04 · What you are buying

What a Mounjaro prescription actually gets you

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 A prescription from a pharmacy is therefore a clinical decision about which strength you start on and how you move up, not just a purchase. If you are pregnant or trying to conceive, Mounjaro must not be used during pregnancy and should be stopped at least one month before trying to conceive.

That is why treatment does not start at a high dose: your prescriber sets the starting strength and reviews you as you go. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).

Tirzepatide has a half-life of about five days. After a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That matters if you are thinking about stopping or pausing: speak to your prescriber rather than guessing.

Mounjaro works alongside a reduced-calorie diet and increased physical activity, not instead of them.11 A good pharmacy should talk to you about that in the consultation and in follow-up reviews. Who can be prescribed Mounjaro for weight management

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition.1 Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.11 It must be used alongside a reduced-calorie diet and increased physical activity.

Those are the licensed criteria, and they are the starting point rather than the whole story. A prescriber will assess your BMI, medical history and current medicines before deciding whether Mounjaro is suitable for you.11 That is why our consultation asks about your health, not just your weight. Meeting the BMI threshold does not mean you will be approved, and a prescriber can decline an order that does not look suitable. If that happens, you are refunded automatically in full.

  • Weight and BMI: your BMI is calculated from your height and weight. If you sit between 27 and 30, a weight-related condition needs to be present for the licence to apply.
  • Existing conditions: type 2 diabetes is one of the listed qualifying conditions, and Mounjaro is separately licensed to treat type 2 diabetes itself.
  • Current medicines: tell the prescriber about everything you take, because interactions are part of the assessment.
  • Other treatments: if you are already on another weight-loss injection, say so, so the prescriber can advise on any change.

06 · NHS or private

NHS or private: what is the honest picture?

Mounjaro is available on the NHS, but access is being phased in over several years.5 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

What does that mean for you in practice? If you are in one of the earliest NHS groups, your GP is the first person to ask. If you are not yet in a priority group, the NHS route may take considerably longer, and the timing depends on how the rollout reaches your area.

A few common sources of confusion are worth clearing up:

  • NHS prescription charge: if you are in England and obtain a medicine on an NHS prescription, the standard charge is £9.90 per item unless you are exempt.
  • Diabetes: Mounjaro is licensed for type 2 diabetes, and NICE has separate guidance on tirzepatide for diabetes.4 Whether you can have it on the NHS depends on your diabetes care team and local arrangements, so ask them directly.
  • Different thresholds: the licence (BMI 30+, or 27–30 with a related condition) is wider than NHS access. Being licensed-eligible does not mean you are NHS-eligible yet.

Neither route is right for everyone. The NHS route can be cost-free at the point of use but is phased; the private route is available now to those who meet the licence and pass a prescriber's assessment, but you pay for the medicine. Whichever you choose, the same safety principles apply: assessment first, regulated supply, and follow-up.

07 · Realistic expectations

What results can you realistically expect?

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

To make that concrete, here is a worked example. For someone starting at 100 kg, 20.9% is about 21 kg, and 15% is about 15 kg. That is straight arithmetic of the trial percentages, not a forecast for you. Trial averages do not predict your own result.

The trial also reported how many participants reached certain thresholds at 72 weeks:2

Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 The comparison group, lifestyle changes alone, lost 3.1% on average.

What should shape your expectations? Three things, all within what the trial describes. First, the results came with a reduced-calorie diet and increased physical activity. Second, the figures are averages across thousands of different people, so some did better and some did worse. Third, the doses in the table are the ones tested, and your prescriber decides which strength suits you, so a higher dose is not automatically the goal.

It is also sensible to ask what happens if you do not respond as hoped, or if you cannot tolerate a dose. Planning the cost over a year

Over a first year, Mounjaro costs £2,711.88 at standard prices if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. That is 52 weeks, or 13 pens. These totals are before discounts: a new customer's first order is £40 less, and every later order is £25 less.

If you step up after 4 weeks on each of 2.5mg, 5mg, 7.5mg and 10mg, your first four pens cost £959.96 at standard prices. After that, what you pay each month depends on the strength your prescriber settles you on.

10 · Choosing a safe pharmacy

How to choose a safe Mounjaro pharmacy

The safest way to buy Mounjaro online is from a pharmacy listed on the GPhC register, after a consultation with a prescriber. 11 · Stopping and switching

Changing pharmacy, pausing or switching treatment

If you are already on Mounjaro and want to change pharmacy, you can switch to us at any dose.
