01 · The honest answer

Can you get Mounjaro for free in the UK?

Yes, but only through the NHS, and only if you meet the access criteria where you live. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶

Full rollout is expected to take years, so many people who meet the licensed criteria are still waiting. That is why some choose regulated private treatment. Free route versus private route

03 · Who qualifies on the NHS

Who qualifies for NHS Mounjaro?

NHS eligibility is narrower than the licence. NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition.⁴ Because the rollout is phased, the first people offered it through primary care were those with the highest need, initially BMI 40+ with four or more qualifying conditions.⁶

Where you live and what your local service has available also matter, so the practical step is to book a GP appointment and ask whether you are eligible now, and when you are likely to be. Bring your BMI, a list of weight-related conditions and your current medicines. Do not expect every practice to give the same answer yet, as the rollout is still in progress.

If you are told you are not eligible yet, ask when your group is expected to be reached, and whether a specialist weight-management service is available locally.

Who can be prescribed privately?

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.¹ It must be used alongside a reduced-calorie diet and increased physical activity.¹¹

05 · Free offers and scams

Free Mounjaro offers: how to spot a scam

Be wary of any website, social media account or message offering Mounjaro free, or offering to supply without a consultation or prescription. Mounjaro is prescription-only, so a legitimate supplier needs a prescriber's review. Counterfeit weight-loss pens are an MHRA concern, and the MHRA publishes counterfeit medicine alerts.⁷

06 · If you cannot afford it

What to do if you cannot afford Mounjaro

Start with the NHS: ask your GP about eligibility and about specialist weight-management services, which is the legitimate free route. NHS access to Wegovy also runs through specialist weight-management services, so ask your GP about that too.

Free prescriptions and prescription charges

An NHS prescription charge of £9.90 applies per item in England unless you are exempt. Exemptions depend on specific rules, and being entitled to free prescriptions does not by itself mean Mounjaro is available on the NHS; the clinical criteria must also be met. Check your own entitlement through official NHS guidance or ask your pharmacist, and keep your exemption evidence to hand.

Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

08 · What to expect from treatment

What you are paying for: dosing and results

Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose rises to 5mg, then can step up by 2.5mg at minimum 4-week intervals to a maximum of 15mg weekly, depending on response and tolerance.¹ Many people settle at 5mg, 10mg or 15mg, and higher is not automatically better.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. For someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages and individual results vary.²
