What does ordering three Mounjaro pens actually give you?

Three pens give you twelve weekly injections, because each pen is designed to deliver exactly four weekly doses.1 That is 12 weeks, or three four-week blocks. If you are new to the medicine, the question is not just how many pens you want. It is which strengths a prescriber considers safe for you at each stage.

Everyone starts on 2.5mg once a week. After at least four weeks the dose can rise to 5mg, then in 2.5mg steps at minimum four-week intervals, up to a maximum of 15mg.1 So a new patient's first three pens will normally be 2.5mg, 5mg and then a higher strength. That depends on how you tolerate each step. The prescriber, not the basket, sets the pace.

How does ordering work?

Ordering has four steps, and every one involves a prescriber because Mounjaro is prescription-only.

  • Free online consultation. A short health questionnaire with no charge and no obligation.
  • Prescriber review. A GPhC-registered prescriber reviews it, not a scoring script. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Unsuitable orders are refunded automatically.

04 · Twelve weeks explained

How far do three pens go?

Three pens go twelve weeks at one injection per week. Mounjaro is the brand name for tirzepatide, a once-weekly injectable made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Each KwikPen contains four weekly doses. The pen is set by turning the dose knob until the correct dose shows in the window.

That design is why three pens is a tidy number. Twelve injections, twelve weeks, no leftovers when used as intended.

A three-pen order is therefore best thought of as a three-month plan. Your first pens will normally be a titration, and later pens may be repeats of a maintenance dose. It also helps to treat the pens as a calendar. Write down which week each pen starts, the day you inject, and the date the next order needs to be placed. Because repeat orders re-check suitability and do not auto-ship, leaving a gap of a few days for review and delivery stops you running short.

05 · Dose ladder for new patients

Which strengths would three pens contain?

For a new patient, three pens normally means a step-up through the dose ladder. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1

After at least four weeks the dose increases to 5mg. It can then step up by 2.5mg at a minimum of four-week intervals, through 7.5mg, 10mg and 12.5mg, to a maximum of 15mg weekly. How far you go depends on how you respond and tolerate it.11 Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber guides the pace.

In practice that means:

  • Pen one: 2.5mg for four weeks, the adjustment phase.
  • Pen two: usually 5mg, if the first weeks went well.
  • Pen three: 7.5mg if you step up again, or another 5mg pen if the prescriber prefers you stay put.

Staying on a dose longer is within the licence. Stepping up is a minimum interval of four weeks, not a target. That is why you should not pre-commit to a fixed three-strength bundle before your consultation.

If you are moving from another provider at a higher dose, you do not restart the ladder automatically. The prescriber reviews your current dose, how long you have been on it and your health history. 06 · Planning the year

What does a year of pens cost?

A first year of Mounjaro is 13 pens over 52 weeks, and the total depends on where you settle. At standard prices, before discounts, a first year costs £2,711.88 if you settle at 5mg, up to £3,599.88 at 15mg. A new customer's first order is £40 less, and every later order is £25 less.

For any other maintenance dose, work from the monthly price of one pen: £279.99 at 7.5mg, £290.99 at 10mg and £319.99 at 12.5mg. Those are monthly pen prices, not year totals, and the 12-month cost at those strengths depends on your titration path.

07 · Who can be prescribed

Who can be prescribed Mounjaro?

Mounjaro is licensed for weight management in adults with a BMI of 30 or above. It is also licensed for those with a BMI between 27 and 30 who have at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1

It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you.11 The licensed thresholds are 30, or 27 with a qualifying condition.

Eligibility is assessed through the free online consultation, a short health questionnaire reviewed by a GPhC-registered prescriber. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

Three points are worth knowing before you apply:

  • Medicines matter. List everything you take. Some medicines, including insulin and sulfonylureas, may need dose adjustments because of low blood sugar risk, and the prescriber needs the full picture.
  • Oral contraception matters. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows.
  • Honesty matters. The review depends on accurate answers. It protects you.

08 · NHS or private

NHS or private: what is the picture?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6

Full rollout is expected to take years. That is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. The two routes use the same medicine. What differs is who is prioritised, how soon you are seen and how you pay.

On the NHS, access follows the rollout stage and your local service. Ask your GP whether you fall into the current group. On the private route, a prescriber reviews you against the licensed criteria, which are broader than the NHS rollout's first groups: a BMI of 30 or above, or 27 and above with a weight-related condition.

The private route has costs. It also has responsibilities. Because you are paying per pen, you will want a plan for how long you intend to stay on treatment and what happens afterwards. Ongoing monthly clinical reviews, dose-titration and follow-up assessments are part of our aftercare, and side-effect support is available.

If you are on the NHS pathway and also consider private pens, tell both prescribers. They need an accurate medicines list to keep you safe. Do not take overlapping supplies of the same medicine.

There is no single right answer. Some people wait for NHS access, some pay privately for a few months, and some do both in sequence. What matters is that every route involves a clinical consultation and a prescription.

09 · What results to expect

What results should you realistically expect?

Weight loss on Mounjaro is gradual by design, and it is measured in months, not days. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks. The comparison group, on lifestyle changes alone, lost 3.1%.2

These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.2

As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. Trial averages do not predict your own result.

For a three-pen run, that matters. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Some people notice appetite change and early loss in week one, and others feel little until a higher dose. Both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.

Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

So after three pens, you will be about 12 weeks in. Judge it by trend, how you feel, and what your prescriber advises. Three pens is not a finish line. It is a checkpoint.

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle. Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. Report anything that worries you to your prescriber, and you can report side effects through the Yellow Card scheme.19

10 · Spotting genuine pens

How do you know the pens are genuine?

Genuine UK Mounjaro comes from a registered pharmacy, and you can verify that pharmacy on the GPhC register. Mounjaro is manufactured by Eli Lilly. Genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies, and every pen carries a batch number and expiry date that can be checked.1

This matters more when you order several pens. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7

Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation. Anyone offering to supply without a consultation or prescription is a red flag.   • The pen comes with a batch number and expiry date.
  • It arrives cold-chain packaged.
  • The solution inside is clear and colourless.

Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been out of the fridge for more than 30 days in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.

11 · Storing three pens

How should you store three pens?

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Three pens means two will usually sit unopened while you use the first, so storage planning matters. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.1

Once a pen is in use, it can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips. After that it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.13

A practical routine looks like this:

  • Put the unopened pens in the fridge as soon as the delivery arrives, away from the freezer compartment and the back wall where items can freeze.
  • Take out only the pen you are starting.
  • Note the date you first use each pen.

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing. Ideally use a cool bag with the prescription label or a copy of your prescription. A pen in use can be kept out of the fridge at a temperature not above 30°C for up to 30 days, so most holidays need no special arrangements. For hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.1

If an unopened pen has been out of the fridge, check with the pharmacy before using it, rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels.

12 · Using each pen safely

How do you use each pen safely?

Inject once a week, under the skin, on the same day each week. Mounjaro is injected subcutaneously at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Follow the pen's instructions for the injection itself.1

There is no best time of day. Mounjaro works the same morning or evening. What matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.13

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.

With three pens in the house, keep the order straight. Use the lowest strength first if you are titrating, and do not pick up the wrong pen. Check the strength printed on the pen before each use.

13 · Why not stretch pens

Can you make three pens last longer?

No. Making pens last longer than their four weekly doses is not a safe way to save money. Mounjaro is licensed as a once-weekly medicine, and its blood levels are designed around a 7-day cycle.

Talk to your clinical team rather than self-adjusting. That conversation is part of the ongoing monthly reviews we include.

It is also worth understanding what stopping looks like. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

What about the maintenance question people ask after three months? The trial evidence for tirzepatide is for weekly dosing alongside diet and exercise, and individual results vary. Your prescriber can discuss options based on your own history.
