03 · Hunger on day 3

Why am I hungry on day 3 of Mounjaro?

Hunger on day 3 is common and does not mean the injection failed. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties. In practice, most people feel fuller sooner and less hungry between meals.1 Most people is not everyone, and the effect is not identical from person to person or from day to day.

The starting dose is deliberately the lowest. Some people feel little appetite change until a higher dose, and that is considered a normal pattern, not a fault.13 Hunger can also be mixed up with habit, boredom or an irregular meal pattern, particularly when your appetite is only partly dampened.

Practical steps within your treatment plan: eat at regular times rather than waiting until you are starving, build meals around protein and fibre-rich foods, and keep fluids up. 04 · Peak and the weekly cycle

What day is Mounjaro at its peak?

There is no single day when you will feel Mounjaro peak. Mounjaro is a once-weekly injection, and the medicine stays in your body between doses. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

That slow clearance is why you are not meant to feel a dramatic on-off switch across the week. Many people do notice that appetite effects and side effects feel stronger on some days after the injection than others, but this varies and is not a reliable guide to whether the dose is working. 05 · Weight loss on 2.5mg

Can you lose weight on 2.5mg of Mounjaro?

Yes, some people do, but losses on 2.5mg are usually modest. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase, and most measurable progress builds from 5mg onward, month by month.1

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

06 · Early scale changes

Mounjaro day 3 results: what the scales can and cannot tell you

Day 3 results on the scales are mostly noise. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.13 A drop can reflect lower food volume and fluid shifts, and a flat reading can reflect the same things in reverse. Neither says much about fat loss.

Weight loss on Mounjaro is also not linear. Plateaus of a few weeks are normal and expected as the body adapts to a lower weight.13 Weigh at the same time of day, once a week at most, and judge progress by trend rather than individual weigh-ins.

Looking further ahead, a sensible three-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.2 Beyond the scales, trial participants also saw average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, which is why prescribers talk about health, not just weight.2

07 · First week and injection questions

What to expect the first week on Mounjaro

The first week is mostly about learning the routine. You inject once, then wait. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13 Keep your meals small and unfussy, and do not read too much into one good or bad day.

Should you inject on a full or empty stomach?

Either is fine: Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. What matters most is a consistent injection day and the correct technique.13

Can Mounjaro be injected incorrectly?

Yes. Pens can be used incorrectly, for example by not following the pen's instructions or by mishandling the device. If you think a dose did not go in properly, do not inject a replacement dose on your own. Contact your prescriber or our support team for advice. Needles, a sharps bin and injection guidance come with every order, and the leaflet explains each step.13 Store unused pens in the fridge at 2–8°C until first use. An in-use pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or frozen.13

08 · Food, alcohol and safety

Treatment works alongside a reduced-calorie diet and increased physical activity, so what you eat still matters.2 Rather than a rigid banned list, most people find that very rich, greasy or heavily sugary meals sit badly while the stomach empties more slowly, and eating smaller portions helps. Protecting protein and staying active supports the plan.

There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and emptier stomach can hit harder than usual. If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar. Alcohol is one of the basics to check when progress stalls, along with sleep, portions, protein and activity.13 It can also add to nausea and dehydration when you are already eating less, so many people cut back. Discuss your own situation with your prescriber.

When to get help

Mild, settling symptoms are expected. Severe or persistent stomach pain, swelling of the face, lips or throat, or difficulty breathing needs medical help straight away. Contraception is also worth flagging: Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

How progress is actually judged

Progress is judged over months, not days. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Day 3 is far from that checkpoint.

If progress stalls later, the responses within the licence are to review food intake honestly, protect protein and activity, check sleep and alcohol, and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum. A plateau is not a reason to double doses or add unlicensed products.13

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
