What does third generation GLP-1 actually mean?
It is a shorthand, mostly used in news coverage and social media, for medicines that act on more hormone pathways than the earlier GLP-1 drugs. There is no regulator-defined "third generation", so different writers draw the lines slightly differently. The most common version runs like this: single-action GLP-1 medicines came first, dual-action GIP and GLP-1 medicines came next, and triple-action candidates that also engage the glucagon pathway come after that.
The practical point is that the third step is still a research step. A medicine described this way is not something a UK pharmacy can supply on prescription, and we do not offer or recommend unlicensed products. If you see a seller offering a "third generation" or "GLP-3" product without a consultation or prescription, treat it as a red flag.

For most adults who meet the licensed criteria, waiting is not a strategy, because the future medicine has no UK licence and no prescribing route. The real choice is between treating now with a licensed medicine and not treating. Your prescriber, not a headline, decides which licensed option fits your health history.

03 · Why Mounjaro is already dual-action
Why Mounjaro is already dual-action
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1
This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic). So when a headline calls something "the next generation", it is worth remembering that the step from single to dual action has already happened and is already available. 04 · What the licensed evidence shows
What the licensed evidence shows
Completed trials of licensed medicines are the way to judge any treatment. For Mounjaro, the headline study is SURMOUNT-1. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2
These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2
For a worked example only: for someone starting at 100 kg, 20.9% is about 21 kg. That is arithmetic on a trial average, and trial averages do not predict your own result.
05 · Why triple agonists are not prescribable
Why triple agonists are not prescribable yet
A medicine can only be prescribed for weight management in the UK once it has a marketing authorisation, which follows completed late-stage trials and a regulatory review. Retatrutide, the medicine most often described as a triple agonist, has not completed that route here. It is referred to by its generic name only because it has no UK brand and no licence.
That has several consequences for you as a patient:
- No UK prescriber can legitimately prescribe it for weight loss outside a trial.
- No UK pharmacy can lawfully dispense it as a licensed medicine.
- Any product sold online under this name is, by definition, not a licensed medicine and may be counterfeit or mislabelled.
- There is no UK product information to tell you about dosing, side effects or who should avoid it.
The last point matters most. Licensed medicines come with a Summary of Product Characteristics and a patient leaflet, which set out dosing, warnings and interactions. A medicine without these has no safety framework you can rely on at home.
06 · Is GLP-3 a real thing?
Is "GLP-3" a real thing?
"GLP-3" is a marketing and social-media phrase, not an established category of medicine. Some people use it to mean triple-action drugs; others use it loosely for anything newer than Ozempic. Because it has no agreed definition, you cannot judge a product's safety or effectiveness from the label.
GLP-1, GLP-2 and other glucagon-like peptides are different natural hormones with different roles. Saying "GLP-1 is better than GLP-2" is a bit like asking whether one hormone is better than another; they are not competing treatments for the same job.
The safest rule is simple: if a product is described as "GLP-3" and offered without a consultation or prescription, do not buy it. A licensed medicine will name its active ingredient, come from a registered pharmacy, and be supplied after a clinical review. 07 · Which GLP-1 is strongest?
Which GLP-1 is best for weight loss?
No licensed medicine is best for everyone; your prescriber decides what suits you. SURMOUNT-5 is the trial that compared tirzepatide with semaglutide directly.
Doses step up gradually, at least 4 weeks on each dose, depending on how you respond and tolerate it.2 Higher is not automatically better; many people settle at a maintenance dose of 5mg, 10mg or 15mg, and your prescriber will guide the right pace.

08 · Tablets and other newer licensed options
What new weight loss drugs are coming in 2026?
Some newer options are already here as licensed products rather than future generations. Wegovy tablets (oral semaglutide) and Foundayo (orforglipron) film-coated tablets both have UK product information published in the electronic medicines compendium.2021 They are oral GLP-1 medicines, which is a change of delivery route, not a step up to a triple-action drug.
Beyond that, the pipeline contains many candidates at different stages: triple agonists, longer-acting versions, combinations and oral small molecules. Most will never reach UK patients in the form headlines suggest. 09 · Who cannot take GLP-1 medicines
Who cannot take GLP-1 medicines?
Eligibility is decided by a prescriber. For Mounjaro the licence covers adults for weight management and for type 2 diabetes, and the prescriber reviews your health history, current medicines and goals before anything is approved. Orders that turn out to be unsuitable are refunded automatically.
Some situations always need a careful conversation. You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. The MHRA advises that Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows, so ask your prescriber about contraception.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you use insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

10 · The poor man's Ozempic myth
What is the "poor man's Ozempic"?
The phrase is a social-media nickname for berberine, a low-cost supplement sold without clinical oversight. It is not a GLP-1 medicine, it is not a licensed treatment for weight management, and it is not equivalent to Ozempic, Wegovy or Mounjaro.
Supplements are not assessed in the way medicines are. Dosing is not standardised, quality varies between products, and marketing often relies on dramatic weight-loss claims that no licensed evidence supports. Nothing about a supplement being cheap or easy to buy makes it comparable to a prescribed medicine with trial data behind it.
This matters for the "generation" question too. Nicknames like this blur the line between licensed and unlicensed products, in the same way the "GLP-3" label does.
11 · NHS access today
NHS access to licensed treatment today
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
NHS access to Wegovy runs through specialist weight-management services; ask your GP. If you are unsure which route suits you, your GP is the best first call for NHS eligibility, and a regulated online pharmacy is an alternative if you prefer to start sooner.
12 · Reading headlines about new drugs
How to read headlines about new weight loss drugs
Most news about "triple" or "next-generation" medicines reports early or mid-stage trial results. A few habits help you read them sensibly.
- Check the stage. A result from a trial is not an approval, and an approval in another country is not a UK licence.
- Check the population. Trials enrol specific groups, such as people with obesity but not diabetes, and results may not carry over to you.
- A percentage means little without knowing the dose, the duration and what it was compared against.
- Check the seller. Any site offering an unlicensed product to buy now, without a consultation or prescription, is a warning sign.
None of this means progress is overhyped. Research into more hormone pathways is genuine and may well produce better options. It simply means that today's decision should rest on today's licensed evidence, which is why we point you to the medicine you can be prescribed after assessment.

13 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Get medical help straight away for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
