Tirzepatide is the active ingredient in Mounjaro, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.¹ It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹ Understanding that structure is the first step to judging what is affordable.

Price is set per pen, and one pen covers four weeks. So a higher strength costs more per pen, but it is the same product.

The pen is more than a commodity. A sensible price covers a genuine product from a supplier you can verify, a clinical assessment, dispensing by a registered pharmacy, temperature-controlled delivery, needles and a sharps bin, and follow-up. Treatment also has a time dimension. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system.¹ There is no way to speed this up, and nothing needs doing while it clears. For budgeting, this means the cost question is not just "what is the first month?" but "how long do I expect to be on treatment, and at what dose?" Per week, the same pens work out at £42.50, £47.50, £61.25, £68.75, £72.50 and £75.00 respectively. Weekly figures are useful when you compare against other weekly injections, but remember you buy by the four-week pen.

Because the discount rules are simple, you can work out your own cost at any stage. Your first order is £40 off the standard price at whichever dose you start on. Every order after that is £25 off. Someone starting at 2.5mg therefore sees the first-order discount on the starter pen, and someone switching from another provider at a higher dose sees it on that higher-dose pen. Who tirzepatide is licensed for, and why it matters for cost

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition.¹ Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes and type 2 diabetes.¹ It must be used alongside a reduced-calorie diet and increased physical activity.¹¹

Why does this belong on a cost page? Because the most expensive purchase is one you should not have made. A prescriber assesses your BMI, medical history and current medicines before deciding whether Mounjaro is suitable for you.¹ If it is not, you are better off knowing before you spend anything. NHS or private: the honest cost picture

Mounjaro is available on the NHS, but access is being phased in over several years.⁶ NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.⁴ The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁶ Full rollout is expected to take years.

On the NHS, the medicine itself is provided through the health service when you are eligible and your area has reached your group. In England, a prescription charge of £9.90 may apply per item for people who pay prescription charges, though exemptions exist. Check the NHS guidance for your own circumstances.⁵

For many people the real question is timing. If you meet the licensed criteria but are not yet in an NHS group, waiting could mean months or years. That is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. The trade-off is straightforward: private treatment costs money now, the NHS route costs little but may not be open to you yet.

These routes are not mutually exclusive. Speak to your GP about NHS eligibility, and if you are not yet reached by the rollout and decide to go private, choose a pharmacy that is GPhC-registered and has a prescriber review your suitability. 08 · Value from the trial results

Value for money: what the trial results say

Judging whether a medicine is affordable means asking what you can reasonably expect from it. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages, and individual results vary.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.² Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.²

As a worked example only: for someone starting at 100 kg, 15% is about 15 kg and 20.9% is about 21 kg. Your own starting weight, diet, activity and response will differ, and trial averages do not predict your own result. We make no promise of any outcome.

The trial figures also come with a condition: treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.² Spending on the pen without changing eating and activity habits will not reproduce trial results. Budget time and effort as well as money.

A fair way to think about value is the cost over the whole first year set against your health goals and your prescriber's view of progress. If the monthly review shows the plan is not working for you, that is useful information, and it is better to know in month three than month twelve. The review is included, so asking the question costs nothing extra.

When you read competitor claims about results, apply the same test. Does the page name the trial, the dose and the timeframe? Does it say that results vary? A page that quotes a single dramatic number with no trial behind it is selling, not informing, and it is not a reliable guide to what your money will buy.

09 · Other options compared

Other options people compare with tirzepatide

Wegovy (semaglutide). Wegovy pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly. The dose steps up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, so the 2.4mg maintenance dose starts from week 17, and 2.4mg is the licensed maintenance dose for weight management.¹⁸ A first Wegovy pen costs £99.99 as a first-order price. Mounjaro works differently: tirzepatide activates two gut-hormone receptors, GIP and GLP-1, whereas semaglutide is a single-action GLP-1 medicine.¹ Check availability at the time of order.

Liraglutide (Saxenda and similar products). Liraglutide is supplied at 6mg per millilitre and is a daily injection, unlike once-weekly Mounjaro and Wegovy. Figures vary, so your prescriber can advise on whether it suits you.

GLP-1 tablets. Wegovy tablets and Foundayo are oral options. No trial has compared Mounjaro directly with either tablet, so results from separate trials should not be set against each other as if they were head to head.

Supplements and "natural" products. Berberine and products marketed as "nature's Ozempic" are low-cost supplements with no clinical oversight, and dosing is not standardised. Cheap is not the same as affordable if the product does not do what the marketing claims. Dramatic weight-loss claims on these products should be treated with caution.

One practical point if you are weighing a switch: do not change medicines on your own to save a month's cost. Moving between GLP-1 medicines involves a different dose ladder and a different set of considerations, so tell your prescriber what you have taken and when. They can advise on where to start on the new medicine and whether a switch is appropriate at all.

10 · Buying safely

How to buy tirzepatide safely and avoid counterfeits

A very low price on tirzepatide should make you more cautious, not less. Counterfeit and unlicensed weight-loss injections have been the subject of MHRA alerts, and the MHRA also runs the Yellow Card scheme for reporting side effects and suspect products.⁷¹⁹ Tirzepatide is a prescription-only medicine, so a legitimate supplier will always carry out a consultation reviewed by a prescriber before supplying it.

Here is a short checklist before you buy from anyone:

• Check the register. A UK online pharmacy should appear on the GPhC public register. Ours is GPhC 9012878.¹²
• Expect a consultation. Treat anyone offering to supply without a consultation or prescription as a red flag.
• Look at the product. Genuine Mounjaro comes as a KwikPen from Eli Lilly's supply chain. We supply through official LillyDirect supply, directly from Eli Lilly's UK distribution.
• Avoid social media sellers and marketplaces. Posts and forum threads offering "compounded" or reconstituted tirzepatide, or vials with no pharmacy behind them, are not a safe route.
• Beware vague pricing. The price shown should be the price paid, with no surprise fees.

If you have already taken a product you are unsure about, speak to a pharmacist or your GP, and report any suspected side effect or suspect product through the Yellow Card scheme.¹⁹

Delivery matters too. Mounjaro pens are stored in the fridge until first use, and ours travel in temperature-controlled, discreet packaging by tracked next-day delivery (DPD), which is free. A cheap listing that ships ambient from overseas is not a saving.

For storage at home, keep unopened pens in the fridge until first use and they keep until the printed expiry date. For anything else about handling a pen, follow the patient information leaflet.¹³ Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or frozen.¹³

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
