01 · The short answer

Is there a Mounjaro dose conversion chart?

There is no licensed or universal conversion chart that turns Mounjaro doses into doses of other GLP-1 medicines. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, while semaglutide (Wegovy and Ozempic) acts on GLP-1 alone.¹ Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. A milligram of one is therefore not a milligram of the other, and no regulator publishes an equivalence table.

What does exist is each medicine's own titration schedule. Those are the only charts worth trusting. If you have seen a table claiming that a given Mounjaro strength equals a given Wegovy or Ozempic dose, treat it as an estimate. It does not come from the product information.

That is why the common questions, such as "what is 7.5mg Mounjaro equal to in Ozempic?" or "what is 2.4 Wegovy equivalent to in Mounjaro?", have the same honest answer. There is no fixed equivalent. A prescriber looks at your current dose, your side effects, your response and your goals, then chooses where on the new ladder you begin.

02 · The chart

Mounjaro and Wegovy dose ladders side by side

Each licensed ladder is listed in order. Rows are rungs, not equivalents: a row does not mean the two doses match.

The Mounjaro ladder comes from the product information: everyone starts on 2.5mg once a week, and after at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.¹ The Wegovy ladder comes from its own label.¹⁸

03 · Your decision

What to do if you are thinking of switching

If you are moving between medicines, the useful question is not "what is the equivalent?" but "where should I start on the new ladder?" That depends on how well you tolerate your current dose, how you have responded, and how long you have been on it. A GPhC-registered prescriber should make that call, not a chart.

04 · Why milligrams do not convert

Why milligrams do not convert between medicines

Milligrams measure the weight of the substance in the pen, not how strongly it acts. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.¹ Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. Semaglutide is a single-action GLP-1 medicine.

Because the molecules differ, the doses chosen for each were found separately in their own trials. Each ladder was built around how that specific molecule is tolerated. Lining up a 2.4mg dose of one against a strength of the other is like comparing the speed limits of two different roads: the numbers sit on different scales.

The practical consequence is simple. If a website offers a tidy conversion grid, ask where the numbers came from. If the answer is not a prescribing authority or the product information, the grid is an opinion.

05 · Moving within Mounjaro

Moving between Mounjaro strengths

Within one medicine a chart genuinely works, and our Mounjaro pen dosage chart shows each pen against its weekly dose.

Still, linear arithmetic is not a reason to jump. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.¹ You step up only when the current dose has been tolerated for at least four weeks and you still need more effect.

The SURMOUNT-1 trial gives a sense of what each strength delivered on average. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

The same trial reports that at 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.²

As a worked example only, for someone starting at 100 kg, 20.9% is about 21 kg.

06 · Best dose for weight loss

Which tirzepatide dose is right for weight loss?

No single dose suits everyone; your prescriber guides the right pace based on how you respond and tolerate it. Everyone starts at 2.5mg, which is about letting your body adjust, not about weight loss itself.¹ Most measurable progress builds from 5mg onward, month by month.¹

Prescribers review your response on a schedule. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.⁴ Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend rather than individual weigh-ins.

In the same SURMOUNT-1 trial, half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.²

07 · Switching providers

Switching Mounjaro provider at the same strength

If you already use Mounjaro and move to a different pharmacy, no conversion is needed, because the medicine is the same. A pen printed with a given strength holds that strength wherever you buy it. What changes is the clinical handover: a new prescriber will want to know your current dose, how long you have been on it, and how you have tolerated it.

08 · Pen dial and clicks

Pen dials, clicks and dose confirmation

Some pages talk about converting Mounjaro doses into pen clicks. That is not how the pen works.

Each KwikPen is made for a weekly schedule, and the injection is given under the skin once a week, on the same day each week, at any time of day, with or without food.¹ There is no best time of day, so pick one you will remember.

09 · Missed doses and timing

Missed doses and changing your injection day

Dose charts often leave out what happens when real life interferes. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.¹

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.¹ This matters when you are comparing schedules between medicines: a switch is a good moment to pick a day that suits you, but the gap rule still applies.

When someone moves between medicines, the old medicine's schedule does not simply carry across. Your prescriber will tell you when to take the first dose of the new one. Do not stack the two to "cover" the changeover.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.¹ That is one more reason the changeover plan matters.

10 · Cheaper alternatives

Cheaper alternatives and the "poor man's Ozempic"

Supplements sold as a "poor man's Ozempic", such as berberine, are low-cost supplements with no clinical oversight, and they are not licensed GLP-1 medicines. Dosing is not standardised, and they are not an established substitute for tirzepatide or semaglutide. Dramatic weight-loss claims around them should be treated with caution.

There is also no dose conversion between a supplement and a prescription injection.

11 · Oral GLP-1 options

Weight loss tablets and oral options

Two licensed GLP-1 tablets are available in the UK: Wegovy tablets and Foundayo.²⁰²¹ They are separate medicines with their own product information and their own dosing.

Their doses do not convert into Mounjaro injection strengths either, and no trial has compared Mounjaro directly with either tablet. There is no tablet form of Mounjaro; tirzepatide is injection-only in the UK.

A prescriber can talk through which fits your history, other medicines and goals, and that conversation is more reliable than any chart.

12 · NHS and private routes

NHS access and private treatment

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Whichever route you use, the dose ladder is the same, because it belongs to the medicine.

14 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
