Can you get prescription weight loss drugs with hypothyroidism?
Yes, you can apply, because eligibility for Mounjaro is set by BMI and weight-related conditions rather than by thyroid status. The licence covers adults with a BMI of 30 or above, or between 27 and 30 with at least one weight-related condition, alongside a reduced-calorie diet and more physical activity.1 A prescriber then looks at your BMI, medical history and current medicines before deciding.11

02 · Before you apply
What to tell the prescriber about your thyroid
Tell the prescriber everything about your thyroid history, because the consultation asks for your full medical history and medicine list.1 Be specific rather than summarising it as a minor issue.

04 · What Mounjaro is
What Mounjaro is and how it works
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 It is not a thyroid medicine, and it does not treat an underactive thyroid.
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.
That dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 This matters if you are ever asked to stop before a procedure or change a treatment plan: your prescriber can plan around it.
It is also worth knowing what Mounjaro is not. It is not a thyroid hormone, it is not a substitute for your levothyroxine or other thyroid medicine, and it should never be a reason to change or stop thyroid tablets without talking to the clinician who prescribes them. Keep taking your thyroid medicine as directed unless that clinician tells you otherwise, and make sure the Mounjaro prescriber knows exactly what you take and when.
05 · Eligibility explained
Does hypothyroidism qualify you for treatment?
Hypothyroidism is not itself a qualifying condition, and it is not an exclusion either; what qualifies you is your BMI and, in some cases, other weight-related conditions. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1
It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.1
So the practical question is not whether your thyroid status qualifies you, but whether your BMI and health picture meet the licence and whether the prescriber is happy with your overall history. Two people with identical thyroid results could get different answers, because one has a BMI of 31 and the other 26.
Ozempic is semaglutide licensed for type 2 diabetes; it is not a UK weight-loss medicine, and a regulated UK pharmacy will not supply Ozempic for weight loss. Wegovy, also semaglutide, is the weight management version, and NHS access to Wegovy runs through specialist weight-management services; ask your GP.
Three situations come up often with thyroid conditions:
- Your BMI is just under 27. The licence does not cover you, regardless of how hard weight is to lose.
- Your BMI is 27 to 30. You need at least one weight-related condition from the licensed list. An underactive thyroid is not on that list, so the prescriber will look for one of those named conditions.
- Your BMI is 30 or above. You meet the BMI part of the licence, and the prescriber then reviews your history and medicines.

06 · Thyroid safety
Is Mounjaro safe with hypothyroidism? The thyroid warning explained
The thyroid warning on tirzepatide is about a rare type of thyroid cancer, not about an underactive thyroid. In rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans, and no increase has been shown in human trials.1
As a precaution, you should tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2, and they will weigh this in your assessment.13 Routine thyroid monitoring is not required for most people.1
This is the point where many people with hypothyroidism get understandably worried. The word thyroid appears in the safety information, and it is easy to assume it applies to any thyroid condition. Hypothyroidism is not named. That does not mean no one should think about it; it means the decision belongs with a prescriber who has your full history in front of them.
Safety depends on your history, your other conditions, your current medicines and your own risk factors. Practical steps that help the review go well:
- Have your current medicine list to hand, with names and strengths.
- Know whether anyone in your close family has had thyroid cancer or MEN 2.
- Mention any recent thyroid blood tests your GP has run, in case the prescriber asks.
- Tell the clinician who manages your thyroid that you are considering treatment.
If you start treatment and notice new symptoms you are unsure about, tell your prescriber and consider reporting them through the Yellow Card scheme.19 If you have severe, persistent stomach pain, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or signs of a severe allergic reaction, get medical help straight away. Do not assume a new symptom is caused by, or unrelated to, any one medicine; get it checked.
07 · Trial results
What the trial evidence shows
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages; individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg.
What the trial does not tell us is how people with an underactive thyroid in particular fared. If you want to know how your thyroid condition might affect your own response, that is a conversation for your prescriber and your thyroid clinician, not something a headline figure can settle.
Beyond the scales, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.
How do you use these numbers sensibly? Treat them as a range of what happened in a controlled trial where everyone also received lifestyle support. Your own result depends on dose, adherence, diet, activity and your individual biology.

08 · Who should not take it
Who should not take Mounjaro
Mounjaro is not suitable for everyone, and the exclusions are clear. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.1 Mounjaro should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.1 Always share your full medical history and medicine list during consultation.
Notice that an underactive thyroid appears on neither list. That is consistent with how the eligibility works, but it is not a permission slip. Many people with hypothyroidism also have other conditions, and it is those that can change the answer. Because the prescriber sees the whole picture, they are the person to weigh it, not a general rule.
When to seek help once you are on treatment: serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.1
If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion); your prescriber may need to adjust those medicines.1
One more practical point: if you are already on several regular medicines, as is common with long-term conditions, do not skip any from the list in the consultation. Over-the-counter products and supplements count too. Leaving something out does not make the review simpler; it makes it less reliable.

09 · NHS and private
NHS or private: which route if you have hypothyroidism?
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46
Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.5
The NHS picture matters for you in two ways. First, the NHS criteria are tighter than the licence: a BMI of at least 35 plus a weight-related condition, versus a licensed BMI of 30 or above, or 27 to 30 with a weight-related condition. Someone with a BMI of 32, for example, may meet the licence but not the current NHS criteria. Second, rollout is phased, so even people who qualify may have to wait their turn.
Your GP is the right first stop to ask where you sit in the local NHS pathway. They also hold your thyroid results and can advise on how any change in weight may relate to your thyroid medicine. A few differences to weigh up honestly:
- Speed. The NHS route is phased and depends on your criteria and local capacity. Private treatment starts after consultation and approval, with same-day dispatch when ordered by 3pm.
- Cost. Private treatment is paid for by you, with pay-monthly and Klarna options and no subscription.
- Eligibility. The NHS criteria are narrower than the licence.
- Support. Either way you should keep your GP informed, particularly about your thyroid care.

10 · Kidneys, liver and mood
Kidneys, liver and mood: other things to raise
Mounjaro is not known to damage kidneys or liver directly, and no dose adjustment is needed for mild to moderate kidney or liver impairment.1 The practical risk is indirect: severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys, so keep fluids up and contact your clinical team if you cannot keep fluids down. People with severe impairment need individual prescriber assessment.
Why mention this on a thyroid page? Because people managing a long-term condition often have a wider set of check-ups and blood tests, and it helps to know which parts of your health a prescriber will want to understand. If you have any kidney or liver condition, say so; it will not automatically stop treatment, but severe impairment needs individual assessment.
Regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17
If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.
Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.
How this plays out in everyday monitoring:
- Drink enough fluids, especially if you have stomach side effects.
- Contact the clinical team if you cannot keep fluids down.
- Raise any change in mood early, not at the next scheduled review.
- Report suspected side effects through the Yellow Card scheme.19
