So Novo Nordisk is not the same thing as Ozempic. It is the manufacturer behind it, in the same way Eli Lilly is the manufacturer behind Mounjaro.

04 · How semaglutide works

How Novo Nordisk's GLP-1 drugs work

Novo Nordisk's weight loss drugs work on the GLP-1 pathway, which is one of the natural gut-hormone signals involved in appetite and digestion.

Semaglutide targets one receptor. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1

For GLP-1 medicines in general, the day-to-day experience is similar. Most people notice that they feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 Stomach emptying slows as well, and that slower pace is behind both the sense of fullness and many of the digestive side effects people mention.1

It helps to be clear about what these medicines do not do. They are not a shortcut that works without effort, and they do not remove the need for balanced meals, protein, hydration and movement. They make a reduced-calorie diet easier to stick to. Treatment is intended to work alongside lifestyle changes, not instead of them.

The slower stomach emptying also has a practical consequence you should know about. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed, which is why your prescriber will ask about contraception during the consultation.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

Both are once-weekly injections taken with a pen. Both lean on the GLP-1 signal. Only tirzepatide adds the GIP signal on top. One more point about how long the effect lasts. These are weekly injections because the medicine stays active in the body for days. For Mounjaro, the half-life is about five days, so after a final dose it takes four to five weeks to clear the system.1 Nothing needs doing while it clears, and there is no way to speed it up.

05 · Wegovy dose ladder

The Wegovy dose ladder and titration

Wegovy is dosed once a week: the dose steps up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, and the 2.4mg maintenance dose starts from week 17.18 The 2.4mg dose is the licensed maintenance dose for weight management.18

That gradual approach exists for a reason. Side effects with this class of medicine tend to appear in the first days after starting or increasing a dose, peak early and settle as your body adapts. A slow ladder gives your body time to adjust. If a new dose is hard to tolerate, prescribers can hold the current dose for longer before stepping up, and the right move is to speak to your clinical team rather than pushing through severe symptoms.

Mounjaro uses a similar idea but a different ladder. Its pen strengths are 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg, with at least 4 weeks on each dose, depending on how you respond.1 If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A few handling points apply to any pen medicine. Use the pen as designed and follow the instructions that come with it.

Storage questions come up constantly. Once in use, a Mounjaro pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.13

Finally, keep your injection routine boring. Pick a day, keep the same day each week, rotate the site and follow the pen's own instructions. Small habits like these do more for comfort than any trick you will read online.

06 · Saxenda, tablets and other names

Saxenda, tablets and other Novo Nordisk names

Beyond Wegovy, Novo Nordisk's other weight-management name you will meet is Saxenda, which contains liraglutide. Liraglutide is supplied at a concentration of 6mg per millilitre and is a daily injection, unlike the once-weekly Wegovy and Mounjaro. Products such as Erly are also liraglutide-based.

Tablets are the other development. The phrase "poor man's Ozempic" usually refers to berberine, a supplement marketed as a natural alternative. Be careful here. Supplements are low-cost, with no clinical oversight and no standard licence behind them, and their dosing is not standardised. They are not equivalent to a regulated GLP-1 medicine, and dramatic weight-loss claims made for them should be treated sceptically.

If you want to know whether you might prefer a company other than Novo Nordisk, remember that the relevant question is the medicine, not the manufacturer. The list of licensed weight-management options is short, and your prescriber is the person to match you to one. 07 · Novo Nordisk vs Mounjaro

Novo Nordisk and Mounjaro: how they differ

Mounjaro is not made by Novo Nordisk; it is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly.1 In the UK it is licensed for weight management in adults and for type 2 diabetes, and it comes as a pre-filled multi-dose pen, the KwikPen, in six strengths.1 The two companies are direct rivals, which is why headlines about one often mention the other.

The mechanism is the clearest difference. Tirzepatide activates both GIP and GLP-1 receptors, whereas semaglutide is a single-action GLP-1 medicine.1

On efficacy, here is what the Mounjaro trial reported. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. For someone starting at 100 kg, 20.9% is about 21 kg.

Wegovy has its own trial, STEP 1, which followed adults over 68 weeks.9 Different trials with different designs cannot be compared as if they were head to head, and a number that looks bigger or smaller across studies tells you little about which medicine will suit you.

Where does access differ? Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. 08 · Side effects to expect

Side effects of Novo Nordisk's weight loss drugs

Side effects with this drug class are mostly digestive, and they tend to show up when you start or step up a dose. Semaglutide belongs to the same broad family, so the pattern will feel familiar, but the specific rates for Wegovy are in its own patient leaflet and product information, which you should read before starting.18

The usual suspects are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions also occur.1 Most side effects appear in the first days after starting or increasing a dose, peak early, and settle as your body adapts. For most people they ease within days to a few weeks.

Practical steps help. Eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, stay well hydrated and avoid lying down straight after eating. For constipation, fluid intake comes first, then fibre from vegetables and whole grains, and daily movement. A pharmacist can recommend a suitable laxative if it persists. If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.

In the Mounjaro weight-management trials, 1.9% of people on 5mg, 3.3% on 10mg and 4.3% on 15mg stopped treatment permanently because of gastrointestinal side effects, compared with 0.5% on placebo.1 Those figures are for Mounjaro. The takeaway is that most people tolerate treatment, while a small minority stop because of stomach effects.

When side effects are serious

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis). For pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems), get medical help straight away. For signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing, get medical help straight away.

If you take insulin or a sulfonylurea alongside one of these medicines, watch for low blood sugar, such as shakiness, sweating or confusion. Your prescriber may need to adjust those medicines. This is not a reason to avoid treatment, but it is a reason to tell your prescriber everything you take.

Reporting a side effect

If you notice something new, tell your prescriber and report it through the MHRA Yellow Card scheme.19 Reports from patients help regulators spot patterns. On mood, regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17 Even so, any big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment. 09 · Myths and body changes

Ozempic face, teeth, breasts and other myths

Questions about body changes with catchy nicknames are common, and most are not established drug effects. If something new appears, discuss it with your prescriber and report it through the Yellow Card scheme.19

"Ozempic face" is the best-known term. It is not a drug effect. It is what losing a significant amount of weight quickly can do to facial volume, on any method, including diet or surgery. The same applies to loose skin. The counter-measures are the ones that protect lean mass: adequate protein, resistance exercise and a loss pace guided by your prescriber rather than maximum speed. Skin commonly adapts gradually after weight stabilises.1

"Ozempic teeth" and "Ozempic breasts" are nicknames, not established effects of the medicine. If you notice dental or breast changes, speak to your dentist, GP or prescriber, as they are best placed to look at the cause. Dry mouth, reflux or vomiting from digestive side effects can matter for teeth, which is another reason to stay hydrated and manage nausea early. Changes in body shape as weight falls are a separate matter from drug action.

Hair shedding is different, because it is a recognised effect of this class. Temporary hair shedding is reported as common in trials and is linked to rapid weight loss itself rather than a direct drug effect. For most people it settles as weight stabilises. Protein intake and overall nutrition matter, which is another reason the reduced-calorie diet should stay balanced.1

Then there are questions like "can you smell when someone is on Ozempic?". Body odour is not on the full list of side effects in the UK Mounjaro patient leaflet. If you or someone else notices a change in smell, it is worth discussing with a clinician, because causes can range from diet to hydration to unrelated conditions.

"Will you lose belly fat on Ozempic?" is a fair question with an honest answer: these medicines are used to reduce overall body weight, and where your body loses fat from varies from person to person. Results come from the whole picture of diet, activity, sleep and the medicine together.

"Why do I feel so good on Ozempic?" has no single established answer. Some people feel lighter and more energetic as weight falls and food stops dominating their thoughts. Others feel tired instead, particularly in the first weeks and after dose increases, because the body is adjusting and eating less. Fatigue is a recognised common side effect for this class of medicine, so keep meals balanced, stay hydrated and keep gentle activity going. Tell your clinical team if tiredness is severe or not settling.

It is not a licensed GLP-1 medicine, there is no clinical oversight when you buy it, and its dosing is not standardised. Dramatic weight-loss claims made for supplements should be treated sceptically.
Will you lose belly fat on Ozempic or Mounjaro?
These medicines are used to reduce overall body weight alongside a reduced-calorie diet and increased physical activity, and where fat comes off first varies from person to person. In the SURMOUNT-1 trial, people on tirzepatide lost far more weight on average than those using lifestyle changes alone, but trial averages do not predict your own result.
