02 · If a parcel is late

Mounjaro delayed in the post: what to do

Contact the pharmacy as soon as you see tracking slip, and do not inject from a pen that you suspect was mishandled until you have been advised. Work through these steps in order:

  • Check the tracking link in your confirmation message. DPD often gives a revised delivery window or a safe-place option.
  • Check with neighbours, a porch, or a building concierge before assuming it is lost.
  • If the parcel arrives, open it promptly and put the pen in the fridge. Unopened pens are stored in the fridge until first use and keep until the printed expiry date.13
  • If it arrives late and you are worried about how long it was in transit, do not guess. Read the patient information leaflet and contact us with the delivery details.
  • If it has not arrived, tell us. We will look into it with the courier and advise on the next step.

03 · After you stop Mounjaro

What happens after your final dose

After a final dose, tirzepatide clears over about four to five weeks. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

Clearing is gradual rather than a switch. The medicine works by activating the GIP and GLP-1 receptors, which help regulate appetite and slow how quickly the stomach empties.1 As levels fall, that help falls with them. Most people notice that they feel fuller later, think about food more often, and want larger portions. This is the medicine wearing off, not a personal failing.

The weight-loss results in the trials were achieved with treatment ongoing. In SURMOUNT-1, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary.

04 · Post-Mounjaro diet

Post-Mounjaro diet: what to eat afterwards

There is no special post-Mounjaro diet, and the sensible approach is to keep the habits that worked while you were on treatment. Treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit or a sustainable maintenance level, not a perfect food list.

The practical building blocks are the same as during treatment:

  • Protein at every meal. Aim to include a protein source at every meal. That gets harder when appetite is low, and it gets different again when appetite returns, because protein keeps you fuller than most alternatives.
  • Vegetables and fibre. Prioritise vegetables and fibre, and keep drinking regular fluids.
  • Portion awareness. While treatment is active, large portions sit heavily because the stomach empties more slowly. Once the medicine clears, that natural brake is gone, so smaller, slower meals become a choice rather than a reflex.
  • Limit the usual traps. Very fatty, fried or very sugary food and high-calorie drinks are where a deficit leaks fastest.

Be wary of social-media rules about coffee or 'reset' foods. Coffee and tea are fine, but high-calorie coffee drinks are an easy place for calories to creep back.13 Alcohol follows a similar logic: it adds calories that work against your goals, and many people find their desire for alcohol drops on treatment, so watch whether that changes when you stop.

Supplements are not a substitute. A standard daily multivitamin is a reasonable safety net while eating much less, but products sold as 'natural GLP-1 boosters' are not equivalents and cannot replace prescribed treatment. They are low-cost supplements with no clinical oversight, and dosing is not standardised.

05 · Muscle and exercise

Protect muscle and keep moving

Resistance exercise is one of the main things you control after rapid weight loss. Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance. The two levers are adequate protein and resistance exercise two or more times a week.

Exercise also protects against regain if you later stop treatment. That makes the months before stopping the best time to build the habit, not the weeks after. Nothing special is required: the evidence-backed pattern is regular moderate activity, such as brisk walking, plus resistance work for muscle. Start where you are and build.

If you have been relying on low appetite to hold your calories down, practise the alternative skills while the medicine is still working. Plan meals ahead, notice real hunger versus habit, and keep a routine for weekends and social events, when most people drift.

06 · Staying on track after GLP-1

Post GLP-1: stop, step down or stay on?

Stopping completely is only one option, and it is a decision to make with your prescriber rather than alone. Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 Some people stay on treatment for the long term, some change dose, and some stop once they have reached a goal and built habits. Questions worth raising at a review before you stop:

  • What is my goal, and is it a number on the scales or a health marker such as blood pressure or blood sugar?
  • How will I know if I am regaining, and at what point do I want to act?
  • What would I do differently this time if appetite returns strongly?
  • Is there a reason to stop now, such as side effects, cost or a planned procedure, that a dose change could solve instead?

On the NHS, Mounjaro is available but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Our weight loss clinic page explains how our monthly reviews work.

Some people consider switching to another medicine rather than stopping. Wegovy is a licensed alternative with its own dose ladder, and you can see it on the Wegovy pen page. That is a prescriber decision, and we would not recommend switching simply to avoid a gap.

07 · Breaks, restarts and long-term effects

Time off, restarting and long-term side effects

Whether you can take a short break from Mounjaro is a question for your prescriber. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time; if more than 4 days have passed, skip the missed dose and take the next one on your normal day — never take two doses within 3 days of each other. Ask your prescriber before pausing or restarting.

On long-term side effects, the full and current list is in the Summary of Product Characteristics and the patient information leaflet, and we recommend reading both.113 One point worth stating plainly is hydration. Drinking enough matters more on Mounjaro than usual, because reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly, which is the main route to the rare kidney problems reported with this class. Treat inability to keep fluids down as a contact-your-clinical-team issue, not something to push through.

If you notice any new symptom, tell your prescriber, and report suspected side effects through the MHRA Yellow Card scheme.19

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
