Mounjaro does not have a built-in finish line. The pen delivers the same licensed dose each week until you and your prescriber agree to change it. Reaching your goal is therefore a decision point, not an event the medicine reacts to.1

04 · Target weight in context
What reaching your target weight means for the medicine
Reaching your target weight means your goal on the scales has been met; it does not change how the pen works. Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 That action continues for as long as you keep taking it at your current dose.
In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 This is worth holding in mind when you plan maintenance. Part of what has made your eating easier is the medicine itself. If you reduce or stop it, you are changing one of the supports your routine has relied on, so the plan around it matters.
Mounjaro is a prescription-only medicine supplied after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity. Individual results vary. Those same two habits are the part of your plan that stays with you whatever you decide about the medicine.
It also helps to be clear about what a target weight is. Many people choose a number at the very start, before they know how they will feel at a lower weight. Once you get close, you may find that your number was too high, too low, or simply the wrong way to measure success. Some people care more about how their clothes fit, their energy, their mobility or their health checks. There is nothing wrong with revisiting the goal with your prescriber.
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. They do not predict your own result, and they say nothing about what happens once you stop.
So the practical meaning is simple. Your target is a marker for a conversation. Treat it as the moment to ask your prescriber what maintenance should look like for you, rather than as the moment the medicine has done its job and can be dropped without a plan.
05 · Hold, step down or stop
Hold your dose, step down or stop: how each works
The licensed route for maintenance is to work with your prescriber on a dose, not to improvise one.
Holding your dose. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1 If your appetite is controlled, side effects are manageable and your weight trend is steady, staying put is a reasonable choice to discuss. It is also the simplest, because nothing about your routine changes.
Stepping down. The dose ladder runs from 2.5mg through 5mg, 7.5mg, 10mg and 12.5mg up to 15mg. Stepping down is a clinical decision, so agree the new dose, the timing and how you will review progress before making any change.
Planned exit. A planned exit means agreeing a stopping point with your prescriber and putting diet and activity support in place first. It is different from simply running out of pens. The medicine takes about four to five weeks to clear after your final dose, and there is no way to speed this up. Nothing needs doing while it clears, but it does mean the change in appetite is gradual rather than sudden.
Which route suits you depends on things only you and your prescriber can weigh up: how long you have been on treatment, how you tolerate your dose, what your eating habits look like now, and how confident you feel. Many people find it helps to write down what a normal week of meals and movement looks like before they change anything, so they have a baseline to compare against.
Whichever you choose, schedule a follow-up. A review a few weeks after a change gives you both a chance to look at the trend and adjust if needed. 06 · Stopping and clearance
What happens in your body after your final dose
After a final dose, tirzepatide has a half-life of about five days, so it takes roughly four to five weeks to clear the system.1 There is no way to speed this up, and nothing needs doing while it clears. That gradual clearance is why stopping tends to feel like a slow fade in the medicine's effects rather than a switch.
Because the medicine helps regulate appetite and slows how quickly the stomach empties, those effects are the ones that wind down as the levels fall.1 The sensible approach is to expect your appetite to feel different over the following weeks, and to plan for it rather than be surprised by it.
Here is a practical way to prepare, framed as questions for your prescriber rather than medical instructions:
- How should I monitor my weight trend after stopping, and how often?
- At what point would you want to see me again?
- If my appetite changes, what are the options within the licence?
- Should I step down first rather than stop from my current dose?
It is also worth knowing what you do not need to do. You do not need to keep pens, finish a pen out of the fridge, or take any special product while the medicine clears. Pens that are no longer needed should be disposed of safely, and your sharps bin guidance came with your order. Keep the leaflet that came with your pen, because the patient information leaflet is the reference for any new symptoms.13
If you do notice something new after stopping or at any point, speak to your prescriber, and report suspected side effects through the Yellow Card scheme.19
If you restart later, you should do so through a new prescriber review rather than from leftover pens. Your prescriber will advise which dose is appropriate, and the dose ladder begins low for tolerability. 07 · Reaching 15mg
What happens when you reach 15mg and still have weight to lose
Fifteen milligrams is the top of the licensed ladder, so there is no higher dose to move to. The dose steps up by 2.5mg at a minimum of four-week intervals, up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Once you are there, the question stops being how to increase and becomes whether to continue, adjust or stop.
Everyone starts on 2.5mg once a week, and that starting dose is about letting your body adjust, not about weight loss itself.1 Many people never need to reach 15mg at all. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1
If you are on the top dose and not seeing the progress you hoped for, your prescriber reviews it. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1
Weight loss on Mounjaro is gradual by design. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1

08 · Choosing a realistic target
Setting and revisiting a realistic target weight
A realistic target is one you set with your prescriber, based on your starting point, how you respond and what you can sustain. Weight loss on Mounjaro is gradual by design: the first weeks on 2.5mg are an adjustment phase and losses are usually modest, and most measurable progress builds from 5mg onward, month by month.1
That pace is relevant to your target for two reasons. First, if you expect to reach your goal quickly you may feel disappointed or push for a higher dose than you need. Second, if you are close to your goal, the final stretch may feel slower than the middle did. This is normal, and a slower trend near the end does not mean the medicine has stopped working.
Some practical ways to think about your target:
- Use a range, not a single number. A range lets you hold steady without feeling you have failed if you drift slightly.
- Judge by trend. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1
- Look beyond the scales. Clothes fit, energy, mobility and health checks all matter and may be what you care about most.
- Revisit at reviews. Your goal can change as you learn how you feel at a lower weight.
The medicine is licensed for weight management in adults, and the NICE guidance sets out who is eligible on the NHS: adults with a BMI of at least 35 plus at least one weight-related condition.4 Private treatment follows the licensed criteria: a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Those thresholds apply to starting treatment. Your target weight and your stopping plan are separate decisions between you and your prescriber.

It is also fine to decide that you are happy where you are, even if you have not hit the number you wrote down at the start. A sustainable weight that you can hold, with a dose you tolerate, is a perfectly good outcome. The aim of maintenance is stability, not an ever-lower figure.
09 · NHS and private routes
NHS and private routes at the maintenance stage
Whether you are treated on the NHS or privately, the maintenance questions are the same: hold, step down or plan an exit, with your prescriber. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46
Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. If you are on the NHS, your GP or weight-management service sets your review schedule, and you should follow their plan. If you are private, the pharmacy's prescribing team does the same job.
Where you get treatment matters for safety as well as cost. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide. Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.712
This matters at the maintenance stage because people who are nearing their goal, or who are worried about cost, can be tempted by cheap offers. Anyone offering to supply without a consultation or prescription is not a safe source. 10 · Practical care in maintenance
Practical care while you are on maintenance
While you stay on treatment, the everyday rules still apply, and they are easy to relax once you have hit your goal. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1
Storage is the other area where habits slip. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Keep the pen cap on to protect it from light.1
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription.1 A genuine pen has clear, colourless solution. If anything about the supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.12

11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
