The medicine activates two gut-hormone receptors, GIP and GLP-1. Together they help regulate appetite and slow how quickly the stomach empties.1 In practice, most people feel fuller sooner and less hungry between meals. That effect is not always strong on the starting dose, and it can vary from day to day across the injection cycle.
The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.2

Why you can feel hungry in week 3 on Mounjaro
Feeling hungry at this stage is common and does not mean the medicine has stopped working. Appetite change is one of the commonest early effects, but how strong it is differs a great deal between people.2 Some people notice little until a higher dose.
The mechanism helps explain why. Tirzepatide helps regulate appetite and slows stomach emptying, so most people feel fuller sooner and less hungry between meals.1 On the lowest strength that effect can be subtle, and it can feel stronger or weaker at different points in the weekly cycle.
Practical steps that fit with treatment, which is used alongside a reduced-calorie diet and increased physical activity:
- Build meals around protein and fibre so you feel satisfied for longer.
- Eat at planned times rather than waiting for strong hunger.
- Keep drinks plentiful, and watch for thirst being mistaken for hunger.
- Note which days of the cycle feel hungriest and plan around them.

04 · Scale changes and pace
What the scales may show by week 3
Weight loss on Mounjaro is gradual by design. Trials measured results over 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.2
Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.2 That cuts both ways. A big drop early on may partly be fluid, and a flat or slightly higher reading may be fluid too, from salt, a heavy meal or a hormonal shift.
For context, in the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. They do not describe what happens in week 3.
Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.
On weighing: for the most useful trend, weigh at the same time of day, on the same scale, in similar clothing. Morning is simply the easiest time to keep consistent. No single reading is your true weight.
05 · Side effects this week
Side effects around week 3
Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects, and they usually settle.2 If yours are easing, that fits the usual pattern. If they are not, you are not doing anything wrong.
Sensible habits while your body adjusts:
- Eat smaller portions and stop when comfortably full.
- Avoid very rich or greasy meals on injection day and the day after.
- Rest if you are tired rather than pushing through.
The UK patient leaflet lists the very common side effects, which may affect more than 1 in 10 people, as feeling sick, diarrhoea, being sick, stomach pain, constipation, feeling tired and reduced appetite.13 If you notice a symptom that is not listed, or one that worries you, speak to your prescriber. You can also report suspected side effects through the Yellow Card scheme.19
06 · Plateau or no loss
Plateau or no weight loss in week 3
A pause this early is not a verdict on the treatment. Weight loss on Mounjaro is not linear. Plateaus of a few weeks are normal and expected as the body adapts to a lower weight.2
Within the licence, the responses are:
- Review food intake honestly, because portions drift back up.
- Protect protein and activity.
- Check the basics, such as sleep and alcohol.
- Discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.

07 · Review and next steps
When your prescriber reviews progress
Your response is reviewed by your prescriber, and the formal checkpoint comes much later than week 3. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.2 That is the point at which a lack of progress means something.

Progress that is not on the scale
Weight is only one of the outcomes that matter. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises. They are why prescribers talk about weight management as health management rather than appearance.

09 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
