04 · How GLP-1 works

How GLP-1 medicines work in the body

GLP-1 is a natural gut hormone, and medicines in this class copy its action to influence appetite and digestion. Mounjaro goes one step further. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1

Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 Those three actions explain almost everything a GLP-1 medicine is licensed to help with. Appetite regulation supports eating less. Slower stomach emptying keeps you feeling full for longer after a meal. Better insulin response matters for blood sugar in people with type 2 diabetes.

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 Notice the wording: the medicine supports a diet, it does not replace one. People who treat the pen as the whole plan often find it harder to build habits that last.

Single-action and dual-action medicines

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1 Wegovy and Mounjaro are both effective licensed treatments, and the better choice depends on your health, tolerance and preference. Why one injection a week

Mounjaro is taken once a week. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. 05 · Weight management evidence

What the weight-loss trial actually showed

SURMOUNT-1 is the main weight-management trial of tirzepatide. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 A trial average describes a large group. It does not predict your own result, and some people lose less while others lose more.

Beyond the average

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 The comparison group on lifestyle changes alone lost far less, which is why the trial supports the medicine as an addition to diet and activity.

A worked example, not a forecast

To make a percentage tangible: for someone starting at 100 kg, 20.9% is about 21 kg. That is straight arithmetic on a trial average, not a target.

Many people ask how much they will lose early on. The honest answer is that early progress varies and trial figures are measured over a much longer period. Dose also matters: treatment starts low and steps up, so the early weeks are about getting used to the medicine rather than reaching full effect. Blood sugar and type 2 diabetes

Type 2 diabetes is the second licensed use for Mounjaro in the UK.1 The reason it fits is the mechanism: the medicine helps improve the way the body responds to insulin.1 NICE has published separate guidance on tirzepatide for treating type 2 diabetes, which is the route used within NHS diabetes care.8

Why this matters for you

If you already have a diabetes diagnosis, treatment decisions belong with your GP or diabetes team. They know your medicines, your monitoring and your history. A private consultation does not replace that relationship, and you should tell any prescriber about every medicine you take.

Some diabetes medicines, notably insulin and sulfonylureas, may need their doses adjusting alongside a GLP-1-based medicine because of the risk of low blood sugar. The licences are separate and the monitoring is different.

Belly fat, face, hair, skin and other claims

Beyond weight management and type 2 diabetes, most claims about GLP-1 medicines are not established licensed uses.

Belly fat

The weight-loss trial measured overall body weight. Where the body loses weight from is individual.

Your face and ageing

People ask whether GLP-1 changes the face or ages it. Any large change in body weight, however it happens, can alter how the face looks. If you notice something that concerns you, discuss it with your prescriber and report side effects through the Yellow Card scheme.19

Hair, eczema, alcohol and drainage

A rule of thumb

If a claim sounds too broad, it probably is. A medicine that supports appetite and insulin response will not necessarily improve every part of your health. Treat dramatic claims with caution, especially from sellers. Anyone offering to supply without a consultation or prescription is a warning sign, and genuine treatment is supplied only after a prescriber review.

08 · Downsides and safety

The downsides of taking GLP-1 medicines

Every medicine has downsides, and a GLP-1-based medicine is no exception. Most side effects are digestive: the most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort, and they are most likely when starting treatment or moving up a dose.

Why digestion is the common topic

Because the medicine slows how quickly the stomach empties, digestive effects are the area people most often raise with their prescriber.1 If effects are troublesome, tell the prescribing team. They can talk through whether the dose, pace of increase or eating habits need adjusting. Reporting problems

If you notice a new symptom, speak to your prescriber. If you have severe, persistent stomach pain, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or swelling of the face, lips, tongue or throat with difficulty breathing, get medical help straight away. You can also report suspected side effects through the MHRA Yellow Card scheme.19 Do this even if you are unsure the medicine is the cause. Tell your prescriber about any new or lasting symptom, even if it is not one of the commonly listed side effects.

Contraception and other medicines

The MHRA has issued guidance on contraception with GLP-1/GIP medicines.7 Because stomach emptying slows, the medicine can reduce absorption of oral contraceptives. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Ask your prescriber what that means for you, and tell them about any medicine you take.

09 · NHS and private access

NHS access versus private treatment

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Who is the NHS route for?

If you are in a group the rollout is reaching, your GP or local service is the place to ask. NHS care comes with its own pathway and support. NHS access to Wegovy runs through specialist weight-management services; ask your GP.

Why some choose private care

People who meet the licensed criteria but are not yet reached by the rollout may prefer to start sooner. A private route still needs a proper clinical review. 10 · Who may not be suitable

Who should be cautious, and why a review matters

Not everyone is a suitable candidate for GLP-1-based treatment, and a prescriber review exists to catch that. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity.

Why the review is not a formality

The consultation is a short health questionnaire with no charge and no obligation. A prescriber then reviews it; it is not a scoring script. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. That is deliberate: treatment is not suitable for everyone.

Things to tell your prescriber

Be open about your medical history, every medicine you take, any plans for pregnancy, and previous reactions to medicines. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Adults only, and licensed use only

The weight-management licence is for adults. Use the pen exactly as your prescriber and the leaflet direct.

Supplements are not equivalent

Low-cost supplements marketed as natural alternatives have no clinical oversight and are not the same as a licensed medicine. Dosing is not standardised, and we do not suggest comparing them with a prescription treatment. If a seller makes dramatic weight-loss claims, treat them with scepticism.

11 · Starting and day-to-day use

What starting treatment looks like

Treatment starts with a low dose and steps up, and the practical details are in the leaflet and from your prescriber. Mounjaro is taken once weekly with a pre-filled KwikPen, and the six strengths run from 2.5mg to 15mg.1

Storage

Store pens in the fridge at 2–8°C until first use; unopened pens keep until the printed expiry date. Once in use, a KwikPen is licensed for up to 30 days (not above 30°C) out of the fridge.13 If you have a question about an unopened pen being out of the fridge, check the patient information leaflet and contact the pharmacy.

Injecting

Follow the pen's instructions for how to inject. Needles, a sharps bin and injection guidance are included with every order, so you are not left to work it out yourself.

Eating and activity

Because the medicine makes most people feel fuller sooner, meals often get smaller. Focus on food that is satisfying and nourishing, and keep moving in ways you enjoy. If you stop

After a final dose it takes roughly 4–5 weeks to clear the system, and nothing needs doing while it clears.1 Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load. Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
