Reviews can tell you what treatment feels like in daily life, but they cannot tell you what you will lose or how quickly. People who write reviews are usually those with a strong opinion, either delighted or frustrated, so the middle ground is under-represented. Reviews also rarely state the dose, how long someone has been treated, or whether they changed their diet and activity alongside the pen.

The pattern most reviews of Mounjaro and Wegovy describe is consistent with how the medicines work. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹ That is why reviewers often talk about quieter cravings and less snacking.

Early effects are also part of the picture. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.¹³ A review written in the middle of an unsettled week will sound very different from one written after the body has adjusted, even though both people are describing the same medicine.

Reviews versus clinical evidence

Clinical trials are the right tool for average results, while reviews are the right tool for lived experience. The two answer different questions, and mixing them up is the commonest mistake readers make. A review might say someone lost a great deal quickly; a trial tells you what happened across a large, monitored group over a defined period.

The headline tirzepatide evidence comes from SURMOUNT-1. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.²

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.² Notice what that tells you: many people did very well, but trial averages do not predict your own result, and a minority will respond less than the average.

For someone starting at 100 kg, 20.9% is about 21 kg. That is simple arithmetic on a trial average, not a forecast of what will happen to you, and it says nothing about how quickly any given amount comes off.

Why a review will rarely match the trial

Several things explain the gap. Trial participants were followed carefully, took the medicine on a set schedule and received lifestyle support. A reviewer may have missed doses, stayed on a low dose, stopped early because of side effects, or written the review before the treatment had time to work. None of that makes their experience false; it just makes it a poor basis for predicting yours.

Reviews can also be more positive than the evidence for a different reason: people who feel they have succeeded are keener to share. Equally, someone who had an unpleasant first fortnight may write an angry review long before the body adjusts. Reading a spread of feedback, rather than a handful of extremes, gives a fairer picture.

What the guidance says about who should be treated

Reviews are also silent on eligibility. NICE recommends tirzepatide for adults with a BMI of at least 35 plus at least one weight-related condition, and that guidance shapes NHS access.⁴ Private treatment follows the licence, which a prescriber assesses in your consultation. A glowing review from someone with a very different starting point is not a reason to expect the same outcome.

The sensible approach is to use reviews to prepare for the experience and the trial data to set realistic expectations.

Side effects and the early weeks

The side effects described in reviews are mostly early, mostly mild and usually settle, but they are real and worth planning for. The most common early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.¹³ If a reviewer describes exactly this, they are describing the expected pattern.

The first weeks vary widely. The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.¹ This is why one reviewer can say nothing happened for weeks while another reports quiet cravings almost at once.

Why side effects happen

The mechanism helps explain the pattern. Tirzepatide slows how quickly the stomach empties, which is part of why people feel fuller sooner and less hungry between meals.¹ Side effects are most likely when starting treatment or moving up a dose, which is one reason Mounjaro uses a gradual dose ladder.

Timing the injection around side effects

There is no "best" time of day — Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember (many people choose a weekend morning so early side effects fall on days off) and stick to it.¹ If a reviewer mentions picking a weekend for this reason, that is a sensible, licensed approach.

Reviews that mention symptoms the label does not

Sometimes reviews describe unusual symptoms and attribute them to the pen. A symptom that is not on the patient leaflet's list is not a listed side effect. Discuss anything new with your prescriber, and report suspected side effects through the MHRA Yellow Card scheme.¹⁹ Reports from you help regulators monitor safety, which is more useful than a one-line review.

When to speak to someone rather than read more reviews

If side effects are making daily life difficult, do not rely on forum advice. A balanced reading of reviews, then, is this: expect an adjustment period, expect it to be milder for most people than the worst reviews suggest, and know that you can get clinical support if it is not. Individual experience varies, and your prescriber is the right person to interpret yours.

The dose ladder and realistic expectations

Most reviews make more sense once you know that treatment starts low on purpose. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.¹ A reviewer who judges the pen after a few weeks on 2.5mg is judging the adjustment phase, not the treatment at full effect.

After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.¹

Thinking in months, not days

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.¹ That is the single most useful lens for reading any review: ask what stage of treatment the reviewer was at.

When a prescriber reviews your response

Response is checked, not assumed. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.⁴ This is why prescriber involvement matters: a review from someone with no oversight cannot tell you whether they were responding well, or whether they should have changed course.

Titration cost versus a review's headline

Reviews that mention cost often skip the structure of titration. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £879.96 at standard prices. A first year costs £2,399.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,279.88 if you step up through every dose to 15mg, again at standard prices before discounts. Why a reviewer's dose matters

A reviewer on a high maintenance dose is describing a very different treatment stage from one on the starter pen. If a review does not say which, treat its conclusions cautiously and use it, at most, as a hint about what the stage feels like rather than what you will achieve.

07 · Using the pen day to day

Using the pen day to day

Convenience is among the most-praised features in reviews, and the licensed routine explains why: it is a once-weekly injection. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.¹ Each KwikPen contains four weekly doses.¹

Where to inject

Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.¹³ Needles, a sharps bin and injection guidance are included with every order.

Missed doses and changing day

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.¹³ You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.¹³

Storage and travel

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.¹

You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.¹

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.¹³

Which weight loss pen is right for you?

There is no honest one-word answer, because effectiveness depends on the person, the dose and how long treatment continues.

Mounjaro (tirzepatide)

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).¹

Wegovy (semaglutide)

Wegovy pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly; titration steps up after at least four weeks, reaching the 2.4mg maintenance dose from week 17. The 2.4mg dose is the licensed maintenance dose for weight management.¹⁸ Wegovy is a respected, licensed option, and for some people it is the right one. Liraglutide

Liraglutide, supplied as Saxenda and in products such as Erly, is supplied at a concentration of 6mg per millilitre; it is a daily injection, unlike once-weekly Mounjaro and Wegovy. Beyond that, figures vary and your prescriber can advise on how it compares for you.

Tablets and supplements

GLP-1 tablets exist; see current Wegovy tablet prices on the relevant product page. Supplements marketed as "natural" alternatives are low-cost supplements with no clinical oversight, and dramatic weight-loss claims about them are not backed by the licensed evidence that applies to prescription pens. Dosing is not standardised for supplements, which is one of the reasons they are not equivalent to a licensed medicine.

How to choose

Choose by suitability, not by the loudest review. Ask your prescriber about your health conditions, other medicines, tolerability and preferences. NHS versus private access

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).⁴⁶

Safety, and what to avoid while on treatment

The most useful safety advice is practical, and reviews rarely cover it.

Do not alter doses

Altering doses outside the licence means unknown dosing accuracy and no safety data.¹

Do not double up

Never take two doses within 3 days of each other.¹³ If you are unsure whether a dose is overdue, contact the prescribing team.

Do not freeze or mishandle pens

Never use a pen that has been frozen, keep the cap on to protect it from light, and discard an in-use pen after 30 days (not above 30°C).¹

Take care with contraception

Mounjaro can reduce absorption of oral contraceptives, because stomach emptying slows.⁷ The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.¹ Speak to your prescriber about the right contraceptive approach before or when starting, and again when doses change.

Tell your prescriber about diabetes medicines

If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar. That does not mean the medicines must never be combined; it means your prescriber should be involved.¹

Avoid unregulated sellers

Counterfeit and unregulated weight loss pens are a genuine hazard. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme.⁷ Be wary of anyone offering to supply without a consultation or prescription, of social media sellers, and of prices that look implausibly low. Avoid reviews that promise results

Dramatic weight-loss claims, miracle language and promises of specific results are red flags in a review. The licensed evidence shows trial averages, and trial averages do not predict your own result. Genuine reviews describe a mixed, gradual experience.

Report anything unexpected

If you notice something new after starting treatment, discuss it with your prescriber and report suspected side effects through the Yellow Card scheme.¹⁹ Good safety reporting is far more valuable than a review score, and it is how regulators learn about rare effects.

Viral lifestyle tricks versus a licensed pen

The role of diet and activity

Weight loss pens are not a replacement for lifestyle change. Mounjaro is used alongside a reduced-calorie diet and increased physical activity, and in SURMOUNT-1 the trial results were measured alongside diet and exercise.² Mounjaro helps regulate appetite and slows how quickly the stomach empties, so most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹ Eating patterns and daily movement still matter, and any approach you can sustain is better than a fashionable one you drop.

Why trends are not treatment

Viral methods usually come with dramatic claims and no oversight. They may suit some people as habits, such as walking more or eating a varied, balanced diet, but "superfoods" do not burn fat on their own. Be sceptical of any single food or trick presented as the answer; the same scepticism you apply to a review should apply here.

Walking and movement

Increased physical activity is part of the licensed use of the pen, so a regular walking routine is consistent with treatment. Your prescriber can discuss what is appropriate for you, particularly if you have other health conditions.

Food noise and snacking

Reviews often mention quieter cravings, sometimes called "food noise". That is a description of lived experience rather than a clinical term in the product information. What the product information supports is that most people feel fuller sooner and less hungry between meals.¹ Use that as a guide to what to expect, not as a promise.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
