How GLP-1 medicines work

GLP-1 medicines work by acting on natural gut-hormone receptors that help regulate appetite. GLP-1 is a hormone your gut releases after eating. Medicines in this class are designed to act on the same receptor, which is why they are described as GLP-1 receptor agonists.

Mounjaro takes this a step further. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

That last point matters. The medicine does not burn fat on its own. It changes how hungry you feel and how quickly you feel full, which makes it easier to eat less. The diet and activity changes are still the part that produces weight loss, and the medicine is there to make them more achievable.

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1 That does not automatically make one a better choice for every person. Both are effective licensed treatments, and the right one depends on your health, your tolerance and your prescriber's judgement.

Several GLP-1 based medicines are licensed in the UK, and they differ in how often you take them, how you take them and what they act on. Knowing which is which stops the name "GLP-1 solution" from blurring them together.

Mounjaro (tirzepatide)

Mounjaro is a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Treatment starts at 2.5mg and steps up under prescriber guidance. Wegovy (semaglutide)

Wegovy pens come in 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once-weekly strengths. Titration steps up after at least four weeks, reaching the 2.4mg maintenance dose from week 17.18 The 2.4mg dose is the licensed maintenance dose for weight management. Liraglutide

Liraglutide (Saxenda, and products such as Erly) is supplied at a concentration of 6mg per millilitre. It is a daily injection, unlike once-weekly Mounjaro and Wegovy. For anything beyond that, such as expected results, your prescriber can advise, as figures vary.

Oral options

Wegovy tablets (oral semaglutide) and Foundayo (orforglipron) are tablet-form GLP-1 treatments with their own product information. No trial has compared Mounjaro directly with either tablet, so results from separate trials should not be set against each other as if they were head to head. Choosing between them

The right option depends on your medical history, what you can tolerate, whether you prefer an injection or a tablet, and what your prescriber recommends. The first large trial that directly compares tirzepatide with semaglutide for obesity is SURMOUNT-5.15 Results from separate trials, with different people, durations and doses, should not be set against each other as if they were head to head. A prescriber can talk you through the differences for your situation.

One further point: medicines still in development or without a UK licence, such as retatrutide, are not available through a regulated UK pharmacy. If a website offers them, that is a warning sign.

06 · What results to expect

What results to expect from GLP-1 treatment

SURMOUNT-1 showed large average weight loss on Mounjaro alongside diet and exercise. These are trial averages, and individual results vary, so treat them as a guide to what is possible rather than a promise.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

The trial also reported how many participants reached particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

Putting the percentages in context

Percentages can feel abstract. For someone starting at 100 kg, 20.9% is about 21 kg. Your own result depends on your starting point, your dose, how well you tolerate treatment and how closely you follow the diet and activity advice.

If a seller promises a precise timescale, that tells you more about the seller than the medicine.

What "fastest" really means

People often ask how to lose weight the fastest on a GLP-1 medicine. The honest answer is that the dose schedule is set by your prescriber and is designed around tolerability, so rushing it is not safe or licensed. The dependable levers are the ones in the trial: taking the medicine as prescribed, eating a reduced-calorie diet and being more physically active.

Is it faster than Wegovy?

Only SURMOUNT-5, the direct head-to-head trial, supports a comparison of tirzepatide and semaglutide.15 In SURMOUNT-5, adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy); both are effective, licensed treatments, and individual results vary.15 Your prescriber can talk through which medicine may suit you better.

07 · Who can and cannot take GLP-1

Who can and cannot take GLP-1 medicines

Suitability is decided by a prescriber, not by a headline rule. These medicines are for adults, are used alongside a reduced-calorie diet and more physical activity, and are supplied only after a consultation reviewed by a GPhC-registered prescriber.1 Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

What the patient information leaflet covers

The patient information leaflet is the document that comes in the box, and it is written for you rather than for clinicians. It covers what the medicine is, who should not use it, how to inject it, possible side effects, how to store it and what to do if something goes wrong.13 Read the whole leaflet before your first dose, and keep it. If anything in it is unclear, ask your prescriber or pharmacist.

Things to tell your prescriber

Be open about your full medical history, every medicine you take and any plans such as pregnancy. A good consultation is a conversation about risk, so withholding information only works against you. Contraception

Tirzepatide can reduce absorption of oral contraceptives because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 The MHRA has published guidance on contraception with these medicines.7 If you use oral contraception, discuss this with your prescriber so you can agree a plan, rather than guessing.

Diabetes medicines

If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar. This does not mean the medicines can never be used together. It means your prescriber needs to know and may change the plan. Never adjust diabetes medicines on your own.

Unlicensed and unregulated sources

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

A sensible first step is to ask your GP what is happening locally. NHS guidance on the medicine explains what it is for and how it is used.5 NHS access to Wegovy runs through specialist weight-management services; ask your GP.

The honest summary is balanced. These are effective licensed treatments with a meaningful evidence base, and they also need a prescriber, a gradual dose increase and a sustained commitment to diet and activity. They are not magic, and they are not harmless. Treatment is intended to work alongside a reduced-calorie diet and increased physical activity, not instead of them.

Another truth is that weight can be regained after treatment stops in many people, which is why long-term planning matters. Continuing treatment is a clinical decision made with a prescriber, not a default.

Potential dangers to take seriously

Report any new or lasting symptom to your prescriber, and get medical help straight away if you feel seriously unwell. Any big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops.

Reporting side effects

If you think a medicine has caused a side effect, you can report it through the MHRA Yellow Card scheme.19 Reporting helps regulators spot patterns. A symptom that is not on the full list of side effects in the UK patient leaflet is not a listed side effect, but tell your prescriber about any new or lasting symptom.

A first year of 13 pens costs £2,711.88 if you settle at 5mg, up to £3,599.88 at 15mg. These totals are before discounts. Anything described as a liquid GLP-1 product needs careful scrutiny. Licensed GLP-1 medicines are prescription injections and tablets.
