose you respond to may still be ahead of you.1

Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 That is the formal checkpoint, and it is the reason a prescriber looks at tolerance as well as strength. The highest tolerated dose might be 10mg for you, and that is a valid answer.

Beyond the scales, the trials also looked at other measures. Weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.1 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.

That wider view helps when you are weighing a step to 12.5mg. If your waist, energy, blood pressure readings and general health are moving the right way on 10mg, there may be less reason to go higher, even if the scales are moving more slowly than before. If you are not seeing improvement anywhere, that is a stronger prompt to talk.

Keep records, whether in a notebook or the GLPTrackr app, and bring them to your review. Weekly trends, measurements and notes about appetite and side effects give your prescriber much more to work with than a single recent weight. Mounjaro is used alongside a reduced-calorie diet and increased physical activity, and individual results vary.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

12 · NHS, private and ordering

Mounjaro is available on the NHS, but access is being phased in over several years.
