04 · Why it needs a prescription

Why tirzepatide needs a prescription

Tirzepatide needs a prescription because it is a prescription-only medicine with an assessment built into its supply. Mounjaro is the brand name for tirzepatide, a once-weekly injectable made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes, and it comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

The prescription exists because a prescriber has to weigh your BMI, medical history and current medicines before deciding whether the treatment suits you.¹ That is not a formality. The patient information leaflet and the product information set out who should and should not use the medicine, and a trained prescriber reads those against your answers.¹³

This is also why "no doctor" is the wrong frame. A site that removes the prescriber removes the safety check. What genuinely helps is removing the friction around the check: the waiting room, the postponed appointment, the postcode lottery. A digital consultation does that while keeping a qualified person responsible for the decision.

In practice, a regulated online pharmacy asks you questions about your height, weight, health conditions and medicines. A GPhC-registered prescriber reviews the answers. 05 · Who is eligible

Who can be prescribed tirzepatide

Adults can be prescribed Mounjaro for weight management if they have a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Weight-related conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹

Meeting those criteria does not mean automatic approval. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you.¹ That is the part a "no doctor" seller skips, and it is the part that protects you.

The consultation questions are there for a reason. Your medical history matters because some conditions and medicines change whether tirzepatide is appropriate. Your current medicines matter too. For example, Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows, and the MHRA has published guidance on contraception with these medicines.⁷ The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.¹ If you take insulin or a sulfonylurea, doses may need adjusting because of the risk of low blood sugar.¹ A prescriber can plan for these. A website with no review cannot.

Honest answers are the whole point. Entering a different weight to pass a check does not change your real clinical picture; it only removes the safeguard designed around it. If you are unsure about your BMI or a condition, say so in the consultation rather than guessing.

06 · Fake pens and red flags

Fake tirzepatide: the real danger of "no doctor" sellers

The main danger of buying with no prescriber is receiving a fake pen. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷

That last point matters most. Insulin can lower blood sugar sharply, and a person who injects it believing it is a weight-loss medicine has no warning of what they are using. The risk is not theoretical, which is why the advice is blunt: only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites.⁷ Genuine treatment always involves a prescription and a clinical consultation.

Red flags are easy to spot once you know them:

• The seller offers to supply without a consultation or prescription.
• The seller cannot be found on the GPhC register, or will not show a registration number.
• The offer arrives by direct message, a social media advert, or a salon or gym.
• The pen has no batch number or expiry date, or the packaging is not cold-chain.
• The seller is vague about where the stock comes from.

07 · Checking your pen

How to check a pen is genuine and still good

A genuine UK Mounjaro pen comes from a registered pharmacy, carries a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been above 30°C or out of the fridge for more than 30 days in use.¹

Run through this short checklist when a pen arrives:

• Is the packaging cold-chain and intact?
• Is there a batch number and an expiry date on the pen and box?
• Is the solution clear and colourless, with no particles?
• Has the pen been kept in the fridge since it arrived?

If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.¹² Our delivery is next-day, tracked and temperature-controlled, so a pen that arrives warm or damaged is something to report straight away. For storage, keep unopened pens in the fridge until first use; they keep until the printed expiry date. 08 · How tirzepatide works

How tirzepatide works and what trials show

Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹

This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).¹ It is a once-weekly injection, and tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.¹

The headline evidence is the SURMOUNT-1 trial of 2,539 adults. People taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.²

To make that concrete with a worked example: for someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. Trial averages do not predict your own result.

The SURMOUNT-5 trial is the one that directly compared tirzepatide with semaglutide in adults with obesity.¹⁵

The trials also show why a prescriber matters. Doses are stepped up over time rather than started high, which is how the six pen strengths (2.5mg to 15mg) are used. Your prescriber decides the pace and the dose that suits you, and reviews you as you go.

None of this requires a surgery visit. It does require someone qualified to look at your history first, which is the step a "no doctor" seller removes.

09 · NHS versus private

NHS or private: the honest picture

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.⁴ The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Many people fall outside the earliest NHS groups, or do not want to wait, and they see a route that skips the queue. The legal route that skips the queue is regulated private prescribing, not an unregulated seller.

The two routes compare like this:

Check the NHS guidance and talk to your GP if you think you may qualify, because the NHS route may suit you if you fit the current groups.⁵ If you do not, or do not want to wait, private treatment through a verified pharmacy is the lawful alternative. Planning the cost over a year

A first year at standard prices costs £2,399.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,279.88 if you step up through every dose to 15mg. That is 52 weeks, or 13 pens, and it is before discounts: a new customer's first order is £40 less, and every later order is £25 less.

11 · Tablets and alternatives

Tablets, semaglutide and "poor man's Ozempic"

Several alternatives come up alongside this question, and each one still needs the right route. Oral options exist, such as Wegovy tablets (oral semaglutide) and Foundayo (orforglipron) film-coated tablets, and both have product information on the electronic medicines compendium.²⁰²¹ Their trials are OASIS 4 and ATTAIN-1.²²²³

Is there a tablet equivalent to Mounjaro? Tirzepatide itself is an injection, and Wegovy tablets and Foundayo are different medicines with different active ingredients. Whether one suits you is a prescriber's call, not something to decide from a headline.

Wegovy (semaglutide) is an injectable with its own dose ladder: 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least 4 weeks and reaching the 2.4mg maintenance dose from week 17.¹⁸ The same rule applies: it is a prescription medicine and should be supplied only after a consultation. Liraglutide, as Saxenda and similar products, is a daily injection supplied at 6mg per millilitre, unlike once-weekly Mounjaro and Wegovy. Figures for other treatments vary, so your prescriber can advise.

Can you buy Ozempic online legally? Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and a regulated UK pharmacy will not supply Ozempic for weight loss. The same warning about fakes and unregulated sellers applies, and the "cheapest way to get semaglutide" is a regulated route you can verify, not an unverified seller.

The phrase "poor man's Ozempic" is usually attached to low-cost supplements such as berberine. They are low-cost supplements with no clinical oversight, they are not the same medicine as tirzepatide or semaglutide, and their dosing is not standardised.

As for new weight-loss drugs coming in 2026, treat any list with caution. Nothing in development can be bought legally with no prescriber. If a seller offers one as available now with no consultation, treat that as a red flag.
