What does GLP-1 maintenance actually involve?

GLP-1 maintenance is the stage after your dose has been built up and your weight has levelled off, when the goal changes from losing weight to keeping it off. For most people it means continuing treatment at a dose your prescriber judges sufficient, with regular reviews.

Mounjaro manages weight; it does not cure the biology behind it.¹⁴ That is why our maintenance dosing guide starts with a plan rather than a date to stop.

Maintenance is not a separate product or a special pen. With Mounjaro, the strengths are the same ones you used while losing weight: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg. Your prescriber decides which strength to stay on.

03 · What to do next

What should you do next?

If you are still losing weight, you are probably not in maintenance yet, so keep going with your current dose and reviews. If your weight has levelled off for a while, raise maintenance at your next review. Ask whether your current dose is the lowest that holds your weight, how you would know if regain started, and what your plan is if side effects return.

04 · The maintenance phase

What is the maintenance phase of GLP-1 treatment?

The maintenance phase is the period after the build-up of your dose when your weight has stabilised and treatment continues to hold it.

After that, the right dose is individual. The SmPC sets out the licensed range and your prescriber chooses within it.¹ With Wegovy, the ladder runs 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, with 2.4mg being the licensed maintenance dose.¹⁸

There is no set length for the maintenance phase. It lasts as long as you and your prescriber judge the treatment is still helping and still safe for you.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages, and individual results vary.

05 · Stopping and regain

What happens to your weight if you stop?

In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.¹⁴ Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

This answers a common question: can you lose fat in a maintenance phase? On average, people who continued treatment in that trial did lose further weight. But maintenance is not a promise of more loss. For many people, holding steady is the success.

If you do stop, tirzepatide has a half-life of about five days, so it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.¹ Our guide to what happens after Mounjaro covers the practical side.

06 · Habits that hold weight

Which habits protect your result?

Regular moderate activity plus resistance work is part of the licence rather than an optional extra. Brisk walking counts, and resistance exercise helps protect muscle. Start where you are and build up. On injection days with early side effects, lighter movement still counts.¹ Exercise also protects against regain if you later stop treatment.

Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.² In maintenance, that means keeping routines that suit you for the long term rather than a short-term crash plan. Regular meals, protein with each meal, and planning for social occasions are all sensible habits to build.

07 · Which medicine suits maintenance

Does one GLP-1 suit maintenance better than another?

Tirzepatide has the continued-treatment trial behind it. Wegovy's licence names 2.4mg as its maintenance dose.¹⁸ Both are licensed once-weekly injections, and your prescriber decides which suits you.

Daily liraglutide products such as Saxenda are supplied at 6mg per millilitre. The Wegovy pen page shows the alternative to Mounjaro.

08 · NHS and private routes

Can you get maintenance support on the NHS?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹ Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. Switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.
