A quick overview
What to know
before you start.
-
Fluid retention and urinary retention are not listed side effects of Mounjaro in the SmPC or patient leaflet.
-
Short-term scale stalls and bloating are common talking points, but they are not proof of fluid retention.
-
New swelling in the ankles, legs or hands needs a prescriber's attention; swelling of the face, lips, tongue or throat needs medical help straight away.
-
Do not self-treat with diuretics or 'water pills' while on Mounjaro without clinical advice.
-
You can report any suspected side effect through the MHRA Yellow Card scheme.
In this guide
01 · The core answer
Does Mounjaro cause fluid retention?
Fluid retention is not a listed side effect in the UK patient leaflet. The same is true of urinary retention, meaning being unable to empty your bladder properly.
That is not the same as saying nothing will ever change on your body. People on treatment notice many things, including bloating, a stalled scale and puffy ankles. Some of these are unrelated to the medicine, and some are worth a conversation with your prescriber.
If you have noticed swelling that is new, one-sided, painful or getting worse, do not wait for it to settle. Get medical help straight away if it comes with breathlessness, chest pain or feeling acutely unwell.
02 · What to do next
Is this relevant to me, and what should I do?
It is relevant if you have noticed puffiness, a sudden scale jump or changes in how you pass urine since starting or increasing your dose. Start by noting when the change began, which dose you take and when you last injected. That timeline helps a prescriber decide whether the medicine is a plausible factor.
Next, tell your prescriber rather than guessing. Our team runs ongoing monthly reviews, dose-titration and follow-up assessments, with side-effect support included. Existing customers can use that review to raise swelling, and anyone can report a suspected side effect to the MHRA Yellow Card scheme.19

03 · Label versus experience
What the product information says and does not say
The product information lists the side effects that have been established for Mounjaro. Fluid retention and urinary retention are not among them.113
Plainly put, the absence of a listing means the medicine is not an established cause. It does not mean that no individual could ever develop swelling while taking it for some other reason. Any new symptom deserves discussion with your prescriber, and any suspected reaction can be reported through Yellow Card.19
Our related guides cover the same question from other angles: water retention on Mounjaro and GLP-1 medicines and water retention.
04 · Why the scale can mislead
Why the scale can mislead you on treatment
Day-to-day scale movement is not the same as fat gain or fluid retention. Body weight moves with what and when you ate, how much salt was in your meals, how much you drank, your bowel habits and, for many women, the stage of the menstrual cycle. None of that requires a medicine to be involved.
Mounjaro slows how quickly the stomach empties, which is part of how it helps regulate appetite.1
The trial results are averages across many people over a long period. In SURMOUNT-1 of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
Those figures describe a long-term trend, not a straight line. A plateau lasting a short while, or a surprising reading after a salty takeaway, does not mean the treatment has failed. Compare weekly averages rather than single weigh-ins, and weigh at the same time of day under the same conditions.
05 · Swelling: when to act
Swelling, puffiness and when to seek advice
Does Mounjaro cause swelling? Swelling of the ankles, legs or hands is not a listed side effect, so it should be treated as a symptom to investigate rather than something to put down to the injection.13
Swelling can have many causes, and a prescriber or GP is the right person to work out which applies to you. They may ask about other medicines you take, your general health, how much you sit or stand during the day, and how long the swelling has been present.
Contact a clinician promptly if swelling is:
- new, sudden or clearly getting worse
- affecting only one leg, especially if it is painful, warm or red
- accompanied by breathlessness, chest pain or difficulty lying flat: get medical help straight away
- noticeable in the face or lips, or with a rash or trouble swallowing, which may be an allergic reaction: get medical help straight away
Get medical help straight away or call 999 if you have chest pain, severe breathlessness or signs of a serious allergic reaction. Do not wait for your next scheduled review.
Mild puffiness around the injection site is a different matter and is a local skin reaction. If it is troublesome or does not settle, follow the pen's instructions on site rotation and tell your prescriber.
06 · Urinary symptoms
Can Mounjaro cause urinary retention?
Urinary retention is not a listed side effect of Mounjaro in the SmPC or patient leaflet.113 If you cannot pass urine, or can only pass small amounts with discomfort, get medical help straight away.
People sometimes mix up several different urinary experiences. Passing more urine than usual, passing less urine, a burning sensation and difficulty emptying the bladder are all separate problems with different possible causes. Describe exactly what you notice, and when it started, so a clinician can sort through them.
We have a separate guide on urinary tract infections and Mounjaro.
07 · Flushing water safely
What is the fastest way to flush water retention?
There is no safe quick fix to recommend without knowing the cause, and the first step is to find out why you are swollen. If a clinician has assessed you and found nothing concerning, general habits are the sensible starting point.
Broadly, that means keeping to regular meals, being mindful of very salty foods, staying properly hydrated, moving around through the day and avoiding long periods sitting still. These are everyday habits rather than medical treatments, and they will not resolve swelling that has an underlying cause.
What we would steer you away from is self-treating. Over-the-counter and online 'detox' products, diuretic supplements and herbal water pills are not part of Mounjaro treatment, and dosing for such products is not standardised. Mention any supplement or other medicine you take when you speak to your prescriber.
Mounjaro itself is used alongside a reduced-calorie diet and increased physical activity.1 If you are unsure what a sensible plan looks like for you, ask for dietary guidance at your monthly review.
08 · Weight gain on treatment
Why am I gaining weight on Mounjaro?
A rise on the scale does not automatically mean the medicine has stopped working, and it is rarely explained by fluid retention from Mounjaro, which is not listed as a side effect in the UK patient leaflet.1 More often it reflects ordinary daily variation, a change in eating or activity, a different time of day for weighing, or digestive changes.
Ask yourself a few practical questions. Have your portion sizes crept up as your appetite has recovered? Have you missed or delayed doses? Did you recently change dose, or are you coming off a break? Have you started another medicine? Any of these can matter, and your prescriber can help you work through them.
Treatment works alongside diet and physical activity, not instead of them.2 If you have genuinely gained weight over several weeks rather than days, book a review rather than changing your dose yourself.
A steady upward trend should never be dismissed, and if it comes with swelling and breathlessness or difficulty breathing, get medical help straight away. Get it checked.
09 · Speeding up results
How to speed up weight loss on Mounjaro
The licensed route is to use Mounjaro alongside a reduced-calorie diet and increased physical activity, taking each dose as prescribed.1
Rushing that process is not safe, and the 2.5mg starting dose is there to help your body adjust.
Results vary widely. Trial figures are averages over 72 weeks, not predictions for any individual.2 If progress feels slow, your monthly review is the place to look at eating patterns, activity, sleep and whether your current strength is appropriate.
Keep your pen in the fridge at 2–8°C until first use. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.13 Follow the pen's instructions for each injection.
10 · Getting support
Getting support from our team
We are a GPhC-registered UK online pharmacy (GPhC 9012878), and every Mounjaro order follows a free online consultation reviewed by a GPhC-registered prescriber. Repeat orders re-check suitability, and there is no auto-shipping.
If you notice something unexpected, our support team can be reached Monday to Friday by phone on 0333 207 0413, by email at support@everydaymeds.co.uk or through the account portal chat. Aftercare includes ongoing monthly reviews, dose-titration and follow-up assessments, and side-effect support.
The free GLPTrackr companion app can help you log your dose day, symptoms and weekly averages, which makes any conversation with a clinician more useful.
You can report any suspected side effect directly to the Yellow Card scheme, whether or not you are sure the medicine is responsible.19
11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
