What changes is the question you are asking. During titration the aim was loss. At goal weight the aim becomes keeping the weight off, and that needs a plan.

Mounjaro is a weekly injection that works alongside a reduced-calorie diet and increased physical activity. It does not cure the biology behind weight gain. That is why the plan at goal weight matters more than the moment you reach it.14

03 · What stopping does

What the evidence shows about stopping

Weight tends to return after stopping tirzepatide. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14

These are trial averages, and trial averages do not predict your own result. Some people regain less, some more. What the finding shows is the direction of travel: the medicine controls appetite while you take it, and appetite tends to return when you do not.

That does not mean you must stay on treatment forever. It means stopping should be a planned step rather than something that simply happens. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.14

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 It also means appetite changes arrive gradually rather than overnight, so judge the effect of stopping over a good while, not a few days. 04 · Maintenance dose planning

Planning a maintenance dose

The dose ladder runs from 2.5mg to a maximum of 15mg weekly, with steps of 2.5mg at a minimum of every four weeks, depending on how you respond and tolerate it.1 Stepping down simply uses the same ladder in the other direction, under clinical guidance.

A sensible approach looks like this. Agree the plan with your prescriber before you change anything. Then watch your appetite and your weight trend over a period of weeks, not days. If hunger returns strongly or the trend rises steadily, discuss it rather than guessing.

Trials measured results at 72 weeks, and the pace of loss is gradual by design, so think in months, not days, and judge progress by trend, not individual weigh-ins.1 The same applies to maintenance: a single heavier morning is not regain. Weigh at consistent times, look at the direction over several weeks and keep notes.

05 · Diet and activity

Habits that carry maintenance

Diet and activity are the part of treatment you keep for life, whichever route you choose. Increased physical activity is part of the licence, not an optional extra. Nothing special is required: the evidence-backed pattern is regular moderate activity, such as brisk walking, plus resistance work for muscle. Start where you are and build. On injection days with early side effects, lighter movement still counts. Exercise also protects against regain if you later stop treatment.

Food habits matter just as much. While appetite is quieter you have a window to practise the routines you will need later: regular meals, sensible portions, protein at meals, planning ahead and noticing the difference between real hunger and habit. Treat the months at goal weight as rehearsal time, not as a reward period.

If you are heading for a planned exit, these habits should already feel normal before the dose drops. If they only exist on paper, the exit is probably too early. Mounjaro is used alongside a reduced-calorie diet and increased physical activity, so maintenance without either is working with half the toolkit.14

06 · What not to do

Shortcuts to avoid at goal weight

The most common mistake at goal weight is self-adjusting doses to save money or cling to the result. Mounjaro is licensed as a once-weekly medicine, and its blood levels are designed around a seven-day cycle.

Talk to your clinical team rather than self-adjusting.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

07 · Stopping and restarting

Stopping, then restarting

Restarting after a break should always go through your prescriber, who decides the dose and checks suitability. Everyone begins Mounjaro on 2.5mg once a week, and that starting dose is about letting your body adjust.1

Because tirzepatide takes four to five weeks to clear, a short gap does not mean the medicine has fully left your system. Contact your prescriber, tell them how long you have been off and what has happened to your appetite and weight, and follow their instructions.

Repeated stop-and-start cycles are worth discussing openly. If the pattern keeps repeating, agree a longer-term plan with your prescriber rather than cycling on and off. 08 · Response and side effects

Long-term response and side effects

Your prescriber reviews how you respond to treatment, and goal weight is a natural moment to review both effect and tolerability. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

On side effects, the first weeks bring the commonest ones. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 The most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort, and for most people they ease within days to a few weeks.1

In SURMOUNT-1, 4.3% of people on 5mg, 7.1% on 10mg and 6.2% on 15mg stopped treatment because of side effects, compared with 2.6% on placebo.2 If you notice anything new or concerning, speak to your prescriber and report it through the Yellow Card scheme.19

09 · NHS and private routes

NHS and private routes at maintenance

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.46

10 · Storage while maintaining

Storing pens during maintenance

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

If you change dose, finish or discard pens as your prescriber advises and keep the sharps bin we include with every order. Do not carry spare strengths around without labelling them, as the strength is shown on the pen label and in the dose window.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
