No, water retention is not listed as a side effect in the UK patient leaflet. Mounjaro is a once-weekly injection that activates the GIP and GLP-1 receptors to help regulate appetite and slow how quickly the stomach empties.1
Still, people do feel puffy, tight in the waistband or stuck on the scale. That is real, and it usually has a different explanation. Tell your prescriber about any new or lasting symptom, and you can report suspected side effects through the MHRA Yellow Card scheme.

04 · Why the scale fluctuates
Why the scale moves around on Mounjaro
Body weight is not the same as body fat, and day-to-day readings mix food, fluid and waste in your gut. Eating less means less food volume moving through you, so early readings can drop without telling you much about fat. Later, the scale may pause even though your clothes fit differently.
Several ordinary things push the number up temporarily. A salty meal, a heavy evening, a hormonal cycle, a hot day or a harder than usual gym session can each hold fluid for a short while. Constipation adds weight too, and it is very common on Mounjaro (it may affect more than 1 in 10 people); a pharmacist can recommend a suitable laxative if it persists.
The practical rule is to weigh at the same time of day, under the same conditions, and judge the direction over several weeks. 05 · Bloating and trapped wind
Bloating, trapped wind and a swollen belly
A bloated belly on Mounjaro is usually about digestion, not water. Tirzepatide slows how quickly the stomach empties, so food stays put longer and large portions sit heavily.2 That can feel like fullness, pressure, belching or trapped wind, especially after a dose increase.
What helps is mostly about how you eat. Many people feel worse after very fatty, fried or very sugary food, and smaller, slower meals work better.1 No food is banned, but protein, vegetables and fibre, with regular fluids, make treatment easier. Gentle walking after meals is a simple way to ease gas.
If you want a quick debloat, there is no drink that works like a switch. Plain water, and tea or coffee in moderation, are fine; strong coffee on an empty stomach can aggravate nausea or reflux for some people, and high-calorie coffee drinks quietly undo your calorie deficit. Social media "coffee rules" have no clinical basis. Indigestion, bloating, burping, wind and reflux are listed as common side effects (up to 1 in 10 people) in the UK patient leaflet; ask your prescriber if symptoms are severe or do not settle.13
06 · Hydration and fluid balance
Hydration: drink more, not less
Drinking enough matters more on Mounjaro than usual. Reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly, which is the main route to the rare kidney problems reported with this class of medicine.1
It can feel backwards to drink more when you feel puffy. Keep water within reach through the day, and spread it out rather than drinking a large amount in one go on a full stomach.

07 · Protein, muscle and movement
Protein, muscle and movement
Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance. The levers are adequate protein, with a protein source at every meal, which is harder when appetite is low, and resistance exercise two or more times a week.1 This matters more, not less, as the dose rises and appetite falls.
Exercise is part of the licence, not an optional extra. Nothing special is required: regular moderate activity such as brisk walking plus resistance work for muscle. Start where you are and build. On injection days with early side effects, lighter movement still counts.1

People often ask about "GLP-1 water retention" as though it were one thing. Each medicine has its own product information, so the honest answer is to check the leaflet for the specific product you take. Swelling that is persistent, sudden, painful or affects only one leg should be looked at by a clinician rather than explained away. Contact your prescriber or GP, and get medical help straight away if swelling comes with breathlessness or chest pain.

10 · Supplements and quick fixes
Water pills, supplements and quick fixes
Avoid shortcuts for water weight. No supplement is required with Mounjaro, but eating much less can leave gaps, so a standard daily multivitamin is a reasonable safety net while losing weight; discuss anything beyond that with a pharmacist.1 Supplements sold as natural GLP-1 boosters are not equivalents and are not substitutes for prescribed treatment.
Over-the-counter diuretics, detox teas and extreme salt or water manipulation can dehydrate you, and dehydration is exactly what you want to avoid on this medicine. A balanced diet with less ultra-processed food, steady fluids, protein and activity does more for fluid balance than any product.
11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
