What does 3rd generation GLP-1 mean?

The phrase refers to a hoped-for next step in medicines that act on gut-hormone pathways. In common usage, the third generation is the group of triple-agonist drugs, with retatrutide the name you will see most often. It is still being developed, and it is not licensed in the UK, so no UK pharmacy can lawfully prescribe it.

The term is not a regulatory category. The MHRA, NICE and the BNF do not sort medicines into generations. Journalists, investors and patients invented the shorthand, so different websites draw the lines slightly differently. Treat it as a useful map, not a rule.

For most people, the answer is no, because you cannot be prescribed it.

03 · Where Mounjaro sits

Where Mounjaro sits in the generations

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

It is usually called second generation because it was the first of its kind to act on two receptors rather than one. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1

04 · What the evidence shows

What the evidence shows for a licensed second generation drug

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

Early headlines are not a licence, and results from one study cannot be set against another study's results as if they were a head-to-head test.

05 · Which is strongest

Which GLP-1 is right for weight loss?

No licensed medicine is best for everyone, and a higher generation number does not automatically mean a better medicine for you. Among licensed UK options, the question is also about tolerability, your health conditions, cost and what your prescriber considers suitable.

Mounjaro acts on two receptors where semaglutide acts on one, and that is the main mechanical difference between them.1 Only a trial that compares the two directly can tell you how they perform against each other. Our Wegovy pen page explains the semaglutide option. Its pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least four weeks to reach 2.4mg from week 17.

Liraglutide, as in Saxenda, is a daily injection supplied at 6mg/ml, unlike the once-weekly options. For a licensed-dose comparison on our side, Mounjaro has six strengths, from 2.5mg to 15mg, so the dose can be matched to how you respond. A prescriber chooses the dose, not the generation label.

06 · Safety and who can take it

Side effects, who cannot take GLP-1 medicines, and interactions

Side effects, exclusions and interactions belong to each individual medicine, so the answer depends on which product you mean. For Mounjaro, the full lists are in the patient information leaflet, and your prescriber will go through them with you.13 Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

Two interaction points are worth knowing. Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows, so ask your prescriber about contraception.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you take insulin or a sulfonylurea, your doses may need adjusting because of the risk of low blood sugar. These medicines are not simply "banned" together, but they need a clinician's oversight.1

Because suitability depends on your health history, the safest route is a proper consultation. Tell your prescriber about every medicine and supplement you take, and any past conditions. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. If you notice any new symptom while on treatment, speak to your prescriber, and you can report it through the Yellow Card scheme.19

Can two GLP-1 medicines be used together? We do not support combining them. Mounjaro must not be combined with other GLP-1 medicines, unlicensed "research" peptides (such as retatrutide, which has no UK licence), or compounded copies. These combinations have no safety data and no regulated supply chain.

07 · Stopping treatment

What happens if you stop taking a GLP-1 medicine?

The medicine leaves your system gradually after a final dose. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber.14 Treatment works alongside a reduced-calorie diet and increased physical activity, so keeping those habits in place is part of any plan.2

08 · NHS access

NHS access to licensed GLP-1 treatment

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46

Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. NHS access to Wegovy runs through specialist weight-management services; ask your GP. A medicine that is unlicensed will not be offered through either route.

09 · Spotting unsafe sellers

How to spot unsafe "next generation" sellers

Hype around future drugs creates a market for sellers who exploit it. Any website, social account or messaging contact offering a triple-agonist injection is offering something that is not licensed here.

  • Offering to supply without a consultation or prescription.
  • Calling a product "research use only" while selling it for self-injection.
  • Dramatic weight-loss claims with no named, published trial.
  • No registered pharmacy you can look up.
