Accountability: the drug versus the programme
Accountability in weight loss comes from the programme around a drug, not from the drug itself. A once-weekly pen builds routine because the whole treatment reduces to one fixed event per week. Tirzepatide, the active ingredient in Mounjaro, is supplied as a pre-filled multi-dose pen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 The strengths give a natural ladder, and each step is a moment to review how things are going.
Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That effect is useful for structure: when hunger is quieter between meals, planned meals are easier to follow.
This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).
Practical ways people build structure around a weekly pen include:

- Logging weight at the same time each week, not daily.

06 · What the trial shows
What SURMOUNT-1 shows about results
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
The trial also reports how many people crossed particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Trial averages do not predict your own result, but they show the range of outcomes seen when treatment is followed over a long period.
A worked example makes the percentages concrete. For someone starting at 100 kg, 15% is about 15 kg, 19.5% is about 19.5 kg and 20.9% is about 21 kg. That is straight arithmetic of the trial percentages, not a forecast for any individual, and it says nothing about how quickly a person reaches those figures.
Why does this matter for accountability? Because the 72-week timeframe in the trial is long. Results of this kind come from sustained treatment alongside lifestyle changes, so a plan that keeps you engaged over many months matters more than a fast start. The comparison group, who made lifestyle changes alone, lost on average 3.1%, which shows the gap between lifestyle effort without medication and with it. It does not show that effort is unnecessary; all participants were working on diet and activity.

What the trial cannot tell you is how you will personally respond. That is the job of your prescriber and your review schedule: to look at your own weight, tolerability and goals and adjust the plan, rather than hold you to an average.
07 · Who is eligible
Who can be prescribed Mounjaro for weight loss
Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition.1 Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you.
That assessment is itself a form of structure. Before you receive a pen, a GPhC-registered prescriber reviews your answers. The most common questions people have about eligibility are:
- Do I need a particular BMI? The licence sets the thresholds above; your prescriber applies them to your circumstances.

08 · Weekly or daily dosing
Weekly injections compared with daily ones
Weekly and daily injections create structure in different ways, and neither is automatically better for every person. Mounjaro and Wegovy are once-weekly injections. Liraglutide, supplied as Saxenda and in products such as Erly at a concentration of 6mg per millilitre, is a daily injection.
Wegovy's pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly. Titration steps up after at least 4 weeks and reaches the 2.4mg maintenance dose from week 17; 2.4mg is the licensed maintenance dose for weight management.18 Mounjaro's ladder has six strengths from 2.5mg to 15mg.1 Both give you scheduled steps and, with them, scheduled conversations about how you are doing.
A daily injection gives a very frequent prompt. For some people that works well, because the habit is tied to the same time every day. For others it feels burdensome. A weekly injection reduces the number of injections and is easy to forget, so a reminder helps.
One difference worth knowing is what happens if you stop. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

09 · NHS and private
NHS and private routes to structured treatment
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
The NHS route can include structured support, and for some people that is the right place to start. Ask your GP what is available in your area. NHS access to Wegovy runs through specialist weight-management services; ask your GP.
The private route is faster to start but you pay for the medicine. What you get for that depends on the provider, so check the service as well as the price. Monthly reviews are the accountability feature that matters most, because they put a clinician in front of your progress at a fixed interval. What should a review cover? A useful one looks at the following:
- How your weight and measurements have changed since last time.
- How your appetite and eating pattern feel at the current dose.
- Any side effects, how long they lasted and how they affected your day.
- Whether to stay at the current strength or discuss moving up.
- Any new medicines, conditions or changes in circumstances.

Side effects deserve a plain mention. The most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort; they are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks.13 If you notice a new symptom, speak to your prescriber, and you can report suspected side effects through the MHRA Yellow Card scheme.19 A symptom that is not on the leaflet's list is not a listed side effect; tell your prescriber about any new or lasting symptom.

A first year of 52 weeks and 13 pens costs £2,711.88 if you settle at 5mg, and up to £3,599.88 if you settle at 15mg. If you settle at another strength, work from that dose's monthly price: £279.99 for 7.5mg, £290.99 for 10mg or £319.99 for 12.5mg, each per 4-week pen. Saturday delivery is £14.99 if you want it. Refunds are straightforward where a consultation is declined, with automatic full refunds. Dispensed prescription medicines cannot be returned except where faulty, damaged or the wrong item, which you must report within 3 days; refunds take 5–10 working days.

12 · Safety and genuine supply
Safety, genuine supply and when to stop
Safety starts with getting genuine medicine from a regulated pharmacy after a proper consultation. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber.1 Be wary of anyone offering to supply without a consultation or prescription, or selling pens at prices that look too good to be true. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting suspected side effects and suspect products.7 You can check any pharmacy on the GPhC public register.12

Keep pens in a fridge at 2–8°C and never freeze them; once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use and must then be thrown away, and the pen's own instructions cover how to use it.13
Two safety topics come up often. First, contraception: Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Second, other medicines: if you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar, so tell your prescriber about them. Neither is a reason to avoid asking; they are reasons to be open in your consultation.
Mood is another topic that readers raise. Regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17 Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.

If you decide to stop, plan it with your prescriber, with diet and activity habits ready to carry the load. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system; nothing needs doing while it clears.1
You may also see the phrase poor man's Ozempic. It is marketing language used for low-cost supplements. These are available cheaply without assessment and come with no clinical oversight. Their dosing is not standardised and we give no efficacy figures for them. Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
