Is opening a weight loss clinic right for you?

It is worth doing only if you can fund proper clinical oversight and are prepared for a regulated, long-term service. A weight loss clinic is worth it when patients are treated safely, followed up properly and supported for as long as they need. It is a poor idea when the aim is quick money from a fashionable medicine.

The honest picture is mixed. Demand is real, because many adults who meet the licensed criteria are still waiting for NHS access. NICE recommends tirzepatide for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, and full rollout is expected to take years.46 That gap explains why private clinics appear. It also explains why regulators, prescribers and the press are paying close attention to them.

Costs sit mainly in people and governance, not in the medicine itself. You will need prescribers, a responsible clinician, a registered manager where the regulator requires one, insurance, safe premises or a compliant online system, and time for follow-up. Competition is heavy, and patients now compare providers on safety and transparency as well as on price.

04 · Regulation and registration

Regulation and registration

Which regulator you register with depends on the nation you operate in and the activities you carry out. In England, services that provide regulated activities, such as treating disease, disorder or injury, generally need to be registered with the Care Quality Commission. Wales, Scotland and Northern Ireland each have their own healthcare regulator. Contact the right one early and ask in writing what applies to your model.

Do not assume that a beauty or wellness licence covers medical treatment. Prescribing a medicine for weight management is a clinical act. It needs a registered clinician, a record of the consultation and a clear decision about suitability.

Pharmacies are regulated separately. The General Pharmaceutical Council keeps a public register of pharmacies and pharmacy professionals, and you can check any supplier against it.12 If you plan to dispense medicines yourself rather than use a registered pharmacy, you will face a further set of premises, storage and superintendent requirements. Take professional advice on this before committing.

Other legal areas also apply: data protection and secure patient records, consumer law, advertising rules for prescription-only medicines, indemnity and insurance, and complaints handling. 05 · Prescribers and governance

Prescribers and clinical governance

Every patient needs a consultation reviewed by a suitably registered prescriber who takes responsibility for the decision to treat. Mounjaro (tirzepatide) is a prescription-only medicine. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 Your protocols should be built on the product information and national prescribing references, not on marketing material.11

Good governance is written down. At a minimum, plan for:

  • An eligibility protocol that names who you will treat and who you will decline or refer on.
  • A named clinical lead who signs off protocols and reviews incidents.
  • Records of every consultation, dose decision and follow-up contact.
  • A process for side-effect reports, including reporting through the Yellow Card scheme.19
  • Training and competence checks for everyone who speaks to patients about treatment.
  • A complaints and significant-event process that you actually use.

A consultation is more than a questionnaire. A scoring script that approves anyone who ticks the right boxes is not clinical judgement. Reviewers need the freedom to decline, to ask for more information, or to refer someone for a different kind of care.

Where a patient is declined, build in a kind, clear explanation and a route to appropriate support. Weight management is a sensitive subject, and how you say no matters.

06 · Safe medicine supply

Safe, traceable medicine supply

Buy medicines only from suppliers you can verify, and keep records that show where every pen came from. Counterfeit and unlicensed weight loss injections are a known problem, and the MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting.7 A supplier that offers to supply without a consultation or prescription is a warning sign, whether you are a patient or a clinic.

Storage is part of supply. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1 A clinic that stores or handles stock needs a monitored fridge, a process for responding when temperatures drift, and a written procedure for returns and disposal.

Transport matters too. Pens that travel to patients need appropriate cold-chain packaging and a tracked service. If you are designing your own, test your process in warm and cold conditions before you rely on it.

Sharps need planning as well. Patients need needles, a sharps bin and clear injection guidance, and your team needs to know how to explain them. We include these with every order.

07 · The patient pathway

A weight management service is a pathway, not a single prescription. Patients need to understand what happens at every stage, and your team needs scripts and protocols that match the product information.

Titration is the heart of it. Everyone starts Mounjaro on 2.5mg once a week, and this starting dose is about letting the body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals up to a maximum of 15mg weekly, depending on response and tolerance.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Your prescriber guides the pace.

Your patient information should cover practical points drawn from the leaflet and label:

  • Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.13
  • There is no best time of day. What matters is choosing a day and time the patient will remember.13
  • If a dose is missed, take it as soon as possible within 4 days (96 hours) of the scheduled time. After that, skip it and take the next on the normal day.1

Set expectations honestly. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Trials measured results at 72 weeks, so patients should think in months, not days.2 Individual results vary, and your clinic should never imply otherwise.

Plan for review. If a patient has lost less than 5% of their starting weight after 6 months on the highest dose you can tolerate, the prescriber should discuss whether continuing or stopping is right for them.1 That conversation needs protected time in your diary.

Finally, plan for stopping. After a final dose, tirzepatide takes roughly four to five weeks to clear the system, and nothing needs doing while it clears.1 Patients who stop will want advice on what happens next, so build that conversation in rather than leaving it to chance.

08 · Safety lines you must hold

Safety lines a clinic must hold

A clinic earns trust by refusing the shortcuts that circulate online. Several need a firm written position.

Dose splitting, pen sharing and repackaging. Pens are dispensed to an individual patient on a prescription. Never split, decant, share or stretch them; altering doses outside the licence means unknown dosing accuracy and no safety data.

Contraception. Tirzepatide can reduce absorption of oral contraceptives because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

Other diabetes medicines. Doses of insulin or sulfonylureas may need adjusting because of the risk of low blood sugar. This is a prescriber decision, not something to leave unaddressed.

Mental health. The EMA's safety committee reviewed the evidence in 2024 and concluded that it does not support a causal link between GLP-1 medicines and suicidal thoughts, and the MHRA keeps this under continuous review. Your protocol should say how staff respond if a patient raises mood concerns, and patients should tell their prescriber about any history of depression.17

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

Whatever your model, make sure patients know how to reach a real person when something feels wrong, and tell them how to report side effects through the Yellow Card scheme.19

09 · Economics and honest revenue

Economics: what is profitable and what is sustainable

Profit depends on your costs, your location and your patients.

Revenue usually comes from consultations, review appointments, medicine supply and ancillary services. Each has a cost. Reviews need clinician time. Medicine supply needs capital, storage and risk management. On increasing revenue, the sustainable levers are retention and reputation, not pressure. Patients who feel looked after attend reviews and recommend you. Patients who feel sold to leave reviews that stay online for years. Avoid urgency tactics on prescription medicines, avoid promising specific results, and be open about what treatment involves.

Partnering, referring and online models

Building every part of the service yourself is not the only route. Many founders start by working with an established regulated provider, and some focus on the local, in-person parts of care such as measuring, support and lifestyle input.

Online models have grown quickly. They can reach patients across the country, but they carry the same clinical duties as a premises-based clinic, and sometimes more because the prescriber never meets the patient.

Patients arrive better informed than they used to. They have read about GLP-1 treatments, compared providers and heard stories about side effects and rebound. Your service should meet them with clear answers.

They expect an honest account of what treatment is: a licensed medicine used alongside a reduced-calorie diet and increased physical activity, with results that vary from person to person. They expect to know who the prescriber is, how to contact the clinic, and what follow-up is included. They also expect fair treatment when they are declined.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
