Do weight loss pens work?

For most people who use a licensed pen as prescribed, yes: the trial evidence shows meaningful average weight loss compared with lifestyle changes alone. The key words are licensed, prescribed and alongside diet and activity. Mounjaro (tirzepatide) is a once-weekly injectable prescription medicine made by Eli Lilly, licensed in the UK for weight management in adults.1

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Some people lose a great deal, some a modest amount, and a minority see little benefit. That spread is normal in every weight-management trial, and it is why your prescriber reviews how you are responding rather than promising an outcome.

The phrase "weight loss pen" covers several different things, and they do not all work equally well.

Is a pen relevant to you, and what next?

A pen is worth considering if you have struggled to lose weight with diet and activity alone and are an adult looking for medically supervised help. It is not for everyone, and only a prescriber can decide suitability after reviewing your health history and current medicines. Pens are always used with a reduced-calorie diet and increased physical activity.1

There are two practical questions. First, can you access treatment through the NHS? Mounjaro is available on the NHS, but access is being phased in over several years. 03 · How pens work

How do weight loss pens work?

Weight loss pens work by copying natural gut hormones that tell your brain you have eaten enough. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. That is the real mechanism: the pen makes eating less easier, but you still have to do the eating less.

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1 Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and a regulated UK pharmacy will not supply Ozempic for weight loss. 04 · What the trials show

What the trials actually measured

The headline trial for tirzepatide in obesity is SURMOUNT-1. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

Averages hide the spread, so the trial also reported how many people reached set thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

Two honest cautions follow. These are trial averages, individual results vary, and they describe people supported with diet and exercise advice. They are also not a direct comparison with trials of other medicines: different studies recruit different people and measure over different durations.

05 · Realistic pace

How fast do weight loss pens work?

Pens work gradually by design, so expect months rather than days. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.2

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Reaching the top of that ladder takes months, which is why early results are rarely dramatic.

What about the first three months?

This is one of the most common questions, and the honest answer is: less than the headlines. A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 Three months in, most people are still on the lower rungs of the dose ladder, and the full averages in the trial were measured at 72 weeks, not at three months. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.

The first weeks

Experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13

There is no best time of day to inject. Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week.

06 · When progress stalls

What if the pen seems not to be working?

A slow start or a stall does not automatically mean the pen has failed. Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight. Within the licence, the sensible responses are to review food intake honestly because portions drift back up, protect protein and activity, check the basics such as sleep and alcohol, and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.2

A plateau is not a reason to double doses or add unlicensed products.

There is also a formal checkpoint. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.2 That is the honest answer to the "does it work for me" question: it is measured, not assumed.

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1

07 · Beyond the scales

Do pens change anything beyond weight?

In trials, they did. Weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2

These are trial-average secondary outcomes, not promises, and your own results may differ. But they explain why prescribers talk about weight management as health management rather than appearance. If you have a weight-related condition, a prescriber is more interested in how your overall health markers move than in a number on the scale alone.

It also explains why a pen is offered with ongoing review rather than as a one-off purchase. Medicines that alter appetite and metabolism need follow-up, dose decisions and a plan for what happens next, not just a delivery.

08 · Safety and side effects

Side effects and long-term safety questions

Pens are prescription medicines, so side effects are part of the picture. The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.13 The patient information leaflet lists the full set of side effects, and you should read it before starting.

If you have questions about long-term side effects or alcohol, ask your prescriber. There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and emptier stomach can hit harder than usual, and alcohol adds calories that work against your goals. If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar. Your prescriber can answer questions about long-term side effects for your health history, and the patient leaflet lists the full set of side effects.13 If you notice a new or lasting symptom that is not on the leaflet list, tell your prescriber and report it through the Yellow Card scheme.19

Contraception and other medicines

The medicine can reduce absorption of oral contraceptives because stomach emptying slows, so tell your prescriber what you take.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar; this is a conversation for your prescriber, not a reason you cannot be treated.

Using the pen properly

Mounjaro is injected under the skin. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded.1

09 · After stopping

What happens when you stop using a pen?

When you stop, the medicine leaves your body over four to five weeks, and there is no way to speed this up. Nothing needs doing while it clears.1

Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 The practical advice is to talk to your prescriber before stopping, not after, so you can agree whether to step the dose down, hold it, or move to a maintenance approach. Continued eating and activity habits matter more than any single dose decision.

A genuine weight loss pen is a prescription-only medicine, supplied only after a consultation reviewed by a GPhC-registered prescriber. Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
