04 · Pen, not vial

Pen, not vial: why you should not use a syringe

The UK Mounjaro solution is supplied inside a KwikPen, and the pen should be used only as designed. Withdrawing liquid from the pen with a syringe is not how the product is licensed or designed to be used. Withdrawing liquid from the pen creates unknown dosing accuracy, risks contamination and bypasses the safeguards built into the device.

The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

Is there liquid left in the pen at the end?

It is common to look at a finished pen and wonder whether a usable amount is left. Each pen is designed and licensed to deliver exactly four weekly doses, so the last bit of liquid is not a spare dose. Do not try to top up a short dose from a new pen, and do not try to scrape out the remainder. Dispose of the finished pen in your sharps bin.

The pen also tells you when it has done its job. Because each pen is built for four weekly doses, a pen on a regular weekly schedule is empty about when your next order is due. Milligrams versus millilitres

Many people see a strength in milligrams on the box and then look for a volume to measure. With a pen, you do not need a volume. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1

Why each strength is a separate pen

Because the solution is pre-filled at a fixed concentration for each strength, you cannot turn a lower-strength pen into a higher one. Each stage of the ladder needs the matching pen.

If you miss a dose

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

Changing your injection day

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 People often do this around travel, shift work or holidays. Pick the new day, count the gap from your last injection, and keep to the new day afterwards.

How to inject the solution

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1 Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.1

Is there a best time of day?

There is no "best" time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember, and stick to it. Many people choose a weekend morning so early side effects fall on days off.1

Setting the dose correctly

The most common worry for new users is whether they have set the pen correctly. The rule is simple: turn the dose knob until the dose indicator shows the correct dose in the window. If the window will not reach a full dose, stop; that pen cannot supply a proper injection.

What the first injections feel like

Teeth grinding is not a listed side effect in the UK Mounjaro patient leaflet.13 If you notice it after starting treatment, mention it to your prescriber or dentist, and report it through the Yellow Card scheme if you suspect a link.19

07 · Storage and travel

Storing the pen so the solution stays reliable

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

The practical points follow from that. Put new pens in the main body of the fridge rather than the door or against the back wall, where items can freeze. Keep the cap on between uses. If a pen has frozen, do not try to thaw and use it; contact the pharmacy for advice.

Unopened pens out of the fridge

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Look at the solution through the pen's window before each injection, and compare it with the description in your patient information leaflet.13 If the solution looks cloudy or discoloured, has particles in it, or the pen has been damaged, frozen or above 30°C, do not use it. Faulty, damaged or wrong items should be reported to us within the stated window so we can sort out a replacement or refund. What the solution can realistically deliver

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 As a worked example only, for someone starting at 100 kg, 20.9% is about 21 kg. Trial averages do not predict your own result.

How much can you lose in three months?

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1 In SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12. A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation, and slower early progress does not predict failure.

Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Some people notice a change in the first week; others notice little until a higher dose.1 Both patterns are normal.

What shapes your own outcome

Your dose, how well you tolerate it, your eating pattern, your activity level and how consistently you inject all play a part. The medicine reduces appetite and slows stomach emptying, but it works best when you use that breathing space to build habits that last. 09 · Combination products

Tirzepatide with cyanocobalamin and other add-ins

Cyanocobalamin is a form of vitamin B. The licensed Mounjaro product is tirzepatide in the KwikPen.1 We supply genuine Mounjaro KwikPens through official LillyDirect supply, and we do not offer a pre-mixed tirzepatide and vitamin liquid in place of it.

Spotting unsafe supply

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

NHS or private: how people get the solution

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If a seller offers a mixed vial, ask who made it and whether it needs a prescription. Any decision about adding a vitamin should be made with your prescriber or GP.
How should I store Mounjaro solution?
Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded. Keep the cap on to protect it from light. Unopened pens keep until the printed expiry date; if one has been out of the fridge, check with the pharmacy before using it.
