04 · What a KwikPen is

What a Mounjaro KwikPen actually is

A Mounjaro KwikPen is a pre-filled, multi-dose injection pen containing tirzepatide, the active ingredient of Mounjaro, a once-weekly prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

The pen is not a vial and you do not draw anything up with a syringe. You attach a new needle, dial the dose and inject under the skin. The pen is designed to deliver exactly four weekly doses, which is why one pen equals one month of treatment.¹³

The strength printed on the pen is the weekly dose. A 5mg pen delivers 5mg each week for four weeks. It does not hold 5mg in total.

The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

One more point is worth stating plainly. A pen is prescribed to one person for that person's dose. 05 · How many pens

How many pens you get and how many you need

Each Mounjaro pen contains four weekly doses, so one pen lasts four weeks and you receive the pen for the strength you are prescribed. Most orders are one pen per month, though you may be prescribed more than one at a time depending on your plan.¹³ A standard order also includes needles, a sharps bin and injection guidance.

Over time the count looks like this. If you step up after 4 weeks on each dose, the first four pens (2.5mg, 5mg, 7.5mg and 10mg) cost £879.96 at standard prices before discounts. A full year of 52 weeks is 13 pens. If you settle at 5mg, the first year costs £2,399.88; at 15mg, up to £3,279.88. A new customer's first order is £40 less and every later order is £25 less.

The dose ladder determines which pens you buy and when. Everyone starts on 2.5mg once a week. After at least four weeks the dose increases to 5mg, and can then step up by 2.5mg at minimum four-week intervals (7.5mg, 10mg, 12.5mg) to a maximum of 15mg weekly, depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber guides the pace.¹

06 · Who can be prescribed

Who can be prescribed a Mounjaro pen

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹

Meeting a number is not the same as being prescribed. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you.¹¹ That is why the consultation is a clinical review and not a checkout step. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

The consultation asks about conditions, medicines and previous treatments. Be accurate. Some medicines interact with Mounjaro, and the prescriber needs the full picture. For example, tirzepatide can reduce absorption of oral contraceptives because stomach emptying slows, and people on insulin or sulfonylureas may need their doses adjusted because of the risk of low blood sugar. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.¹ Your prescriber will advise on both.⁷

NHS or private?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions. Full rollout is expected to take years.⁴⁶

That phasing is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. The two routes are not mutually exclusive in principle, but it is worth asking your GP what applies locally. NHS prescriptions in England carry the standard £9.90 charge per item unless you are exempt, whereas private supply is priced by the pharmacy.

07 · How it works

How Mounjaro works and what the trials showed

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic). Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.¹

What SURMOUNT-1 found

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.²

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.² As a worked example only: for someone starting at 100 kg, 20.9% is about 21 kg. Trial averages do not predict your own result.

Comparing with semaglutide

In the first large head-to-head trial (SURMOUNT-5), adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy).¹⁵ Numbers from different trials, such as SURMOUNT-1 and STEP 1, should not be placed side by side as if head to head. Both medicines are licensed, effective treatments, and the right choice depends on your history, tolerance and preference. A prescriber can discuss this with you during the consultation.

What this means for buying a pen: the strength you buy reflects a gradual process, not a promise of a particular outcome. The trial doses reached in SURMOUNT-1 took many months of stepping up. Starting at 2.5mg is about letting your body adjust, not about weight loss itself, so the first pen is the foundation rather than the finish line.¹

Because the mechanism acts on appetite and stomach emptying, the day-to-day experience is usually described in terms of fullness and reduced snacking, with early effects such as nausea or tiredness in the days after each injection.

08 · Fake pens

Avoiding fake pens when you buy

The safest way to buy a Mounjaro pen is from a pharmacy you can verify on the GPhC register, because the MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷

Only buy from a registered pharmacy, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation. If a seller offers to supply without a consultation or prescription, walk away. Dramatic weight-loss claims and unusually low prices are further warning signs.

How to check a seller

Checking the pen when it arrives

A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days in use, or above 30°C.¹³

09 · Storage and travel

Storing and travelling with your pen

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once a pen is in use, it can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.¹³

Because a pen contains four weekly doses, an in-use pen runs for four weeks, within the limit of up to 30 days out of the fridge not above 30°C. Most people simply keep the pen in the fridge between doses and take it out shortly before injecting.

Unopened pens

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, and the UK patient information leaflet is the reference to follow.¹³ If a delivery sits on a doorstep for a long time, contact us before injecting.

Flying and holidays

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing. Ideally use a cool bag, and carry the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates, use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.¹³

Everyday handling

• Fridge, not freezer: never place the pen next to the ice compartment or cooling element.
• Cap on between doses to protect the solution from light.
• Use a new needle each time and put used needles in the sharps bin.
• Check the solution is clear and colourless before each injection.
• Discard any pen that has been frozen, looks cloudy or contains particles.

10 · Injecting and missed doses

Injecting your pen and handling missed doses

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.¹

Follow the pen's own instructions for the injection steps, including how long to hold it in place, and use the guidance sent with your order. Do not rely on a timing figure from a video or forum. Choosing a day and time

There is no best time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off, then stick to it.¹

If you miss a dose

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.¹

Changing your injection day

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.¹ This is helpful before holidays or when a work pattern changes. Note the new day and keep to it from then on.

When the pen will not cooperate

11 · Realistic expectations

Realistic results, early weeks and what not to do

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.²

Your prescriber reviews your response. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.¹¹ That honest checkpoint is part of the monthly clinical reviews included with your treatment.

The first weeks

The 2.5mg starting weeks are about adjustment, and experiences differ widely. Some people notice appetite change and early loss in week one, while others feel little until a higher dose. Both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.¹

Setting up for success

Think of the pen as one tool within a plan. The trials paired tirzepatide with a reduced-calorie diet and increased physical activity, and the licence requires the same. Practical habits help: a regular injection day, tracking in an app, protein-conscious meals and regular contact with your prescriber if side effects trouble you.

12 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
