05 · When not to inject

When not to use a pen

Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days, or above 30°C in use.1

Each of those is a reason to stop and ask, not to push on and hope. A pen that fails any of them is not one you should inject, however convincing the box looks and whatever a code says when you scan it.

If the supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.13 Describe what you see and keep the packaging, because the pharmacy may want the batch number.

Unopened pens belong in the fridge until first use, and they keep until the printed expiry date. For any question about an unopened pen that has been left out, read the patient information leaflet and ask your pharmacy rather than guessing. Storage is a clinical detail with no shortcut answers.

If you feel unwell after an injection and suspect the pen was not genuine, get medical help straight away and tell the clinician exactly what you injected. Serious side effects are uncommon but need urgent attention: seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. You can also report suspected problems through the Yellow Card scheme.19

07 · How to scan a QR code

How to scan a QR code on your phone

Most modern smartphones can scan a QR code with the built-in camera, so you usually do not need an app. Open the camera, hold it steady over the code so the whole square is in view, and wait for a link or banner to appear on screen. Tap it to open.

If nothing happens, check your phone's camera settings, because the code-reading option can be switched off. Many phones also include a scanner in their search or control panel. Good lighting and a flat surface help, and a code printed on a curved bottle or a shiny label may need a slightly different angle.

Some people ask whether they can scan a code from a photo on their own phone, or online without an app. Often yes: many phones read a code from a saved image through the photo viewer, and some websites decode an uploaded image. Be cautious with third-party sites, though, because you would be uploading an image of your medicine packaging to a service you know little about.

Before you tap anything that appears, read the web address shown. It should match the organisation you expect, such as a pharmacy, the manufacturer or a recognised health body. Treat any code that asks you to log in, pay or install something as suspect. This applies to a code on a box just as much as one in an unsolicited message.

Remember that a successful scan only tells you the code is readable. It says nothing about whether the pen beside it is real.

Mounjaro is available on the NHS, but access is being phased in over several years.
