04 · How Mounjaro works

How Mounjaro works, whoever supplies it

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly.1 In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 That is the same product whether you buy through iQ Doctor, a high-street pharmacy or us. The medicine does not change with the seller. What changes is the service around it.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

Weekly dosing and the pen

The pen is used once a week, on the same day each week. Each strength is a separate pen, and the prescriber decides when you move up. That is why the pens are not interchangeable, and why you should never guess a dose from the pen alone. How long it stays in your system

Tirzepatide has a half-life of about five days. After a final dose it takes roughly 4–5 weeks to clear the system.1 There is no way to speed this up, and nothing needs doing while it clears. This matters if you are comparing providers, because a missed or delayed order does not instantly end the medicine's effect. It is also a reason to plan repeat orders ahead rather than leave a gap.

Why the supplier still matters

Because the medicine is identical, the real differences are the supply route, the quality of the prescriber review, the price, what is included and the aftercare. 05 · Who can be prescribed

Can iQ Doctor, or any online service, prescribe Mounjaro?

Yes, a GPhC-registered prescriber working for an online service can prescribe Mounjaro privately, as long as you meet the licensed criteria and the consultation shows it is suitable.

Mounjaro is licensed for weight management in adults with a BMI of 30 or above. It is also licensed for a BMI between 27 and 30 with at least one weight-related condition. The examples are high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity.1

A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.11 Treatment is a clinical decision, so it is not assured to be approved just because you apply. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

What a good consultation looks like

A genuine consultation asks about your weight and height, your health conditions, the medicines you take and previous weight-loss attempts. It should leave room for a prescriber to follow up or decline. Be wary of any service offering to supply without a consultation or prescription. Mounjaro is prescription-only, and no legitimate pharmacy can skip that step.

Online prescribers rely on what you tell them, so accurate answers matter, especially about medicines and conditions. It is sensible to let your own GP know you are taking Mounjaro, because they hold your full medical record. Whether a particular provider contacts your GP is that provider's policy, so ask iQ Doctor or any other service before you start and read what they say in writing.

If you do not meet the criteria

If your BMI or health picture falls outside the licence, a prescriber should say so and decline rather than supply anyway. That is the safeguard working as intended. A decline is not a judgement on you, and it should come with a refund of anything paid for the consultation or order.

What results does Mounjaro produce in trials?

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

The trial also reported how many people reached particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

What those percentages mean in kilograms

A worked example helps. For someone starting at 100 kg, 15% is about 15 kg, 19.5% is about 19.5 kg and 20.9% is about 21 kg. Those are straight arithmetic on the trial averages. They are not a forecast for you, and they do not tell you how quickly any loss would happen. Trial averages do not predict your own result.

Why this matters when choosing a provider

Be sceptical of any provider that implies specific outcomes or timescales. The honest position is the trial position: average results achieved alongside lifestyle change, with wide individual variation. A provider's job is to supply genuine medicine, prescribe at an appropriate dose and support you through titration. It is not to promise a number.

The prescriber decides your maintenance dose based on how you respond and tolerate treatment. Some people settle at 5mg, others go higher. 07 · NHS and private

NHS or private: which route suits you?

Mounjaro is available on the NHS, but access is being phased in over several years.4 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Who private treatment suits

Private treatment suits people who meet the licensed criteria but not yet the NHS ones. A BMI of 30, for example, falls inside the licence but below the NICE threshold. It also suits people who cannot wait for their area to reach their group. The trade-off is cost, which is why comparing providers properly matters.

Choosing the private provider

Moving between NHS and private

Some people start privately and later move to NHS care when they qualify, or the reverse. Tell each prescriber what you have been taking and at what dose. Do not restart at a dose you were not prescribed after a gap. Your current prescriber needs the full history to set a safe starting point.

The honest way to compare providers is by the cost of a year, not the price of a first pen. Mounjaro is titrated upwards, so the cost rises with the dose. At our standard prices, the first four pens (at 2.5mg, 5mg, 7.5mg and 10mg) come to £959.96.

A first year (13 pens) costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. Those totals are at standard prices before discounts. A new customer's first order is £40 less, and every later order is £25 less.

What to ask iQ Doctor

When you look at iQ Doctor's pricing, ask whether the monthly fee is the same at every dose, whether there is a separate consultation or review fee, and whether delivery is free. Then multiply by the number of pens you expect to need. A price that looks low at 2.5mg may not look low at maintenance.

Spreading the cost

09 · Checking registration

Is iQ Doctor a registered pharmacy, and how do you check any provider?

You can check any UK online pharmacy yourself on the GPhC public register, and you should do so for iQ Doctor rather than rely on us or on its marketing.12 The register lists registered pharmacies and pharmacy professionals. Search by pharmacy name or registration number. If you cannot find a clear registration number on a provider's site, ask for it.

Some online clinics operate with one registered pharmacy dispensing and another brand fronting the service. That is not necessarily a problem, but you should know who is dispensing. Look for the registered pharmacy's name and number on the site, usually in the footer or on the about page. Ours is GPhC 9012878, and you can verify it on the public register.

Why registration matters for Mounjaro

Mounjaro is a prescription-only medicine. It can only be lawfully supplied following a prescription, and online that means a consultation reviewed by a prescriber. Counterfeit pens have been the subject of MHRA alerts, and fake pens have caused hospitalisations. Only buy from a pharmacy you can verify on the GPhC register.7

Red flags

  • Offering to supply without a consultation or prescription.
  • No pharmacy registration number, or one you cannot find on the register.
  • Social media sellers, or products without packaging and a patient leaflet.
  • Promises of specific results or timescales.

11 · Safety and what to avoid

What to avoid and what to tell your prescriber

The full list of cautions is in the patient information leaflet, and you should read it before your first injection.13

Use it with lifestyle change

Mounjaro must be used alongside a reduced-calorie diet and increased physical activity.1 It is not a replacement for either.

Contraception

Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows.7 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Tell your prescriber if you use the pill, and ask what they advise. Do not rely on the pill alone without that conversation.

Diabetes medicines

If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar. This is something for your prescriber or diabetes team to manage with you. It does not mean the medicines can never be combined with Mounjaro.

Anything else you notice

If you notice a new symptom that is not on the patient leaflet's list of side effects, tell your prescriber and report it through the Yellow Card scheme.19 Do not assume it is related or unrelated. Describe it plainly and let a clinician decide. Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

Storing the pen

Store unopened pens in the fridge until first use. They keep until the printed expiry date. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or frozen.13 For any other storage question, check the leaflet or ask the pharmacy.

Stopping or pausing

Tell your prescriber if you use oral contraceptives, because Mounjaro can reduce their absorption as stomach emptying slows. Switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. If you take insulin or sulfonylureas, doses may need adjusting. Report new symptoms to your prescriber and through the Yellow Card scheme.
Does an iQ Doctor inform your GP?

Yes, Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions. Ask your GP whether you qualify, or consider regulated private treatment if you meet the licensed criteria.
