The step after 7.5mg is 10mg
The next strength after 7.5mg is 10mg, taken weekly. Mounjaro comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Once you are on 7.5mg you have already completed most of the ladder, and the remaining strengths are 10mg, 12.5mg and 15mg.
The licensed titration works like this. Everyone starts on 2.5mg weekly. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 So 10mg is the next rung, but it is a permitted step, not an automatic one.

Should you step up or stay on 7.5mg?
You should step up only if your prescriber agrees it is right for you, and staying put is a perfectly legitimate choice. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Your prescriber will guide the right pace.1 That means 7.5mg is often a stepping stone, though it can also be where someone holds while side effects settle or progress continues. Questions worth asking yourself before you request an increase: Is my appetite still being controlled on the current dose? Are side effects manageable? Is my weight still trending down over weeks, rather than judged on a single weigh-in? Plateaus of a few weeks are normal as the body adapts to a lower weight, so a short stall alone is not proof you need more medicine. If you are not yet at the maximum, the sensible step is to discuss with your prescriber whether a step-up is appropriate, after reviewing food intake, protein, activity, sleep and alcohol.1

The full Mounjaro dose ladder, rung by rung
Mounjaro (tirzepatide) is a weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 The key phrase is depending on how you respond and tolerate it. The ladder sets the maximum speed, not the required speed.
That is why the answer to the question for 7.5mg is straightforward on paper but personal in practice. On paper, 10mg follows. In practice, your prescriber looks at how you feel, how your appetite behaves and how your weight is trending before agreeing to move up.
Where you are on the ladder also shapes expectations. The 2.5mg weeks are an adjustment phase, and most measurable progress builds from 5mg onward, month by month.
Each pen holds four weekly doses, so a pen lasts about a month at a single strength. If you move from 7.5mg to 10mg, you will switch to a new pen at that strength rather than adjusting the old one. The pen is set by turning the dose knob until the indicator shows the right number in the window, and you should always confirm the strength on the label and in the window before injecting.1

05 · Holding versus stepping up
Holding on 7.5mg versus stepping up to 10mg
Holding on 7.5mg is a legitimate decision, and stepping up to 10mg is another. Neither is a failure or a shortcut. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1
Reasons your prescriber might hold you on 7.5mg for longer include side effects that have not yet settled after the last increase, or progress that is still going well on the current dose. A prescriber can hold a dose longer or step down within the licence if side effects are the concern.1 That flexibility sits within the licensed ladder, so you never need to improvise your own schedule.
Reasons your prescriber might agree to step up include appetite returning to its earlier level, a sustained stall in weight that is not explained by diet drift, and good tolerability of the current dose. Before an increase, the sensible checks are honest ones: have portions crept back up, is protein and activity being protected, and are sleep and alcohol working against you? Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal.1

06 · Results by dose in SURMOUNT-1
What SURMOUNT-1 showed at different doses
The main trial evidence reports results at 5mg, 10mg and 15mg, not at every strength on the ladder. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
The trial reported results at 5mg, 10mg and 15mg only. What the figures do show is that the averages at 10mg were higher than at 5mg, and the averages at 15mg higher again.
The responder figures tell a similar story. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Those are group outcomes after more than a year on treatment, not what to expect within weeks of a dose change.
For a worked example, for someone starting at 100 kg, 15% is about 15 kg, 19.5% is about 19.5 kg and 20.9% is about 21 kg. That is straight arithmetic of the trial percentages, not a prediction for you, and it says nothing about how quickly the loss happens.
Trial averages do not predict your own result. Some people respond strongly on a lower dose, while others need to climb higher, and both patterns are normal. That is why prescribers treat the choice of dose as individual rather than as a race to the top.
Beyond the scales, weight loss on tirzepatide came in trials with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they explain why weight management is talked about as health management.
If you have not seen the response you hoped for, talk to your prescriber rather than pushing the dose yourself. A step to 10mg is one option among several, alongside reviewing food, activity and sleep.
07 · Pace of results
Realistic pace: what to expect around the 7.5mg stage
Weight loss on Mounjaro is gradual by design, so a dose change should be judged over weeks and months. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1
Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. In SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12.2 A sensible expectation on a standard titration is mid single digits as a percentage of body weight at that point, with wide individual variation. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.
If you have reached 7.5mg, you are likely beyond that early stage. It is reasonable to ask whether progress has been steady and whether the next increase is needed. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1
That review point matters when thinking about the next dose. It is based on the highest dose you can tolerate.
When a step up does happen, expect a short settling-in period. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 Experiences differ widely: some people notice appetite change and early loss quickly, while others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend matters more than any weigh-in.
Many people choose a weekend morning for their injection so that early side effects fall on days off. There is no best time of day: Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week.1 If side effects feel more than mild, or new symptoms worry you, contact your prescriber, and you can report suspected side effects through the Yellow Card scheme.19
08 · Using a new strength safely
Using the new 10mg pen safely
When you move up, the practical change is a new pen at a higher strength, and the injection routine stays the same. Mounjaro is injected under the skin (subcutaneously) weekly, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1
The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window. You confirm the dose by the number in the window, never by counting clicks. Each pen contains four weekly doses, and the pen will not let you dial past the medicine left inside. If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.1 Follow the pen's instructions for the injection itself, and the Patient Information Leaflet remains the reference.13
Check the strength on the box before you start the new pen, particularly if you still have part of a 7.5mg pen. Do not combine remaining medicine from different pens, and do not use leftover 7.5mg doses alongside the new strength.
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 This applies on the day you step up as much as at any other time.
You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 If you plan to change day at the same time as a dose increase, mention it to your prescriber so that the timing is clear.

09 · Storage and travel
Storing and travelling with your new pen
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1 These rules are the same at every strength, so a new 10mg pen is stored just like your 7.5mg one.
Unopened pens should be kept in the fridge, and they keep until the expiry date printed on the box. Because dose changes often mean you hold a spare pen for a while, that matters: store the next strength in the fridge until you start it. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.1
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates use an insulated pouch, and check destination rules for travelling with prescription injectables before you fly.1
If a trip falls on the week you plan to step up, think ahead. Starting a new strength while travelling is possible, but you may prefer to settle the change at home first, so ask your prescriber about timing. Your injection day can be moved if there are at least 3 days between two doses.1

If you ever stop treatment, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1
10 · How tirzepatide works
Why the dose matters: how tirzepatide works
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.
This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1
Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 This applies to the pill only — implants, coils and injections are unaffected. Similarly, if you take insulin or a sulfonylurea, doses may need adjusting because of the risk of low blood sugar, and your prescriber can advise on that.

11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
12 · NHS, private and ordering
NHS or private: who manages your dose changes
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.6 Whichever route you use, dose steps should be agreed with a prescriber.
