If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £879.96 at standard prices. A first year (52 weeks, 13 pens) costs £2,399.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,279.88 if you step up through every dose to 15mg. These totals are before discounts: a new customer's first order is £40 less, and every later order is £25 less.

What GLP-1 injections are and which ones you can buy

GLP-1 injections are prescription medicines that act on gut-hormone receptors to help regulate appetite. The two names most UK buyers meet are Mounjaro (tirzepatide) and Wegovy (semaglutide). Our broader guide to GLP-1 injections covers the family in more detail.

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).¹

Wegovy is a semaglutide pen taken once weekly. Its dose ladder runs 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after at least 4 weeks and reaching the 2.4mg maintenance dose from week 17. The 2.4mg dose is the licensed maintenance dose for weight management.¹⁸

Liraglutide (sold as Saxenda, among others) is a different case. It is supplied at 6mg per millilitre and is a daily injection, unlike once-weekly Mounjaro and Wegovy.

05 · Who can be prescribed

Who can be prescribed GLP-1 injections online

Eligibility is set by the licence, not by the pharmacy. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Those conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹

A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.¹ That is why the consultation is a genuine clinical step. It is reviewed by a GPhC-registered prescriber, and if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

• Your medicines matter. The prescriber checks what you already take. Insulin and sulfonylureas, for example, may need their doses adjusting because of the risk of low blood sugar; that is a conversation for your clinical team, not a reason to avoid asking.
• Contraception matters. Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.¹ Tell the prescriber what contraception you use. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
• Your history matters. Be complete and accurate in the consultation. The review depends on what you tell us.

One more point: the licence defines a treatment, not a target. A BMI above the threshold does not automatically mean a prescription, and a BMI just below it does not mean you cannot improve your health in other ways. The prescriber decides on the whole picture.

06 · Spotting counterfeits

How to avoid counterfeit GLP-1 pens

The biggest risk of buying GLP-1 injections online is a fake pen. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷

Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.⁷ Our registration number is 9012878, and you can look it up yourself.¹²

When your pen arrives, run through this checklist:

• It comes from a registered pharmacy and shows a batch number and expiry date.
• It arrives cold-chain packaged.
• The solution inside is clear and colourless.

Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been above 30°C or out of the fridge for more than 30 days in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.¹³

Red flags for the seller itself are easy to list. Be wary of anyone who:

• offers to supply without a consultation or prescription;
• sells through social media messages or a salon;
• promises specific results;
• cannot show a GPhC registration you can check;
• offers a price far below every other provider, with no explanation.

Lower prices are not wrong in themselves. Providers set their own prices, and our index shows real gaps between them. But a price that seems impossible combined with no clinical questions is the pattern to avoid. If you suspect a falsified pen, report it through the MHRA Yellow Card scheme, which also takes side-effect reports.¹⁹

07 · Doses and injecting

The dose ladder and how you inject

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg. It can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.¹

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.¹

There is no "best" time of day. Mounjaro works the same morning or evening. What matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.¹³ Follow the pen's instructions for how to inject.

Missed a dose? Take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on as usual. If more than 4 days have passed, skip the missed dose and take the next one on your normal day. Never take two doses within 3 days of each other. You can change your weekly injection day if at least 3 days (72 hours) have passed since your last dose.¹

The first weeks on 2.5mg are about adjustment, and experiences differ widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose. Both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.¹³

Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.¹⁹

08 · Realistic results

What results to expect, honestly

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.¹

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.²

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.²

To make a percentage tangible: for someone starting at 100 kg, 20.9% is about 21 kg. That is an average at the 15mg dose after 72 weeks, not a forecast for you, and trial averages do not predict your own result.

That is also the honest answer to "can I lose a set amount in two months?" Weight loss is gradual, the first weeks are an adjustment phase, and early loss includes water weight. Anyone promising a fixed amount in a fixed short period is selling a headline, not a treatment.

Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.¹

As for where the weight comes off, such as belly fat, nothing suggests the medicine targets one area. Your body decides that, and your prescriber can discuss realistic expectations. The same goes for what to eat. The medicine is used with a reduced-calorie diet, and your prescriber or a registered dietitian can tailor food advice to you, including what you tolerate well in the days after an injection.

09 · Storage and travel

Storage, delivery and travelling with your pen

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.¹

For unopened pens, store them in the fridge until first use. They keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, so the UK patient information leaflet and the pharmacy are the right sources.¹³

10 · NHS or private

NHS or private: which route suits you

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

This is a real choice with trade-offs, so here is how to weigh it.

The two routes are not mutually exclusive in principle. You can ask your GP about NHS access while deciding whether private treatment is right for you. NHS access to Wegovy runs through specialist weight-management services, so ask your GP.

If you are on the NHS pathway and your eligibility changes, or you want to talk through the options, your GP is the right first conversation. If you choose private treatment, make sure the pharmacy is on the GPhC register and that a prescriber reviews you before anything is dispatched. The aim is the same either way: a licensed medicine, a proper assessment, and follow-up.

11 · Shortcuts and myths

Shortcuts, supplements and myths to avoid

Some supplements, such as berberine, are sold as a "poor man's Ozempic" or "nature's Ozempic". These are low-cost supplements with no clinical oversight, and dosing is not standardised. If you are weighing a supplement against a prescription, discuss it with a pharmacist or prescriber.

If you notice any new symptom after starting treatment, discuss it with your prescriber and report it through the Yellow Card scheme.¹⁹

The common thread is that safe access is simple to describe: a regulated pharmacy, a consultation, a licensed dose and a pen used as designed. Anything else adds risk without evidence.
